Alveolar-capillary Reserve

Alveolar-capillary reserve is the capacity of the lung’s alveoli and pulmonary capillary network to increase gas exchange when oxygen demand rises, particularly during exercise. As pulmonary blood flow increases, previously underperfused capillaries are recruited and existing vessels distend, expanding the exchange surface and helping maintain efficient oxygen transfer despite a shorter red-cell transit time. This reserve supports increases in cardiac output without major impairment of arterial oxygenation and provides a functional measure of pulmonary health. Assessing it can help characterize limitations caused by pulmonary vascular disease, interstitial lung disease, emphysema, or other conditions that reduce diffusion capacity.
