Endovascular Perforation

Endovascular perforation is a breach in a blood vessel wall that occurs during or after an endovascular procedure, allowing blood or contrast material to escape into surrounding tissue. It can result from guidewire or catheter manipulation, balloon dilation, stent deployment, or excessive intravascular pressure, with the injury ranging from a small contained tear to uncontrolled hemorrhage. Prompt angiographic detection is essential because treatment may involve prolonged balloon inflation, covered-stent placement, embolization, or surgical repair, depending on the vessel and severity. Understanding this complication supports safer procedural planning, rapid intervention, and improved outcomes in vascular medicine.

The video demonstrates the induction of subarachnoid hemorrhage via endovascular perforation in a mouse model. An incision is made in the neck region of the anesthetized mouse to access the common carotid artery and its bifurcation. A filament is inserted through the arteries to perforate an intracranial artery branch, inducing subarachnoid hemorrhage.

Histological and biochemical modifications after aneurysm endograft exclusion are still unclear. We describe a new model of endograft implantation on a murine aneurysm. Through thrombus analysis with and without persistant circulating blood stream interface, and new endovascular biomaterials evaluation, this model will help to better understand endovascular aneurysm exclusion pathobiology.
