Transplant Vasculopathy

Transplant vasculopathy is a progressive vascular complication in transplanted organs, marked by narrowing of the graft’s arteries and a major contribution to late graft dysfunction and failure. It develops when immune-mediated endothelial injury triggers inflammation, smooth muscle proliferation, and accumulation of extracellular matrix within the vessel wall, producing diffuse intimal thickening and reduced blood flow. Because the process may advance without distinctive symptoms, clinicians use angiography, intravascular imaging, biopsy, and functional monitoring to assess graft health. Understanding transplant vasculopathy supports risk assessment, immunosuppressive management, surveillance strategies, and research into therapies that preserve long-term organ function.
