Duodenum-preserving Surgery

Duodenum-preserving surgery is a pancreatic surgical approach that removes diseased tissue from the pancreatic head while retaining the duodenum, the first segment of the small intestine, to limit disruption of gastrointestinal anatomy. The operation involves carefully dissecting the pancreatic head and its blood supply, then reconstructing pancreatic drainage while maintaining duodenal perfusion and, when feasible, preserving the nearby bile duct. In cancer research, this technique supports studies of organ-sparing resection, postoperative recovery, and the balance between functional preservation and complete tumor clearance; its suitability depends on tumor location, vascular involvement, and oncologic safety. Comparative studies help define its role alongside standard pancreaticoduodenectomy.

This protocol presents robotic duodenum-preserving total pancreatic head resection (R-DPPHRt) for intraductal papillary mucinous neoplasms (IPMNs). A video demonstrates the technique in a patient with main duct-IPMN, highlighting its feasibility as a minimally invasive, organ-preserving alternative to pancreaticoduodenectomy.

In early gastric cancer, the aim of surgery is to precisely remove the distal stomach including the primary tumor. To do this, accurate localization of the tumor is crucial, especially in totally laparoscopic surgery. This protocol describes a procedure for intraoperative gastroscopy in totally laparoscopic subtotal gastrectomy.

Innovative Strategies for Organ Preservation in Heart Transplantation: Uniform Cooling Preservation and Ex-situ Normothermic Perfusion

Donor heart preservation is a major determinant of success in heart transplantation. Technological advances have led to optimizing the conditions of preservation, along with a more accurate assessment of the donor heart before transplantation. We herein describe two innovative technologies currently applied for cardiac preservation: uniform cooling preservation and normothermic ex-situ heart perfusion.

University of Notre Dame, IN The Guide for the Care and Use of Laboratory Animals1 dictates that rodent survival surgery be performed aseptically. Aseptic technique utilizes specific practices that minimize the contamination of the surgical site, including patient preparation, surgeon preparation, sterilization of instruments and other supplies, and the use of a clean and controlled environment.
