Barrel Chest

Barrel chest is a physical examination finding in which the chest appears rounded, with an increased anteroposterior diameter relative to its width. It most often reflects chronic lung hyperinflation, as trapped air and altered respiratory mechanics keep the ribs more expanded; emphysema is a classic associated condition, although the appearance is not specific to one diagnosis. In clinical practice, recognizing barrel chest helps clinicians assess respiratory disease alongside breath sounds, work of breathing, patient history, spirometry, and imaging. The finding can support evaluation of chronic obstructive pulmonary disease, but it should not be used alone to confirm disease or determine severity.

Overview of Flail Chest Flail chest is a severe and potentially life-threatening condition characterized by the fracture of three or more adjacent ribs in multiple places. It is most commonly caused by direct impacts and trauma, such as motor vehicle accidents or injuries from a steering wheel impact. It can also occur due to falls in elderly individuals with osteoporosis, or assaults involving sharp objects. Pathophysiology The pathophysiology of flail chest is complex, involving fractures of...

Managing flail chest, a condition characterized by a segment of the chest wall moving independently from the rest of the thoracic cage, requires a comprehensive approach. It includes a thorough assessment of the patient's condition, a diagnostic evaluation to determine the extent of the injury, and the implementation of appropriate medical interventions tailored to the individual's needs. Assessment: 1. Clinical Evaluation: History: • Nurses obtain a detailed history of the traumatic event,...

Chest Physiotherapy (CPT) is a therapeutic technique used in respiratory care to improve ventilation, clear bronchial secretions, and enhance the efficiency of respiratory muscles. This therapy includes three primary procedures: postural drainage, percussion, and vibration. It can be performed on spontaneously breathing patients and those who are intubated and mechanically ventilated. Purpose CPT is primarily used for patients with excessive bronchial secretions who have difficulty clearing...

Source: Blanc, J., et al. Functional Magnetic Resonance Spectroscopy at 7 T in the Rat Barrel Cortex During Whisker Activation. J. Vis. Exp. (2019)This video demonstrates the method of analyzing metabolic changes in the rat barrel cortex using functional magnetic resonance spectroscopy during whisker stimulation. Baseline metabolite spectra are compared with stimulated spectra to detect lactate increase in the barrel cortex, confirming successful neuronal activation.

We demonstrate the fabrication, calibration and properties of two types of ion-selective microelectrodes (double-barreled and concentric) for measurement of ion concentrations in brain tissue. These are then used in the mouse hippocampal slice preparation to show that excitatory activity changes both extracellular potassium and sodium concentrations.
