Abdominal Gunshot Injury

Abdominal gunshot injury is a penetrating traumatic injury caused by a projectile entering the abdomen, potentially damaging solid organs, hollow viscera, blood vessels, and the abdominal wall. The projectile transfers kinetic energy along its path, producing tissue disruption, hemorrhage, contamination from intestinal contents, and sometimes secondary injury from fragments or cavitation. Clinical assessment prioritizes airway, breathing, circulation, and rapid identification of shock, while focused ultrasonography, computed tomography, and serial examinations help evaluate internal damage in stable patients. Prompt resuscitation and appropriate operative or nonoperative management can limit blood loss, infection, organ failure, and death in emergency and surgical care.

Understanding the cellular and molecular mechanisms of re-endothelialization following arterial denudation injury is of paramount importance in preventing thrombosis and restenosis of arteries. Here we describe a protocol for reproducible arterial denudation injury of the infrarenal abdominal aorta. The procedure was developed to investigate the underlying mechanisms that regulate endothelial regeneration using mouse models.

Abdominal Exam IV: Acute Abdominal Pain Assessment

2023

Source: Joseph Donroe, MD, Internal Medicine and Pediatrics, Yale School of Medicine, New Haven, CT Abdominal pain is a frequent presenting concern in both the emergency department and the office setting. Acute abdominal pain is defined as pain lasting less than seven days, while an acute abdomen refers to the abrupt onset of severe abdominal pain with features suggesting a surgically intervenable process. The differential diagnosis of acute abdominal pain is broad; thus, clinicians must have a...

Exposing Infrarenal Abdominal Aorta in Porcine Model: A Surgical Procedure to Access Porcine Infrarenal Abdominal Aorta for Further Studies

This video demonstrates the surgical procedure to locate and dissect the infrarenal aorta from the surrounding tissues for accessing it. The exposed aorta can be used for further studies.

Abdominal Exam II: Percussion

Source: Alexander Goldfarb, MD, Assistant Professor of Medicine, Beth Israel Deaconess Medical Center, MA Medical percussion is based on the difference in pitch between the sounds elicited by tapping on the body wall. The auditory response to tapping depends on the ease with which the body wall vibrates, and is influenced by underlying organs, strength of the stroke, and the state of the body wall. There are three main medical percussion sounds: resonance (heard over lungs), tympany (heard over...

Abdominal Exam III: Palpation

Source: Alexander Goldfarb, MD, Assistant Professor of Medicine, Beth Israel Deaconess Medical Center, MA Abdominal palpation, if performed correctly, allows for examination of the large and relatively superficial organs; for a skilled examiner, it allows for assessment of the smaller and deeper structures as well. The amount of information that can be obtained by palpation of the abdominal area also depends on the anatomical characteristics of the patient. For example, obesity might make...
