Critical Limb Ischemia

Critical limb ischemia is a severe manifestation of peripheral artery disease in which inadequate blood flow threatens the viability of a limb, commonly causing ischemic rest pain, nonhealing ulcers, or gangrene. Atherosclerotic narrowing or arterial occlusion limits oxygen delivery to distal tissues, producing hypoxia, impaired wound healing, and progressive tissue damage. Clinical evaluation uses symptoms, vascular examination, ankle-brachial or toe pressures, Doppler studies, and imaging to assess perfusion and guide treatment. Revascularization through endovascular or surgical procedures, combined with wound care, infection management, and cardiovascular risk reduction, can relieve symptoms, promote healing, and reduce the risk of amputation.

Lower limb amputation may occur even after angioplasty of obstructed vessels in Critical Limb Ischemia (CLI). Mononuclear Progenitor Cells (MPCs) reflect vascular repair. The present protocol describes the quantification of MPCs from circulation close to angioplasty, and its relationship with endothelial dysfunction and prediction of lower limb amputation.

Preclinical Model of Hind Limb Ischemia in Diabetic Rabbits

We describe a surgical procedure used to induce peripheral ischemia in rabbits with hyperlipidemia and diabetes. This surgery acts as a preclinical model for conditions experienced in peripheral artery disease in patients. Angiography is also described as a means to measure the extent of introduced ischemia and recovery of perfusion.
