Abstract
心停止は公衆衛生上の大きな負担となります。急性腎障害(AKI)は、心肺蘇生が成功した後の自発循環(ROSC)の復帰後の心停止の生存者における有害マーカーです。逆に、AKIからの腎機能の回復は、良好な神経学的転帰と退院の予測因子です。しかし、ROSC後の心停止による腎障害を予防するための効果的な介入は不足しており、追加の治療戦略が必要であることが示唆されている。腎低灌流と再灌流は、心停止後にAKIを引き起こす2つの病態生理学的メカニズムです。両方の腎臓の虚血再灌流誘発AKI(IR-AKI)の動物モデルは、臨床現場でROSCに続くAKI患者に匹敵します。しかし、両腎臓のIR-AKIは、死亡率が高く、腎臓の損傷に大きなばらつきがあり、分析に影響を与える可能性があるため、解析が技術的に困難である。軽量マウスを選択し、イソフルランによる全身麻酔下に配置し、背外側アプローチで手術を行い、手術中に体温を維持することで、組織の損傷を軽減し、再現性のある急性腎IR-AKI研究プロトコルを確立しました。
Introduction
米国では年間80,000回以上、心停止が発生しています1,2。心停止の死亡率は非常に高い3,4,5,6です。AKIは、ROSC7、8、9、10、11、12、13後の心停止患者における高い死亡率と神経学的転帰の不良に関連する主要な危険因子です。AKIからの回復は、良好な神経学的転帰と退院の優れた予測因子です14,15,16。しかし、IR-AKIの効果的な治療法はまだ不足しています15、16、17、18、19。この疾患の臨床転帰をさらに改善するには、追加の治療戦略が必要です。

Protocol
すべての動物実験は、米国国立衛生研究所が発行した実験 動物のケアと使用に関するガイド(NIH出版物番号85-23、1996年改訂)に従って実施されました。研究プロトコルは、Fu-Jen Catholic UniversityのInstitutional Animal Care and Use Committeeのガイドラインに従って承認されました。このプロトコルで使用されるすべての材料と機器の詳細については、 材料表 を参照してください。
1.マウスの準備
- 体重21〜23gの8週齢のC57BL/6雄マウスを選択します。
- 制御された温度(21〜2°C)で12時間の明暗サイクルでマウスを飼育±維持し、餌、標準的なマウスの餌ペレット、水道水を自由に利用できるようにします。
2.麻酔
- サージカルマスクと滅菌手袋を着用してください。
- 誘導チャンバー内で、2%イソフルランと酸素を1L/minで混合した麻酔下に置きます。
- ペダル反射による麻酔のレベルを評価します。
注意: ペダル反射は、つま先がしっかりとつままれたときに後ろ足を引っ込めることです。ペダル反射が消えると麻酔は完了です。 - 麻酔が完了したら、各マウスを電気毛布で手術台にうつ伏せの位置に移動させ、体温を維持します。手術前に体温を安定させ、直腸温度プローブで監視します。乾燥を防ぐために両目に眼軟膏を塗ります。
- マウスの足をボードにテープで固定します。
- マウスの顔にマスクを装着し、1%イソフルランと1L/minの酸素を一定に供給します。
- ペダル反射による麻酔のレベルを定期的に評価し、手術中にそれに応じて麻酔薬の投与を調整します。
3.両側腎IR-AKI手術
- 背中を触って、マウスの腰椎を手動で見つけます。脊椎に沿って頭側に移動し、マウスの最後の肋骨の両側の下にある肋骨角を探します。
- 肋骨角部の両側に脱毛ローションを約30秒間塗布した後、生理食塩水で毛皮を取り除きます。
- 剃った皮膚をベタジン溶液3回と綿球を使用した75%アルコールで消毒します。
注:手術中、手術のために無菌フィールドを維持することが鍵となります。外科用ドレープを適用し、滅菌器具を使用します。 - 先端の細い鉗子を使用して、左肋骨角より下の皮膚を静かに持ち上げ、次にハサミを使用して、左脇腹の腰椎正中線から皮膚の緊張線に沿って1cmの斜め背外側切開を作成します。ハサミを使って左脇腹の筋肉壁を切除し、左腎臓を視覚化します。
- 前述の外科的処置を繰り返して、右腎臓を視覚化します。滅菌綿棒で施術中に生成された少量の血液を取り除きます。
- 鉗子で左腎臓を周囲の組織から慎重に押して分離します。左腎臓が露出した後の腎椎弓根を特定します。
注意: 副腎や周囲の血管を傷つけないように注意してください。 - 左腎椎弓根を微小血管クリップで25分間クランプします。腎臓の色がピンクから濃い赤に目に見える変化で虚血を確認します。
- 左腎椎弓根クランプ中の乾燥を避けるために、クランプされた腎臓を滅菌生理食塩水湿った綿球で覆います。.
- 前述の外科的処置を繰り返して、右腎椎弓根を微小血管クリップで25分間クランプします。

- 左微小血管クリップを開いて、左腎臓の再灌流を開始します。左腎臓の濃い赤からピンクへの目に見える色の変化によって再灌流を確認します。
- 右の微小血管クリップを開いて、右腎臓の再灌流を開始します。
- 腎臓の色の変化が確認されたら、腎臓を腹腔に戻します。
- 腹腔と皮膚を6-0の吸収性縫合糸材料で閉じます。
- 綿球を使用して、ベタジン溶液と75%アルコールで傷口をこすり洗いして消毒します。
- 動物が自由に動き始め、餌を食べ始めるまで、動物を注意深く観察します。
注:動物が胸骨臥位を維持するのに十分な意識を取り戻すまで、動物に細心の注意を払ってください。 - 術後の痛みを防ぐために、カルプロフェン(0.2 mLで5 mg / kg、皮下投与)を2〜3日間投与します。

Representative Results
両側腎IR-AKI手術の質は、さらなる顕微鏡的または分子的分析の前に評価されるべきである。手術中、腎椎弓根を微小血管クリップで固定した直後に腎臓の色がピンク色から暗赤色に変化したかどうかを確認することで、腎虚血を確認する必要があります(図1)。手術後、IR-AKI手術によって引き起こされた腎臓の損傷は、生化学的分析のために顎下採血を通じて数マイクロリットルの血清でさらに検証することができ、その結果、ベースラインからの血中尿素窒素およびクレアチニンのレベルの上昇が示されました(図2)。

Discussion
提案された両側IR-AKIプロトコルは、両方の腎臓の低灌流および再灌流損傷のメカニズムを調査するのに適しています。プロトコルは、軽量マウス、イソフルランによる全身麻酔、手術への背側アプローチ、および手術中の体温維持が、関連する技術的困難を軽減し、手術の期間を短縮し、急性両側腎IR-AKI研究の手順の一貫性を高めることを示唆しています。

全身麻酔はIR-AKI試験の結果に影響を与えます。麻酔が長引くと、手術中の動物の損失が増加します33。文献では、中枢神経系を抑制する長時間作用型バルビツール酸塩であるフェノバルビタールナトリウムが、IR-AKI手術のために皮下投与されています26,33,35。フェノバルビタールは5分後に開始し、少なくとも15分で外科的麻酔を達成するのに役立ちます45。したがって、フェノバルビタールは、麻酔の延長(>60 mg / kg)と手術中の動物の損失を避けるために、熟練した外科医によってのみ投与されるべきです33。対照的に、この研究のイソフルランの使用は、不燃性の吸入麻酔薬であり、7〜10分で外科的麻酔を達成し、吸入を中止してから15分で効果が停止する急速な発症を誘発しました46。イソフルランと酸素の送達は、手術中にオペレーターが即座に開始、維持、停止することが容易であり、腎臓IR-AKI手術に推奨されます。
最後に、腎椎弓根に近づく方法は、IR-AKI手術の質に影響を与える可能性があります。いくつかのIR-AKI研究では、腹腔が開かれ、腹膜と腸が腎臓にアクセスするために脇に押し出された正中開腹術を使用して腎椎弓根を調査しました。ただし、そうすることで、体液と熱の損失、手術関連の外傷、および手術期間が長くなる可能性があります32,35。したがって、このプロトコルは、IR-AKI研究の背外側アプローチを提案しています 体温を維持し、手術関連の損傷を最小限に抑えるために、脇腹と後腹膜から腎臓を露出させ、その後、手術状態を改善します 研究の一貫性。
