Pharmacotherapy Integration

Pharmacotherapy integration is the coordinated use of medication therapy within a patient’s broader clinical care plan, linking drug treatment to diagnosis, comorbidities, patient goals, and other interventions. It works through systematic assessment of pharmacokinetics, pharmacodynamics, drug interactions, adherence, therapeutic response, and adverse effects, allowing clinicians to select, initiate, monitor, and adjust treatment as the patient’s condition changes. In medicine, this approach supports individualized care for acute and chronic disease, helps balance benefits against risks, and improves continuity across healthcare settings. By connecting prescribing decisions with clinical monitoring and shared decision-making, pharmacotherapy integration can reduce preventable medication problems and strengthen treatment safety and effectiveness.
