Transkraniell magnetisk stimulering (TMS) er en non-invasiv verktøy for å få innsikt i fysiologi og funksjon av det menneskelige nervesystemet. Her presenterer vi vår TMS teknikker for å studere cortical excitability av overekstremitet og lumbar muskulaturen.
Transkraniell magnetisk stimulering (TMS) har vært i bruk i mer enn 20 år 1, og har vokst eksponentielt i popularitet det siste tiåret. Mens bruk av TMS har utvidet til studiet av mange systemer og prosesser i denne perioden, innebærer det opprinnelige programmet, og kanskje en av de vanligste bruksområdene for TMS studere fysiologi, plastisitet og funksjon av det menneskelige nevromuskulære systemet. Enkelt puls TMS brukt til motor cortex excites pyramidale nevroner transsynaptically 2 (figur 1) og resulterer i en målbar elektromyografisk respons som kan brukes til å studere og evaluere integriteten og excitability av corticospinal tarmkanalen hos mennesker 3. I tillegg gir de siste fremskritt innen magnetisk stimulering nå for oppdeling av kortikale versus spinal eksitabilitet 4,5. For eksempel kan parvise puls TMS brukes til å vurdere intracortical facilitatory og hemmende egenskaper ved å kombinere en tilstanding stimulus og en test stimulans på ulike interstimulus intervaller 3,4,6-8. I denne videoen artikkelen vil vi demonstrere metodiske og tekniske aspektene av disse teknikkene. Spesielt vil vi demonstrere single-puls og parvise puls TMS teknikker som brukes til flexor carpi radialis (FCR) muskelen samt erector spinae (ES) muskulatur. Våre laboratoriestudier FCR muskelen som det er av interesse for vår forskning på effekten av håndledd-hand støpt immobilisering om redusert muskel ytelse 6,9, og vi studere ES musklene pga disse musklene klinisk relevans som gjelder ryggsmerter 8. Med dette sagt, bør vi være oppmerksom på at TMS har blitt brukt til å studere mange musklene i hånd, arm og ben, og bør iterate at våre demonstrasjoner i FCR og ES muskelgrupper er bare utvalgte eksempler på TMS brukes til å studere menneskets nevromuskulære system.
Det overordnede målet med denne artikkelen er å gi forskere og klinikere en visuell beskrivelse av våre laboratorier bruk av transkranial magnetisk stimulering. Men i tillegg til å gi en visualisering av disse eksperimentene, nedenfor diskuterer vi grunnleggende spørsmål å vurdere når du utfører TMS på denne måten, gir en kort oversikt over fysiologi av TMS reaksjoner, og også diskutere vår bruk av TMS med hensyn til bruken av andre.
