Deze studie is gericht op het ontwikkelen van een standaardprotocol van intra-operatieve neuraal controle van schildklier chirurgie in een varkens model. Hier presenteren we een protocol om aan te tonen van de narcose, om te vergelijken verschillende soorten elektroden, en te onderzoeken het elektrofysiologische kenmerken van de normale en gewonden terugkerende laryngeal zenuwen.
Intraoperatieve letsel aan de terugkerende laryngeal zenuw (RLN) kan leiden tot vocale snoer verlamming, die interfereert met spraak en potentieel kan interfereren met de ademhaling. In de afgelopen jaren is intraoperatieve neuraal controle (IONM) sterk aangepast als een aanvulling techniek te lokaliseren van de RLN, sporen RLN letsel en voorspellen van vocale snoer functie tijdens de operaties. Vele studies hebben ook dierlijke modellen gebruikt te onderzoeken van nieuwe toepassingen van IONM technologie en betrouwbare strategieën ter voorkoming van intraoperatieve RLN letsel te ontwikkelen. Het doel van dit artikel is om een standaardprotocol voor het gebruik van een varkens model in IONM onderzoek. Het artikel toont aan dat de procedures voor het inducerende narcose, back tracheale intubatie en experimenteel design te onderzoeken het elektrofysiologische kenmerken van RLN verwondingen. Toepassingen van dit protocol kunnen verbeteren totale werkzaamheid bij de uitvoering van het 3R-principe (vervanging, vermindering en verfijning) in varkens IONM studies.
Hoewel trachtte nu een veelgebruikte procedure wereldwijd is, is postoperatieve stem disfunctie nog steeds gebruikelijk. Intraoperatieve letsel aan de terugkerende laryngeal zenuw (RLN) kan leiden tot vocale snoer verlamming, die interfereert met spraak en potentieel kan interfereren met de ademhaling. Letsel aan de externe tak van de superieure laryngeal zenuw kan bovendien een belangrijke stem wijzigen door op het gebied van pitch en vocale projectie.
Intraoperatieve neuraal controle (IONM) tijdens de operaties van de schildklier heeft brede populariteit als een aanvulling techniek voor toewijzing en de bevestiging van de RLN, de nervus vagus (VN) en de externe tak van de superieure laryngeal zenuw (EBSLN) behaald. Omdat IONM nuttig is voor bevestiging en het ophelderen van de mechanismen van RLN letsel en voor het opsporen van anatomische variaties in de RLN, kan het worden gebruikt om vocaal snoer functie na trachtte te voorspellen. Daarom, IONM voegt een nieuwe functionele dynamiek in de chirurgie van de schildklier en machtigt chirurgen met informatie die niet kunnen worden verkregen door rechtstreekse visualisatie alleen1,2,3,4,5 , 6 , 7 , 8 , 9 , 10.
Onlangs, vele prospectieve studies varkens modellen hebben gebruikt voor het optimaliseren van het gebruik van IONM technologie en om betrouwbare strategieën ter voorkoming van intraoperatieve RLN letsel11,12,13,14 ,15,16,17,18,19,20. Varkens modellen hebben ook gebruikt om te beoefenaars voorzien van essentiële onderwijs en opleiding in klinische toepassingen van IONM.
Dus, de combinatie van diermodellen en IONM technologie is een waardevol instrument voor de studie van de pathofysiologie van RLN letsel21. Het doel van dit artikel was het gebruik van een varkens model in IONM onderzoek te demonstreren. In het bijzonder het artikel laat zien hoe induceren van narcose, uitvoeren van tracheale intubatie en experimenten voor het onderzoeken van het elektrofysiologische kenmerken van verschillende typen van RLN letsel opzetten.
