Tweedekker videoradiografie kan schouderkinematica met een hoge mate van nauwkeurigheid kwantificeren. Het hierin beschreven protocol is speciaal ontworpen om de scapula, opperarmbeen en de ribben te volgen tijdens vlakke humerusverhoging en schetst de procedures voor het verzamelen, verwerken en analyseren van gegevens. Unieke overwegingen voor het verzamelen van gegevens worden ook beschreven.
De schouder is een van de meest complexe gewrichtssystemen van het menselijk lichaam, waarbij beweging plaatsvindt door de gecoördineerde acties van vier individuele gewrichten, meerdere ligamenten en ongeveer 20 spieren. Helaas komen schouderpathologieën (bijv. Rotator cuff-scheuren, gewrichtsdislocaties, artritis) vaak voor, wat resulteert in aanzienlijke pijn, invaliditeit en verminderde kwaliteit van leven. De specifieke etiologie voor veel van deze pathologische aandoeningen is niet volledig begrepen, maar het is algemeen aanvaard dat schouderpathologie vaak wordt geassocieerd met veranderde gewrichtsbeweging. Helaas is het meten van schouderbeweging met de nodige nauwkeurigheid om op beweging gebaseerde hypothesen te onderzoeken niet triviaal. Radiografische bewegingsmeettechnieken hebben echter de vooruitgang opgeleverd die nodig is om op beweging gebaseerde hypothesen te onderzoeken en een mechanistisch begrip van de schouderfunctie te bieden. Het doel van dit artikel is dus om de benaderingen te beschrijven voor het meten van schouderbeweging met behulp van een aangepast dubbelplanair videoradiografiesysteem. De specifieke doelstellingen van dit artikel zijn het beschrijven van de protocollen voor het verkrijgen van biplanaire videoradiografische beelden van het schoudercomplex, het verkrijgen van CT-scans, het ontwikkelen van 3D-botmodellen, het lokaliseren van anatomische oriëntatiepunten, het volgen van de positie en oriëntatie van het opperarmbeen, het schouderblad en de romp van de biplanaire radiografische beelden en het berekenen van de kinematische uitkomstmaten. Bovendien zal het artikel speciale overwegingen beschrijven die uniek zijn voor de schouder bij het meten van gewrichtskinematica met behulp van deze aanpak.
De schouder is een van de meest complexe gewrichtssystemen van het menselijk lichaam, waarbij beweging plaatsvindt door de gecoördineerde acties van vier individuele gewrichten, meerdere ligamenten en ongeveer 20 spieren. De schouder heeft ook het grootste bewegingsbereik van de belangrijkste gewrichten van het lichaam en wordt vaak beschreven als een compromis tussen mobiliteit en stabiliteit. Helaas komen schouderpathologieën vaak voor, wat resulteert in aanzienlijke pijn, invaliditeit en verminderde kwaliteit van leven. Rotator cuff-scheuren treffen bijvoorbeeld ongeveer 40% van de bevolking ouder dan 601,2,3, met ongeveer 250.000 rotator cuff-reparaties die jaarlijks worden uitgevoerd4, en een geschatte economische last van $ 3-5 miljard per jaar in de Verenigde Staten5. Bovendien komen schouderdislocaties vaak voor en worden ze vaak geassocieerd met chronische disfunctie6. Ten slotte is glenohumerale gewrichtsartrose (OA) een ander belangrijk klinisch probleem met betrekking tot de schouder, waarbij bevolkingsstudies aangeven dat ongeveer 15% -20% van de volwassenen ouder dan 65 jaar radiografisch bewijs van glenohumerale OA7,8 heeft. Deze aandoeningen zijn pijnlijk, tasten de activiteitsniveaus aan en verminderen de kwaliteit van leven.
Hoewel de pathogeneses van deze aandoeningen niet volledig worden begrepen, wordt algemeen aanvaard dat veranderde schouderbeweging geassocieerd is met veel schouderpathologieën9,10,11. In het bijzonder kan abnormale gewrichtsbeweging bijdragen aan de pathologie9,12, of dat de pathologie kan leiden tot abnormale gewrichtsbeweging13,14. Relaties tussen gewrichtsbeweging en pathologie zijn waarschijnlijk complex en subtiele veranderingen in gewrichtsbeweging kunnen belangrijk zijn in de schouder. Hoewel hoekbeweging bijvoorbeeld de overheersende beweging is die optreedt bij het glenohumerale gewricht, treden gezamenlijke translaties ook op tijdens schouderbewegingen. Onder normale omstandigheden zijn deze translaties waarschijnlijk niet groter dan enkele millimeters15,16,17,18,19 en kunnen daarom lager zijn dan het niveau van in-vivo nauwkeurigheid voor sommige meettechnieken. Hoewel het verleidelijk kan zijn om aan te nemen dat kleine afwijkingen in gewrichtsbeweging weinig klinische impact kunnen hebben, is het belangrijk om ook te erkennen dat het cumulatieve effect van subtiele afwijkingen gedurende jaren van schouderactiviteit de drempel van het individu voor weefselgenezing en -herstel kan overschrijden. Bovendien zijn in-vivo krachten in het glenohumerale gewricht niet onbelangrijk. Met behulp van op maat gemaakte geïnstrumenteerde glenohumerale gewrichtsimplantaten hebben eerdere studies aangetoond dat het verhogen van een gewicht van 2 kg tot hoofdhoogte met een uitgestrekte arm kan resulteren in glenohumerale gewrichtskrachten die kunnen variëren van 70% tot 238% van het lichaamsgewicht20,21,22. Bijgevolg kan de combinatie van subtiele veranderingen in gewrichtsbeweging en hoge krachten geconcentreerd over het kleine dragende oppervlak van de glenoïde bijdragen aan de ontwikkeling van degeneratieve schouderpathologieën.
Historisch gezien is de meting van schouderbeweging bereikt door middel van een verscheidenheid aan experimentele benaderingen. Deze benaderingen omvatten het gebruik van complexe cadaverische testsystemen die zijn ontworpen om schouderbeweging te simuleren23,24,25,26,27, videogebaseerde motion capture-systemen met oppervlaktemarkeringen28,29,31, opbouw elektromagnetische sensoren32,33,34,35 , botpennen met reflecterende markers of andere sensoren bevestigd36,37,38, statische tweedimensionale medische beeldvorming (d.w.z. fluoroscopie39,40,41 en röntgenfoto’s17,42,43,44,45), statische driedimensionale (3D) medische beeldvorming met MRI46,47, computertomografie48 en dynamische, 3D single plane fluoroscopische beeldvorming49,50,51. Meer recent hebben draagbare sensoren (bijv. traagheidsmeeteenheden) aan populariteit gewonnen voor het meten van schouderbewegingen buiten de laboratoriumomgeving en in vrije leefomstandigheden52,53,54,55,56,57.
In de afgelopen jaren is er een proliferatie geweest van tweedekker radiografische of fluoroscopische systemen die zijn ontworpen om dynamische, 3D in-vivo bewegingen van de schouder nauwkeurig te meten58,59,60,61,62. Het doel van dit artikel is om de aanpak van de auteurs te beschrijven voor het meten van schouderbeweging met behulp van een aangepast biplanair videoradiografiesysteem. De specifieke doelstellingen van dit artikel zijn het beschrijven van de protocollen voor het verkrijgen van biplanaire videoradiografische beelden van het schoudercomplex, het verkrijgen van CT-scans, het ontwikkelen van 3D-botmodellen, het lokaliseren van anatomische oriëntatiepunten, het volgen van de positie en oriëntatie van het opperarmbeen, het schouderblad en de romp van de biplanaire radiografische beelden en het berekenen van kinematische uitkomstmaten.
