Dessverre er skulderpatologier (f.eks. rotatormansjett tårer, ledddislokasjoner, leddgikt) vanlige, noe som resulterer i betydelig smerte, funksjonshemming og redusert livskvalitet. Den spesifikke etiologien for mange av disse patologiske forholdene er ikke fullt ut forstått, men det er generelt akseptert at skulderpatologi ofte er forbundet med endret felles bevegelse. Dessverre er det ikke trivielt å måle skulderbevegelsen med det nødvendige nøyaktighetsnivået for å undersøke bevegelsesbaserte hypoteser. Imidlertid har radiografiske baserte bevegelsesmålingsteknikker gitt fremskrittet som er nødvendig for å undersøke bevegelsesbaserte hypoteser og gi en mekanistisk forståelse av skulderfunksjon. Formålet med denne artikkelen er derfor å beskrive tilnærmingene for måling av skulderbevegelse ved hjelp av et tilpasset biplanet videoradiografisystem. De spesifikke målene med denne artikkelen er å beskrive protokollene for å skaffe seg biplanet videoradiografiske bilder av skulderkomplekset, skaffe CT-skanninger, utvikle 3D-beinmodeller, finne anatomiske landemerker, spore posisjonen og orienteringen til humerus, scapula og torso fra de biplanare radiografiske bildene, og beregne de kinematiske utfallstiltakene. I tillegg vil artikkelen beskrive spesielle hensyn som er unike for skulderen ved måling av leddkinematikk ved hjelp av denne tilnærmingen.
Skulderen er et av menneskekroppens mest komplekse leddsystemer, med bevegelse som skjer gjennom koordinerte handlinger fra fire individuelle ledd, flere leddbånd og ca. 20 muskler. Skulderen har også det største bevegelsesområdet for kroppens store ledd og beskrives ofte som et kompromiss mellom mobilitet og stabilitet. Dessverre er skulderpatologier vanlige, noe som resulterer i betydelig smerte, funksjonshemming og redusert livskvalitet. For eksempel påvirker rotatormansjett tårer omtrent 40% av befolkningen over 601,2,3, med omtrent 250,000 rotator mansjett reparasjoner utført årlig4, og en estimert økonomisk byrde på $ 3-5 milliarder per år i USA5. I tillegg er skulderdislokasjoner vanlige og er ofte forbundet med kronisk dysfunksjon6. Til slutt er glenohumeral leddartritt (OA) et annet betydelig klinisk problem som involverer skulderen, med befolkningsstudier som indikerer at omtrent 15% -20% av voksne over 65 år har radiografiske bevis på glenohumeral OA7,8. Disse forholdene er smertefulle, svekker aktivitetsnivået og reduserer livskvaliteten.
Selv om patogenene i disse forholdene ikke er fullt ut forstått, er det generelt akseptert at endret skulderbevegelse er forbundet med mange skulderpatologier9,10,11. Spesielt kan unormal leddbevegelse bidra til patologien9,12, eller at patologien kan føre til unormal felles bevegelse13,14. Sammenhenger mellom felles bevegelse og patologi er sannsynligvis komplekse, og subtile endringer i felles bevegelse kan være viktige i skulderen. For eksempel, selv om vinkelbevegelse er den dominerende bevegelsen som forekommer ved glenohumeralleddet, oppstår felles oversettelser også under skulderbevegelse. Under normale forhold overstiger disse oversettelsene sannsynligvis ikke flere millimeter15,16,17,18,19, og kan derfor være under nivået av in-vivo nøyaktighet for noen måleteknikker. Selv om det kan være fristende å anta at små avvik i felles bevegelse kan ha liten klinisk innvirkning, er det viktig å også erkjenne at den kumulative effekten av subtile avvik over år med skulderaktivitet kan overstige individets terskel for vevsheling og reparasjon. Videre er in-vivo krefter på glenohumeral joint ikke inkonsekvente. Ved hjelp av tilpassede instrumenterte glenohumerale leddimplantater har tidligere studier vist at å øke en vekt på 2 kg til hodehøyde med en utstrakt arm kan resultere i glenohumerale leddkrefter som kan variere fra 70% til 238% kroppsvekt20,21,22. Følgelig kan kombinasjonen av subtile endringer i felles bevegelse og høye krefter konsentrert over glenoidens lille bærende overflateområde bidra til utvikling av degenerative skulderpatologier.
Historisk har måling av skulderbevegelse blitt oppnådd gjennom en rekke eksperimentelle tilnærminger. Disse tilnærmingene har inkludert bruk av komplekse kadaveriske testsystemer designet for å simulere skulderbevegelse23,24,25,26,27, videobaserte bevegelsesfangstsystemer med overflatemarkører28,29,31, overflatemonterte elektromagnetiske sensorer32,33,34,35 , benpinner med refleksmarkører eller andre sensorer festet36,37,38, statisk todimensjonal medisinsk bildebehandling (dvs. fluoroskopi39,40,41 og radiografer17,42,43,44,45), statisk tredimensjonal (3D) medisinsk bildebehandling ved hjelp av MRI46,47, beregnet tomografi48, og dynamisk, 3D enkeltplan fluoroskopisk bildebehandling49,50,51. Mer nylig har bærbare sensorer (f.eks. inertial måleenheter) blitt populære for måling av skulderbevegelser utenfor laboratoriemiljøet og i fritt leveforhold52,53,54,55,56,57.
De siste årene har det vært en spredning av biplanradiografiske eller fluoroskopiske systemer designet for å nøyaktig måle dynamiske, 3D in-vivo bevegelser av skulderen58,59,60,61,62. Hensikten med denne artikkelen er å beskrive forfatternes tilnærming til måling av skulderbevegelse ved hjelp av et tilpasset biplanet videoradiografisystem.
