К сожалению, патологии плеча (например, разрывы вращательной манжеты, вывихи суставов, артрит) являются распространенными, что приводит к значительной боли, инвалидности и снижению качества жизни. Специфическая этиология для многих из этих патологических состояний не до конца понятна, но общепризнано, что патология плеча часто связана с измененным движением сустава. К сожалению, измерение движения плеча с необходимым уровнем точности для исследования гипотез, основанных на движении, не является тривиальным. Тем не менее, радиографические методы измерения движения обеспечили прогресс, необходимый для исследования гипотез на основе движения и обеспечения механистического понимания функции плеча. Таким образом, целью данной статьи является описание подходов к измерению движения плеча с помощью пользовательской бипланарной видеорадиографической системы. Плечо также имеет наибольший диапазон движения основных суставов тела и часто описывается как компромисс между подвижностью и стабильностью. К сожалению, патологии плеча распространены, что приводит к значительной боли, инвалидности и снижению качества жизни. Например, разрывы ротаторной манжеты затрагивают около 40% населения в возрасте старше 601,2,3 лет, при этом ежегодно выполняется около 250 000 ремонтов ротаторных манжет4, а экономическое бремя в Соединенных Штатах составляет 3-5 миллиардов долларов в год5. Кроме того, вывихи плеча распространены и часто связаны с хронической дисфункцией6. Наконец, остеоартрит плечевого сустава (ОА) является еще одной значительной клинической проблемой, связанной с плечом, причем популяционные исследования показывают, что примерно 15-20% взрослых в возрасте старше 65 лет имеют рентгенологические доказательства гленогечевого ОА7,8. В нормальных условиях эти переводы, вероятно, не превышают нескольких миллиметров15,16,17,18,19, и поэтому могут быть ниже уровня точности in-vivo для некоторых методов измерения. Хотя может быть заманчиво предположить, что небольшие отклонения в движении суставов могут иметь небольшое клиническое воздействие, важно также признать, что кумулятивный эффект тонких отклонений в течение многих лет активности плеча может превышать порог человека для заживления и восстановления тканей. Кроме того, силы in vivo в плетеностном суставе не являются несущественными. Используя специальные инструментальные имплантаты плетеногечевого сустава, предыдущие исследования показали, что повышение веса 2 кг до высоты головы с вытянутой рукой может привести к силам плесневому сустава, которые могут варьироваться от 70% до 238% массы тела20,21,22. Следовательно, сочетание тонких изменений в движении суставов и высоких сил, сосредоточенных над небольшой площадью несущей поверхности гленоида, может способствовать развитию дегенеративных патологий плеча.
Исторически сложилось так, что измерение движения плеча осуществлялось с помощью различных экспериментальных подходов. Эти подходы включали использование сложных трупных испытательных систем, предназначенных для моделирования движения плеча23,24,25,26,27, систем захвата движения на основе видео с поверхностными маркерами28,29,31, поверхностных электромагнитных датчиков32,33,34,35 , костные штифты с прикрепленными отражающими маркерами или другими датчиками36,37,38, статическая двумерная медицинская визуализация (т.е. рентгеноскопия39,40,41 и рентгенограммы17,42,43,44,45), статическая трехмерная (3D) медицинская визуализация с использованием МРТ46,47, компьютерная томография48, и динамическая, 3D одноплоскостная флюороскопическая визуализация49,50,51. В последние годы наблюдается распространение бипланных рентгенографических или флюороскопических систем, предназначенных для точного измерения динамических 3D-движений плеча in vivo58,59,60,61,62.
