Pulmonary Ventilation: Inhalation

Die Atmung ist ein rhythmischer Prozess des abwechselnden Ein- und Ausatmens.
Bei der ruhigen Inhalation dehnt sich die Brusthöhle aufgrund eines Abfalls des Gasdrucks in der Lunge aus, wodurch Luft einströmt.
Das Zwerchfell ist der wichtigste Muskel für die Inhalation. Es zieht sich zusammen und senkt sich etwa 1 cm ab und flacht ab, wodurch die Höhe der Brusthöhle erhöht wird und eine Druckdifferenz von etwa 1 bis 3 mm Hg entsteht.
Durch die Kontraktion der äußeren Interkostalmuskulatur wird der Brustkorb angehoben und das Brustbein nach oben gezogen.
Die Rippen ragen nach außen hervor und erweitern den Durchmesser des Thorax lateral und anteroposterior.
Obwohl sich die Thoraxmaße nur um wenige Millimeter verändern, vergrößert sich das Thoraxvolumen um etwa 500 ml.
Mit zunehmender Vergrößerung des Brustkorbs nimmt das intrapulmonale Volumen zu, was zu einer Abnahme des intrapulmonalen Drucks um etwa 1 mm Hg unter den atmosphärischen Druck führt.
Luft strömt in die Lunge, bis der intrapulmonale Druck dem atmosphärischen Druck entspricht. Gleichzeitig sinkt der intrapleurale Druck auf etwa -6 mm Hg relativ zum Atmosphärendruck.

Das Atemzugvolumen (TV) bezieht sich auf das Luftvolumen, das bei normaler Atmung ohne bewusste Anstrengung ein- oder ausgeatmet wird.

Das inspiratorische Reservevolumen (IRV) ist das maximale Luftvolumen, das über das Atemzugvolumen hinaus forciert eingeatmet werden kann.

Exspiratorisches Reservevolumen (ERV): Das maximale Luftvolumen, das über das Atemzugvolumen hinaus zwangsweise ausgeatmet werden kann.

Residualvolumen (RV): Das Luftvolumen, das nach maximaler Ausatmung in der Lunge verbleibt.

Diese Atemvolumina fließen in die Berechnung wichtiger Messwerte wie beispielsweise der Vitalkapazität ein.

Die Vitalkapazität (VC) ist die Summe aus Atemzugvolumen, inspiratorischem Reservevolumen und exspiratorischem Reservevolumen.

Darüber hinaus ist die Beziehung zwischen Atemvolumen und Druck eng miteinander verknüpft. Während der normalen Einatmung ziehen sich das Zwerchfell und die äußeren Interkostalmuskeln zusammen und weiten den Brustkorb. Diese Ausdehnung vergrößert das Lungenvolumen, was zu einer Abnahme des intrapulmonalen Drucks führt, sodass Luft aus der Atmosphäre in die Lunge strömen kann.

Die Mechanik der Inspiration (Einatmung)

Das Einatmen erfolgt durch eine Reihe miteinander verbundener Prozesse, an denen das Zwerchfell, die Interkostalmuskulatur und die Ausdehnung der Brusthöhle beteiligt sind. Schauen wir uns die einzelnen Schritte an:

Beim Einatmen zieht sich das Zwerchfell zusammen und flacht ab, wodurch das Volumen des Brustkorbs zunimmt. Dadurch entsteht ein negativer intrapulmonaler Druck, der Luft in die Lunge zieht.</li>
	<li>Einsatz der Interkostalmuskeln: Die Interkostalmuskeln sind eine weitere wichtige Gruppe von Muskeln, die an der Einatmung beteiligt sind. Sie befinden sich zwischen den Rippen und werden in äußere und innere Zwischenrippenmuskeln unterteilt.
	Äußere Interkostalmuskeln: Beim Einatmen ziehen sich diese Muskeln zusammen und heben und dehnen den Brustkorb. Diese Bewegung vergrößert das Volumen der Brusthöhle weiter und verbessert die Luftaufnahme.

	Innere Interkostalmuskeln: Diese Muskeln unterstützen das tiefe Einatmen und kräftige Ausatmen. Beim kräftigen Einatmen ziehen sich die oberen inneren Interkostalmuskeln zusammen, wodurch der Brustkorb noch mehr angehoben wird und eine größere Lungenausdehnung möglich wird.
	</li>
	<li>Ausdehnung der Brusthöhle: Wenn sich das Zwerchfell zusammenzieht und die Interkostalmuskeln sich anspannen, dehnt sich die Brusthöhle in mehrere Richtungen aus. Diese Ausdehnung senkt den Druck in der Lunge und erzeugt einen Druckunterschied zwischen der Außenluft und der Luft im Atmungssystem. Infolgedessen strömt Luft in die Lunge und füllt die Alveolen (kleine Luftsäckchen) für den Gasaustausch.</li>
</ol>

Forcierte Inhalation: Bei erhöhter körperlicher Anstrengung oder wenn eine erhöhte Sauerstoffzufuhr erforderlich ist, kommt es zur forcierten Inhalation. Bei der forcierten Inhalation werden zusätzliche Muskeln eingesetzt, um die Lungenausdehnung zu maximieren:

<ol>
	<li>Musculus sternocleidomastoideus: Der Musculus sternocleidomastoideus im Nacken spielt eine wichtige Rolle bei der forcierten Einatmung. Wenn er aktiviert wird, hebt er das Brustbein an und unterstützt so die weitere Ausdehnung des Brustkorbs.</li>
	<li>Skalenusmuskeln: Die Skalenusmuskeln befinden sich auf beiden Seiten des Halses und heben beim forcierten Einatmen die oberen Rippen an. Ihre Kontraktion hilft, mehr Platz für die Lungenausdehnung zu schaffen.</li>
	<li>Kleiner Brustmuskel: Der kleine Brustmuskel, der sich unterhalb des großen Brustmuskels befindet, hilft beim Anheben der Rippen und der Erweiterung des Brustkorbs.</li>
</ol>

Die Rolle des Drucks bei der Lungenventilation

Eine ausreichende Lungenbeatmung hängt vom Zusammenspiel dreier unterschiedlicher Druckverhältnisse ab:

<ol>
	<li>Intrapulmonaler Druck: Auch als Alveolardruck bekannt. Bezieht sich auf den Druck in der Lunge. Während der Inspiration (Einatmung) sinkt der intrapulmonale Druck, wodurch Luft in die Lunge strömt. Im Gegensatz dazu steigt der intrapulmonale Druck während der Ausatmung an, wodurch die Luft leichter ausgestoßen werden kann.</li>
	<li>Intrapleuraler Druck: Dieser Druck herrscht in der Pleurahöhle, dem Raum zwischen der parietalen und viszeralen Pleura. Er ist in der Regel niedriger als der atmosphärische und der intrapulmonale Druck, was auf das Gleichgewicht der Kräfte zwischen dem Rückstoß der Lunge und der Ausdehnung der Brustwand zurückzuführen ist, wodurch die Lunge gegen die Brustwand ausgedehnt bleibt. Jede Veränderung des intrapleuralen Drucks kann zu einem Lungenkollaps oder anderen Atemproblemen führen.</li>
	<li>Atmosphärendruck: Der Atmosphärendruck ist der Druck, der von der Erdatmosphäre an einem bestimmten Ort ausgeübt wird. Er dient als Basiswert, an dem der intrapulmonale Druck gemessen wird. Pulmonary ventilation depends on breathing movements and the pressure changes that move air into and out of the lungs. It supports the exchange of oxygen and carbon dioxide by letting the body take in oxygen and remove waste carbon dioxide. Boyle's law is important here because changes in volume and pressure affect how air moves in the respiratory system.

Several respiratory volumes help describe how much air the lungs move. Tidal volume is the air inspired or expired during normal, quiet breathing. Inspiratory reserve volume is the extra air that can be forcefully inhaled after a normal breath, while expiratory reserve volume is the extra air that can be forcefully exhaled after a normal breath. Residual volume is the air that remains in the lungs after maximal expiration. These values are used to calculate vital capacity, which is the sum of tidal volume, inspiratory reserve volume, and expiratory reserve volume.

Normal inhalation begins when the diaphragm contracts and the external intercostal muscles contract. The diaphragm is a dome-shaped muscle that separates the thoracic cavity from the abdominal cavity, and when it flattens, the chest cavity gets larger. The external intercostal muscles lift and expand the rib cage, which also increases thoracic volume. As the chest cavity expands, intrapulmonary pressure falls and air flows from the atmosphere into the lungs.

Forced inhalation uses additional muscles when the body needs more oxygen, such as during physical exertion. The sternocleidomastoid muscle in the neck raises the sternum. The scalene muscles on either side of the neck lift the upper ribs. The pectoralis minor also helps raise the ribs and expand the chest cavity even more.

Breathing also depends on three related pressures. Intrapulmonary pressure, also called alveolar pressure, is the pressure inside the lungs. It drops during inspiration and rises during expiration. Intrapleural pressure is the pressure in the pleural cavity, between the parietal and visceral pleurae. It is usually lower than atmospheric and intrapulmonary pressure because of the balance between lung recoil and chest wall expansion, which helps keep the lungs expanded against the chest wall. If intrapleural pressure changes too much, lung collapse or other breathing problems can result. Atmospheric pressure is the pressure of the air around us, and it serves as the reference point for the pressure changes that move air in and out of the lungs.

Content elaboration: The digestive system consists of the gastrointestinal tract (alimentary canal) as well as various accessory organs including the teeth, tongue, salivary glands, liver, gallbladder and pancreas. The digestive system processes and supplies the molecules needed to sustain the living tissues within the body through the absorption of nutrients. Six major functions of the digestive system include secretion, ingestion, mechanical processing, enzymatic digestion, absorption and excretion. The lining of the digestive system protects surrounding tissues from the mechanical and enzymatic stresses of the digestive process. Processes of the digestive system include the mechanical and chemical breakdown of food into small molecules which are then absorbed by the digestive tract. Specific actions within the digestive system include mastication, peristalsis, segmentation and the release of hormones and enzymes necessary for digestion. The metabolic functions of the accessory organs play strategic roles in the breakdown of food products, the maintenance of glucose levels within the blood and the regulation of homeostasis in the body. Indigestible material is excreted as waste. Homeostatic imbalances are explored. These include, but are not limited to, conditions such as gallstones, heartburn, ulcers, dehydration, diarrhea, cirrhosis and cancers of the digestive system. Investigations are used to understand and explain the digestive system in a variety of inquiry and design scenarios that can incorporate evolutionary concepts, scientific reasoning, comparative analysis, communication skills and real-world applications.
