Factors Affecting Pulmonary Ventilation

Breathing is influenced by the alternating pressure difference between the external environment and the lungs, alveolar surface tension, lung compliance, and airway resistance.
In the alveoli, the luminal surface is lined by an aqueous alveolar fluid with a surfactant mixture of phospholipids and lipoproteins.
This alveolar surfactant reduces the attractive forces between the polar water molecules, reducing the surface tension of the alveolar fluid. As a result, less energy is required to expand the alveoli during inhalation.
Additionally, the alveolar fluid and the elastic fibers in the lung tissue increase lung compliance, reducing the effort required to stretch the lungs to fill air.
The walls of the airways, especially in smaller bronchioles, offer resistance to the air filling the lungs.
Signals from the sympathetic nervous system relax the smooth muscles in the bronchial walls, increasing their diameter to decrease the resistance.
On the other hand, the parasympathetic nervous system stimulates the contraction of the bronchial smooth muscles, or bronchoconstriction, and increases airway resistance.

Besides the pressure difference between the external environment and the lungs, the airflow rate and ease of pulmonary ventilation are also influenced by three other factors: surface tension of the fluid in the alveoli, compliance of the lungs, and airway resistance.
Alveolar Surface Tension
The alveolar fluid lines the luminal surface of the alveoli and exerts a force called surface tension. This force is caused by the polar water molecules in the liquid being more strongly attracted to each other than gas molecules in the air. This results in a tendency for the fluid to collapse the alveoli. This surface tension must be overcome to expand the lungs during inhalation. However, in premature infants with a deficiency of surfactant, the surface tension is significantly increased and can lead to respiratory distress syndrome. This condition requires more significant effort during inhalation to reopen the collapsed alveoli.
Lung Compliance
The next factor affecting pulmonary ventilation is compliance, which refers to how much effort is required to stretch the lungs and chest wall. High compliance implies that the lungs and chest wall expand easily, while low compliance implies resistance to expansion. In the lungs, elasticity and surface tension contribute to changes in compliance. The elastic fibers in lung tissue allow them to expand readily due to the easy stretching, and the surfactant in the alveolar fluid reduces the surface tension. However, certain conditions, such as tuberculosis or pulmonary edema, can decrease lung compliance by scarring lung tissue or filling it with fluid. Conversely, emphysema increases lung compliance due to the destruction of elastic fibers in the alveolar walls, making the lungs more easily expandable but less effective at expelling air.
Airway Resistance
Finally, airway resistance also plays a role in pulmonary ventilation. The airflow rate through the airways depends on the pressure difference and the resistance. As the lungs expand during inhalation, airway resistance decreases due to the enlargement of the bronchioles. However, during exhalation, when bronchioles decrease in diameter, resistance increases and is regulated by smooth muscles in the walls of the airways, which can either constrict or relax depending on signals from the sympathetic or parasympathetic divisions of the ANS. Certain conditions, such as asthma or COPD, can increase airway resistance due to the obstruction or collapse of airways.
In conclusion, the rate of airflow and ease of pulmonary ventilation are affected by the surface tension of alveolar fluid, lung compliance, and airway resistance. Understanding these factors is crucial for maintaining healthy breathing and identifying respiratory disorders that require medical intervention.

Breathing ease in the lungs depends on alveolar surface tension, lung compliance, and airway resistance. These factors affect how hard it is to move air during pulmonary ventilation, along with the pressure difference between the outside environment and the lungs.

Alveolar surface tension comes from the fluid lining the inside of the alveoli. Water molecules in this fluid attract each other more strongly than they attract the air, so the liquid tends to pull the alveoli inward. That force must be overcome during inhalation to expand the lungs. In premature infants with too little surfactant, surface tension becomes much higher and can cause respiratory distress syndrome, which makes inhalation harder because the collapsed alveoli must be reopened.

Lung compliance describes how easily the lungs and chest wall stretch. High compliance means they expand with less effort, while low compliance means they resist expansion. Elastic fibers and surfactant help the lungs stretch more easily, but tuberculosis and pulmonary edema can lower compliance by scarring lung tissue or filling it with fluid. Emphysema has the opposite effect and increases compliance because elastic fibers in the alveolar walls are destroyed, making the lungs easier to expand but less effective at pushing air out.

Airway resistance also changes how smoothly air moves through the respiratory tract. Airflow depends on both pressure difference and resistance. During inhalation, the bronchioles widen as the lungs expand, so resistance decreases. During exhalation, bronchioles become narrower, so resistance increases. Smooth muscle in airway walls helps control this change, and it can constrict or relax based on signals from the sympathetic or parasympathetic divisions of the autonomic nervous system. Asthma and COPD can raise airway resistance by blocking or collapsing the airways, which makes breathing more difficult.
