Pulmonary Cycle: Exhalation

Die Ausatmung – der Prozess des Ausstoßens von Luft – während der ruhigen Atmung bei gesunden Menschen beruht auf der Elastizität der Lunge und nicht auf der Muskelkontraktion.
Der Rückstoß von elastischen Fasern, die beim Einatmen gedehnt werden, verursacht in erster Linie den Rückstoß der Lunge.
Darüber hinaus trägt die innere Anziehungskraft der Oberflächenspannung, die durch die intrapleurale Flüssigkeitsschicht zwischen den viszeralen und parietalen Pleuren erzeugt wird, zu diesem Lungenrückstoß bei.
Wenn die an der Inhalation beteiligten Muskeln ihre Aktivität einstellen, entspannen sich das Zwerchfell und die äußeren Interkostalen, wodurch das Zwerchfell aufsteigt und die Rippen absinken.
Diese Bewegungen verkleinern die Dimensionen der Brusthöhle und verringern dadurch das Lungenvolumen.
Während der Ausatmung steigt der intrapulmonale Druck auf 1 mm Hg über dem Atmosphärendruck. Und wenn der intrapulmonale Druck größer ist als der atmosphärische Druck, strömt Gas aus der Lunge.
Im Gegensatz zu dieser ruhigen Ausatmung beinhaltet die kraftvolle Ausatmung die Kontraktion der Bauch- und Interkostalmuskulatur. Es zwingt die Membran, aus ihrer normalen Position aufzusteigen, um die Luft mit Kraft auszustoßen.

Bei der menschlichen Atmung beruht das Ausstoßen von Luft, das sogenannte Ausatmen, auf dem Prinzip des Druckgradienten. Beim Ausatmen übersteigt der Druck in der Lunge den der umgebenden Atmosphäre. Unter normalen Bedingungen erfolgt ruhiges Atmen passiv und ohne Muskelkontraktionen. Dies liegt daran, dass der Ausatmungsprozess durch die natürliche elastische Rückfederung der Lunge und der Brustwand angetrieben wird, die beide die inhärente Tendenz haben, nach der Ausdehnung in ihre ursprüngliche Position zurückzukehren.

Wenn die an der Einatmung beteiligten Muskeln ihre Aktivität einstellen, entspannen sich das Zwerchfell und die äußeren Interkostalmuskeln. Dies führt dazu, dass sich das Zwerchfell hebt und die Rippen absinken. Durch diese Bewegungen wird die Dimension der Brusthöhle verkleinert, wodurch sich das Lungenvolumen verringert und der Alveolardruck auf etwa 762 mmHg steigt. Dieser Druckunterschied zwingt die Luft von der Lunge (höherer Druck) in die Atmosphäre (niedrigerer Druck). Das Ausatmen erfordert nur bei erhöhter körperlicher Anstrengung, wie z. B. beim Sport oder beim Spielen eines Blasinstruments, aktive Anstrengung. In solchen Momenten werden die an der Ausatmung beteiligten Muskeln – die Bauchmuskeln und die inneren Interkostalmuskeln – aktiviert, was Druck im Bauchbereich und im Brustkorb erzeugt. Die Kontraktion der Bauchmuskeln komprimiert die Bauchorgane und drückt das Zwerchfell nach oben, während die inneren Interkostalmuskeln die Rippen durch ihre untere und hintere Verlängerung zwischen benachbarten Rippen nach unten ziehen. Obwohl der Druck im Brustfell immer niedriger bleibt als in den Alveolen, kann er bei einem kräftigen Ausatmen, wie z. B. beim Husten, den atmosphärischen Druck vorübergehend überschreiten.

	Neonatales RDS oder hyaline Membrankrankheit betrifft vor allem Frühgeborene. Diese Erkrankung entsteht durch die unzureichende Produktion von Surfactant in der Lunge, einer Substanz, die hilft, die Alveolen offen zu halten. Ohne ausreichend Surfactant kollabieren die Alveolen beim Ausatmen, was dem Säugling das Atmen erschwert.

	Zu den Symptomen des RDS gehören schnelle und flache Atmung, geblähte Nasenlöcher, Grunzgeräusche und eine bläuliche Verfärbung der Haut (Zyanose). Die Behandlung umfasst häufig die Verabreichung von zusätzlichem Sauerstoff und einer Surfactant-Ersatztherapie zur Verbesserung der Lungenfunktion. Bei entsprechender medizinischer Intervention erholen sich die meisten Säuglinge innerhalb weniger Tage bis Wochen.

	<li>Atelektase
	Atelektase ist der teilweise Kollaps eines Lungenabschnitts. Dieser Zustand kann aus verschiedenen Gründen auftreten, beispielsweise blockierte Atemwege, Stich- oder Schusswunden im Brustkorb, chirurgische Öffnung des Brustkorbs oder geschwächtes Lungengewebe. Häufige Ursachen sind Schleimpfropfen, Tumore, Fremdkörper und postoperative Komplikationen.

	Die Symptome einer Atelektase hängen vom Ausmaß des Lungenkollapses ab und können Brustschmerzen, Kurzatmigkeit, erhöhte Herzfrequenz und Husten umfassen. Die Behandlungsmöglichkeiten reichen von Atemübungen und Hustentechniken bis hin zu Bronchodilatatoren oder Operationen in schweren Fällen.

	<li>Bronchitis
	Bronchitis ist eine Entzündung der Bronchien. Die Erkrankung kann akut oder chronisch sein, wobei Virusinfektionen die häufigste Ursache für akute Bronchitis sind. Die chronische Bronchitis hingegen ist oft mit einer langfristigen Einwirkung von Reizstoffen wie Tabakrauch und Luftverschmutzung verbunden.

Zu den Symptomen einer Bronchitis können anhaltender Husten, Schleimbildung, Brustbeschwerden und Keuchen gehören. Die Behandlung umfasst in der Regel Ruhe, ausreichende Flüssigkeitszufuhr, Sauerstoff, Bronchodilatatoren und die Behandlung der zugrunde liegenden Ursache. Wenn eine bakterielle Infektion vorliegt, können Antibiotika verschrieben werden.

Exhalation is the process of expelling air from the lungs. It depends on pressure differences between the lungs and the atmosphere. During normal quiet breathing, exhalation is passive and does not require muscle contraction. The elastic recoil of the lungs and chest wall helps both structures return to their resting positions after inhalation.

When the muscles used for inhalation stop working, the diaphragm and external intercostal muscles relax. The diaphragm moves upward, and the ribs move downward. These changes make the thoracic cavity smaller and reduce lung volume. As a result, alveolar pressure rises to about 762 mm Hg, and air flows out of the lungs into the lower-pressure atmosphere.

Active exhalation happens during exercise or when playing a wind instrument. In these cases, the abdominal muscles and internal intercostals contract. Abdominal muscle contraction compresses the abdominal organs and pushes the diaphragm upward. The internal intercostals pull the ribs downward because of their inferior and posterior position between adjacent ribs.

The pressure in the pleural space remains lower than the pressure inside the alveoli, but it can briefly rise above atmospheric pressure during forceful exhalation, such as coughing. The same pressure changes that help air leave the lungs can also relate to lung problems. Neonatal respiratory distress syndrome, atelectasis, and bronchitis are all conditions connected to pulmonary ventilation.

Neonatal respiratory distress syndrome, also called hyaline membrane disease, mainly affects premature infants. It happens when the lungs do not produce enough surfactant, a substance that helps keep alveoli open. Without enough surfactant, alveoli can collapse during exhalation, which makes breathing difficult. Common signs include rapid shallow breathing, nostril flaring, grunting, and cyanosis, or a bluish skin color. Treatment often includes supplemental oxygen and surfactant replacement therapy, and many infants recover within days to weeks.

Atelectasis is a partial collapse of part of a lung. It can result from blocked airways, stab or gunshot wounds, surgical opening of the chest, or weakened lung tissue. Common causes include mucus plugs, tumors, foreign objects, and post-surgical complications. Symptoms may include chest pain, shortness of breath, a faster heart rate, and coughing. Treatment can include deep breathing exercises, coughing techniques, bronchodilators, or surgery in severe cases.

Bronchitis is inflammation of the bronchial tubes. Acute bronchitis is most often caused by viral infections. Chronic bronchitis is often linked to long-term exposure to irritants such as tobacco smoke and air pollution. Symptoms can include a persistent cough, mucus production, chest discomfort, and wheezing. Treatment usually includes rest, hydration, oxygen, bronchodilators, and care for the underlying cause. If a bacterial infection is present, antibiotics may be used.

Content elaboration: The digestive system consists of the gastrointestinal tract (alimentary canal) as well as various accessory organs including the teeth, tongue, salivary glands, liver, gallbladder and pancreas. The digestive system processes and supplies the molecules needed to sustain the living tissues within the body through the absorption of nutrients. Six major functions of the digestive system include secretion, ingestion, mechanical processing, enzymatic digestion, absorption and excretion. The lining of the digestive system protects surrounding tissues from the mechanical and enzymatic stresses of the digestive process. Processes of the digestive system include the mechanical and chemical breakdown of food into small molecules which are then absorbed by the digestive tract. Specific actions within the digestive system include mastication, peristalsis, segmentation and the release of hormones and enzymes necessary for digestion. The metabolic functions of the accessory organs play strategic roles in the breakdown of food products, the maintenance of glucose levels within the blood and the regulation of homeostasis in the body. Indigestible material is excreted as waste. Homeostatic imbalances are explored. These include, but are not limited to, conditions such as gallstones, heartburn, ulcers, dehydration, diarrhea, cirrhosis and cancers of the digestive system. Investigations are used to understand and explain the digestive system in a variety of inquiry and design scenarios that can incorporate evolutionary concepts, scientific reasoning, comparative analysis, communication skills and real-world applications.
