Respiratory Volumes and Capacities

Los volúmenes respiratorios definen las cantidades variables de aire que entran y salen de los pulmones, mientras que las capacidades respiratorias son combinaciones de volúmenes respiratorios.

El sistema respiratorio es responsable de la ingesta de oxígeno y la expulsión de dióxido de carbono del cuerpo. Los volúmenes respiratorios describen el volumen de aire en los pulmones en diferentes fases del ciclo respiratorio. El volumen corriente es el aire que se inhala y exhala durante una respiración normal y tranquila. El volumen de reserva inspiratoria es el aire que se puede inspirar con fuerza más allá del volumen corriente. Por el contrario, el volumen de reserva espiratoria se refiere al aire que se puede expulsar de los pulmones después de una espiración normal. El volumen residual, que mantiene abiertos los alvéolos y evita el colapso pulmonar, es el aire en los pulmones después de una exhalación enérgica. 

Capacidades respiratorias 

Las capacidades respiratorias, como la capacidad pulmonar inspiratoria, residual funcional, vital y total, se miden para diagnosticar trastornos respiratorios. La capacidad inspiratoria (CI) es la cantidad máxima de aire que se puede inhalar después de una exhalación normal, calculada sumando el volumen corriente (VT) y el volumen de reserva inspiratoria (IRV). La capacidad residual funcional (CRF) representa el aire en los pulmones después de una espiración tranquila. Es la combinación del volumen residual (VR) y el volumen de reserva espiratorio (ERV). La capacidad vital (VC) es la cantidad máxima de aire que una persona puede exhalar después de tomar la respiración más profunda posible y es la suma de TV, IRV y ERV. Finalmente, la capacidad pulmonar total (TLC) es la suma de todos los volúmenes pulmonares y proporciona información crucial sobre el estado respiratorio de una persona. En las enfermedades pulmonares obstructivas, la TLC, la CRF y el RV pueden aumentar porque los pulmones se hiperinsuflan, mientras que en las enfermedades restrictivas, la VC, la TLC, la CRF y el RV disminuyen porque la expansión pulmonar es limitada. 

Ventilación minuto 

La ventilación minuto, o el volumen total de aire inspirado y espirado cada minuto, se calcula multiplicando el volumen corriente por la frecuencia respiratoria. En un adulto sano en reposo, la ventilación minuto suele rondar los 6000-8000 ml/min. Una ventilación minuto inferior a la normal puede indicar un problema en los pulmones o en el sistema respiratorio. Tenga en cuenta que no todo el aire inhalado llega a la zona respiratoria del sistema respiratorio, ya que parte permanece en las vías respiratorias de conducción, conocidas como espacio muerto anatómico. 

Ventilación alveolar 

La ventilación alveolar es el volumen de aire por minuto que llega a la zona respiratoria y participa en la respiración externa. Esta medida suele rondar los 4200 ml/min en un adulto sano en reposo. Lung volume measurements show how air moves in and out of the respiratory system during breathing. They help explain how the body takes in oxygen and removes carbon dioxide. Respiratory volumes and capacities also give useful information about lung function.

Tidal volume is the air breathed in and out during normal, quiet breathing. Inspiratory reserve volume is the extra air that can be forcefully inhaled beyond tidal volume. Expiratory reserve volume is the extra air that can be forcefully exhaled after a normal breath out. Residual volume is the air left in the lungs after a forceful exhale, and it helps keep the alveoli open so the lungs do not collapse.

Respiratory capacities combine these volumes into larger measurements. Inspiratory capacity is the maximum air that can be inhaled after a normal exhalation. It equals tidal volume plus inspiratory reserve volume. Functional residual capacity is the air left in the lungs after a quiet expiration. It equals residual volume plus expiratory reserve volume.

Vital capacity is the greatest amount of air a person can exhale after taking the deepest breath possible. It is the sum of tidal volume, inspiratory reserve volume, and expiratory reserve volume. Total lung capacity is the sum of all lung volumes and gives important information about respiratory status. In obstructive pulmonary disease, total lung capacity, functional residual capacity, and residual volume may increase because the lungs hyperinflate. In restrictive disease, vital capacity, total lung capacity, functional residual capacity, and residual volume decline because lung expansion is limited.

Minute ventilation is the total volume of air inspired and expired each minute. It is calculated by multiplying tidal volume by respiratory rate. In a healthy adult at rest, minute ventilation is usually about 6000 to 8000 mL/min. Lower-than-normal minute ventilation may suggest a problem with the lungs or the respiratory system.

Not all inhaled air reaches the respiratory zone. Some of it stays in the conducting airways, which is called anatomic dead space. Alveolar ventilation is the volume of air per minute that reaches the respiratory zone and takes part in external respiration. It is calculated by multiplying respiratory rate by the difference between tidal volume and the air left in anatomic dead space. In a healthy adult at rest, alveolar ventilation is usually about 4200 mL/min. It gives a better measure of gas exchange in the lungs than minute ventilation alone. Together, minute ventilation and alveolar ventilation are key measures of pulmonary function and can help diagnose respiratory disorders and monitor treatment progress.

The digestive system processes and supplies the molecules needed to sustain the living tissues within the body through the absorption of nutrients. Six major functions of the digestive system include secretion, ingestion, mechanical processing, enzymatic digestion, absorption and excretion. The lining of the digestive system protects surrounding tissues from the mechanical and enzymatic stresses of the digestive process. Processes of the digestive system include the mechanical and chemical breakdown of food into small molecules which are then absorbed by the digestive tract. Specific actions within the digestive system include mastication, peristalsis, segmentation and the release of hormones and enzymes necessary for digestion. The metabolic functions of the accessory organs play strategic roles in the breakdown of food products, the maintenance of glucose levels within the blood and the regulation of homeostasis in the body. Indigestible material is excreted as waste. Homeostatic imbalances are explored. These include, but are not limited to, conditions such as gallstones, heartburn, ulcers, dehydration, diarrhea, cirrhosis and cancers of the digestive system. Investigations are used to understand and explain the digestive system in a variety of inquiry and design scenarios that can incorporate evolutionary concepts, scientific reasoning, comparative analysis, communication skills and real-world applications.

The airways include the nasal and oral cavities, pharynx, larynx, trachea, bronchi, bronchioles and alveoli. The respiratory system transports and exchanges gases including oxygen and carbon dioxide. Processes involved in the respiratory system include respiration mechanics and gas exchange. Respiration mechanics is the process by which humans breathe and includes the movement of the diaphragm and pressure-volume relationships. Gas exchange refers to the diffusion of gas across the alveolar epithelium in the respiratory system and capillary endothelium of the cardiovascular system. Lung volumes and capacities can be measured using spirometry. Homeostatic imbalances are explored. These include, but are not limited to, asthma, chronic obstructive pulmonary disease (COPD), tuberculosis, cystic fibrosis and the effects of smoking and pollution.
