The Bronchial Tree

Au niveau de la carène, située à la limite supérieure de la cinquième vertèbre thoracique, la trachée se divise en bronches primaires droite et gauche, chacune menant à son poumon respectif. La bronche primaire droite est plus courte, plus large et plus verticale que la gauche.
Semblables à la trachée, les deux bronches primaires sont composées d’anneaux cartilagineux incomplets et d’une muqueuse épithéliale cylindrique pseudostratifiée ciliée.
En entrant dans les poumons, chaque bronche primaire se ramifie en bronches lobaires ou secondaires plus petites, avec une bronche desservant chaque lobe dans les deux poumons.
Ces bronches lobaires se ramifient en bronches segmentaires plus petites, desservant des segments bronchopulmonaires spécifiques à l’intérieur des lobes.
La segmentation se poursuit, formant des bronchioles qui se ramifient en bronchioles terminales, marquant la fin de la zone conductrice de l’arbre bronchique.

La trachée, est un tube qui relie le larynx (boîte vocale) aux bronches. Au niveau d'un point appelé carène, elle se divise en deux bronches primaires. La bronche primaire droite est plus large, plus courte et plus verticale que la bronche primaire gauche. Cette différence anatomique est souvent associée à une incidence plus élevée d'aspiration de corps étrangers et d'infections dans le poumon droit.

Située au point de bifurcation, la carène est une crête cartilagineuse qui contribue au maintien de la perméabilité des voies respiratoires. Elle possède également une forte densité de récepteurs de la toux, ce qui la rend très sensible aux irritants et déclenche un réflexe de toux pour dégager les voies respiratoires.

Les bronches primaires se divisent ensuite en bronches secondaires ou lobaires, dont trois à droite (supérieure, moyenne et inférieure) et deux à gauche (supérieure et inférieure). Ces bronches lobaires pénètrent dans les poumons et alimentent en air des lobes spécifiques.

Les alvéoles sont de minuscules sacs aériens où se déroulent les échanges gazeux, permettant à l'oxygène de pénétrer dans la circulation sanguine tandis que le dioxyde de carbone est expulsé.

Tout au long de l'arbre bronchique, les structures subissent des changements de composition importants. Par exemple, la muqueuse des bronches principales est constituée d'un épithélium cylindrique pseudostratifié. Cette composition se transforme en un épithélium cylindrique pseudostratifié cilié avec des cellules caliciformes dans les bronchioles plus grandes, en un épithélium cuboïde simple cilié sans cellules caliciformes dans les bronchioles plus petites et en un épithélium cuboïde simple non cilié dans les bronchioles terminales. Cette couche muqueuse contribue à humidifier et à filtrer l'air inhalé tout en piégeant les particules étrangères et les agents pathogènes.

La sous-muqueuse se trouve sous la muqueuse et contient des glandes qui produisent le mucus nécessaire pour piéger et éliminer les particules nocives. De plus, la sous-muqueuse contient des vaisseaux sanguins, des nerfs et des vaisseaux lymphatiques qui contribuent au fonctionnement général de l'arbre bronchique. The bronchial tree carries air from the trachea into the lungs and helps control breathing. The trachea, or windpipe, connects the larynx to the bronchi. At the carina, the trachea splits into two primary bronchi. The right primary bronchus is wider, shorter, and more vertical than the left. Because of that shape, foreign objects and infections are more likely to enter the right lung.

The carina is a cartilaginous ridge at the split in the airway. It helps keep the airway open. It also has many cough receptors, so it is very sensitive to irritants and can trigger a cough to clear the airways.

The primary bronchi branch into secondary, or lobar, bronchi. The right lung has three lobar bronchi: superior, middle, and inferior. The left lung has two: superior and inferior. These lobar bronchi enter the lungs and carry air to specific lobes. They then divide into segmental, or tertiary, bronchi that supply air to specific segments within each lobe.

Deeper in the bronchial tree, the segmental bronchi become smaller bronchioles. Bronchioles do not have cartilage. They are mostly made of smooth muscle. This muscle allows bronchodilation and bronchoconstriction, which changes airflow based on the body’s needs. The airways continue to branch into smaller tubes, including terminal bronchioles, respiratory bronchioles, alveolar ducts, and alveoli. The alveoli are tiny air sacs where gas exchange happens. Oxygen enters the bloodstream, and carbon dioxide leaves the body.

The airway lining also changes as the bronchial tree branches. The mucosa in the main bronchi is pseudostratified columnar epithelium. In larger bronchioles, it becomes ciliated pseudostratified columnar epithelium with goblet cells. In smaller bronchioles, it becomes ciliated simple cuboidal epithelium without goblet cells. In terminal bronchioles, it becomes nonciliated simple cuboidal epithelium. This mucosal layer helps humidify and filter inhaled air while trapping particles and pathogens.

Beneath the mucosa is the submucosa. It contains glands that make mucus, which helps trap and remove harmful particles. The submucosa also contains blood vessels, nerves, and lymphatics. Larger airways such as the trachea, primary bronchi, and lobar bronchi have cartilage for support. Farther into the bronchial tree, cartilage decreases and smooth muscle becomes more important. This shift gives the smaller airways more flexibility and control over airflow.

Several diseases can affect the bronchial tree and make breathing harder. Asthma is a chronic condition that causes airway inflammation and narrowing. Chronic obstructive pulmonary disease, or COPD, includes progressive lung diseases such as chronic bronchitis and emphysema. Other conditions include bronchitis, bronchiectasis, and bronchiolitis. Learning the structure of the human bronchi and bronchial tree helps explain how the respiratory system works.

Content elaboration: The digestive system consists of the gastrointestinal tract (alimentary canal) as well as various accessory organs including the teeth, tongue, salivary glands, liver, gallbladder and pancreas. The digestive system processes and supplies the molecules needed to sustain the living tissues within the body through the absorption of nutrients. Six major functions of the digestive system include secretion, ingestion, mechanical processing, enzymatic digestion, absorption and excretion. The lining of the digestive system protects surrounding tissues from the mechanical and enzymatic stresses of the digestive process. Processes of the digestive system include the mechanical and chemical breakdown of food into small molecules which are then absorbed by the digestive tract. Specific actions within the digestive system include mastication, peristalsis, segmentation and the release of hormones and enzymes necessary for digestion. The metabolic functions of the accessory organs play strategic roles in the breakdown of food products, the maintenance of glucose levels within the blood and the regulation of homeostasis in the body. Indigestible material is excreted as waste. Homeostatic imbalances are explored. These include, but are not limited to, conditions such as gallstones, heartburn, ulcers, dehydration, diarrhea, cirrhosis and cancers of the digestive system. Investigations are used to understand and explain the digestive system in a variety of inquiry and design scenarios that can incorporate evolutionary concepts, scientific reasoning, comparative analysis, communication skills and real-world applications.

Content elaboration: The respiratory system is comprised of the airways, lungs and diaphragm. The airways include the nasal and oral cavities, pharynx, larynx, trachea, bronchi, bronchioles and alveoli. The respiratory system transports and exchanges gases including oxygen and carbon dioxide. Processes involved in the respiratory system include respiration mechanics and gas exchange. Respiration mechanics is the process by which humans breathe and includes the movement of the diaphragm and pressure-volume relationships. Gas exchange refers to the diffusion of gas across the alveolar epithelium in the respiratory system and capillary endothelium of the cardiovascular system. Lung volumes and capacities can be measured using spirometry. Homeostatic imbalances are explored. These include, but are not limited to, asthma, chronic obstructive pulmonary disease (COPD), tuberculosis, cystic fibrosis and the effects of smoking and pollution. Investigations are used to understand and explain the respiratory system in a variety of inquiry and design scenarios that can incorporate evolutionary concepts, scientific reasoning, comparative analysis, communication skills and real-world applications.
