Processamento auditivo é a base de voz e música relacionada com o processamento. Estimulação Magnética Transcraniana (TMS) foi usado com sucesso para estudar cognitivas, sistemas sensoriais e motores, mas raramente tem sido aplicada para a audição. Aqui nós investigamos TMS combinados com ressonância magnética funcional para entender a organização funcional do córtex auditivo.
Córtex auditivo se refere ao processamento de som, que está na base da fala ou música relacionada com o processamento 1. No entanto, apesar dos recentes progressos consideráveis, as propriedades funcionais e de lateralização do córtex auditivo humano estão longe de serem totalmente compreendidos. Estimulação Magnética Transcraniana (TMS) é uma técnica não-invasiva que pode transitoriamente ou duradoura modular a excitabilidade cortical através da aplicação de pulsos localizados de campo magnético, e representa um método exclusivo de explorar a plasticidade e conectividade. Ele só recentemente começou a ser aplicada para entender a função auditiva cortical 2.
Uma questão importante no uso de TMS é que as consequências fisiológicas da estimulação são difíceis de estabelecer. Apesar de muitos estudos TMS fazer a suposição implícita de que a área alvo da bobina é a área afectada, esta necessidade não é o caso, em particular para as funções cognitivas complexas which dependem de interações em várias regiões cerebrais 3. Uma solução para este problema é combinar TMS com ressonância magnética funcional (fMRI). A idéia aqui é que fMRI irá fornecer um índice de mudanças na atividade cerebral associada com TMS. Assim, fMRI daria um meio independente de avaliar quais áreas são afetadas pela TMS e como eles são modulados 4. Além disso, a IRMf permite a avaliação de conectividade funcional, o que representa uma medida do acoplamento temporal entre regiões distantes. Assim, pode ser útil não só para medir a actividade de modulação de líquido induzido pela TMS em determinados locais, mas também do grau em que as propriedades da rede são afectados pela TMS, via quaisquer alterações observadas na conectividade funcional.
Abordagens diferentes para combinar TMS e de imagem funcional de acordo com a ordem temporal dos métodos. RM funcional pode ser aplicado antes, durante, após ou antes e depois da TMS. Recentemente, Alguns estudos intercalados TMS e fMRI, a fim de proporcionar linha de mapeamento das alterações funcionais induzidas por TMS 5-7. No entanto, esta combinação de linha tem muitos problemas técnicos, incluindo os artefactos estáticos resultantes da presença da bobina TMS na sala de scanner, ou os efeitos dos impulsos de TMS sobre o processo de formação de imagens MR. Mas, mais importante, o ruído acústico alto induzida por TMS (aumentada em comparação com a utilização padrão, devido à ressonância do scanner furo) e as vibrações de aumento da bobina (TMS causado pelas fortes forças mecânicas devido ao campo magnético estático do scanner de MR) constituem um problema crucial quando se estuda o processamento auditivo.
Esta é uma razão pela qual fMRI foi realizado antes e depois da TMS no presente estudo. Abordagens semelhantes têm sido utilizados para atingir o córtex motor 8,9, córtex pré-motor 10, principal córtex somatossensorial 11,12 e língua áreas relacionadas com a 13, Mas até agora nenhum estudo combinado TMS-fMRI investigou o córtex auditivo. O objetivo deste artigo é o de fornecer pormenores sobre o protocolo e considerações necessárias para combinar com sucesso essas duas ferramentas neurocientíficas para investigar processamento auditivo.
Anteriormente nós mostramos que TMS (EMTr) em freqüências altas e baixas (resp. 10 Hz e 1 Hz) aplicados sobre o tempo de resposta córtex auditivo modulado (RT) em uma tarefa de discriminação melodia 2. Também mostramos que a RT de modulação foi correlacionada com a conectividade funcional na rede auditivo avaliadas utilizando fMRI: quanto maior a conectividade funcional entre os córtices auditivos esquerdo e direito durante o desempenho da tarefa, maior é o efeito facilitador (isto é, redução RT) observada com a rTMS. No entanto, essas conclusões foram principalmente correlacional, como fMRI foi realizada antes da EMTr. Aqui, fMRI foi realizada antes e imediatamente após o TMS para fornecer medidas diretasda organização funcional do córtex auditivo, e mais especificamente da reorganização de plástico da rede neural auditiva que ocorre após a intervenção neural fornecida pelo TMS.
RMf combinadas e TMS aplicados sobre o córtex auditivo deve permitir uma melhor compreensão dos mecanismos cerebrais de processamento auditivo, fornecendo informações fisiológicas sobre efeitos funcionais do TMS. Este conhecimento pode ser útil para muitas aplicações de neurociência cognitiva, bem como para optimizar a aplicações terapêuticas de TMS, particularmente em distúrbios relacionados auditivo.
