Protokolls beskriver samtidiga, bilaterala bedömningen av corticomotor svar av tibialis anterior och soleus under vila och tonic frivilliga aktiveringen med hjälp av en enda puls transkraniell magnetisk stimulering och neuronavigation systemet.
Distala benmuskler få neurala input från motor kortikala områden via corticospinal tarmkanalen, som är en av de viktigaste motor fallande clearanceväg människor och kan bedömas med transkraniell magnetisk stimulering (TMS). Tanke på rollen för distala benmuskler i upprätt postural och dynamiska uppgifter, såsom promenader, en växande forskningsintresse för bedömning och modulering av de corticospinal trakter i förhållande till funktionen av dessa muskler har vuxit fram under det senaste decenniet. Metodologiska parametrar som används i tidigare arbete har dock varierat i studier att göra tolkningen av resultat från tvärsnittsdata och longitudinella studier mindre robust. Därför kan användning av ett standardiserat TMS specifika för bedömningen av benmusklerna corticomotor respons (CMR) för direkt jämförelse av resultat över studier och kohorter. Syftet med denna uppsats är att presentera ett protokoll som ger dig flexibilitet att samtidigt bedöma den bilaterala CMR av två huvudsakliga fotled antagonistiska muskler, den tibialis anterior och soleus, med enda puls TMS med ett neuronavigation system. Detta protokoll är tillämpligt medan undersökta muskeln är antingen fullt ut avslappats eller isometrically iklätts en definierad andel av maximal isometrisk frivillig kontraktion. Med varje försökspersonens strukturella MRI med neuronavigation systemet säkerställer korrekt och exakt positionering av spolen över benet kortikala framställningarna under bedömning. Inkonsekvensen i CMR härledda åtgärder får beskriver det här protokollet också en standardiserad beräkning av dessa åtgärder med hjälp av automatiska algoritmer. Om detta protokoll inte bedrivs under upprätt postural eller dynamiska uppgifter, kan det användas för att bedöma bilateralt något par av benmusklerna, antingen antagonistiska eller synergistisk, i både neurologiskt intakta och nedsatt ämnen.
Tibialis anterior (TA) och soleus (SOL) är fotled antagonistiska muskler belägna i främre och bakre facket av underbenet, respektive. Båda musklerna är uniarticular, medan den huvudsakliga funktionen av TA och SOL är att dorsiflex och plantarflex det talocrural gemensamt, respektive1. Dessutom är TA mer funktionellt för långa muskel utflykter och mindre viktig för produktion av kraft, medan SOL är en antigravity muskel som utformats för att generera hög kraft med liten utflykt av muskel2. Båda musklerna är särskilt relevant under upprätt postural och dynamiska uppgifter (t.ex. promenader)3,4. Angående kontroll av nerver få motorneuron pooler av både muskler neurala drive från hjärnan via motorn fallande vägar5,6, förutom varierande grad av sensorisk bilresa.
Huvudmotorn fallande pathway är den corticospinal tarmkanalen, som påbörjar från det primära, premotor och kompletterande motor och avslutar i spinal motorneuron pool7,8. I människor, kan funktionella tillståndet för denna trakt (corticomotor svar – CMR) lämpligen bedömas med transkraniell magnetisk stimulering (TMS), en icke-invasiv hjärnan stimulering verktyg9,10. Sedan införandet av TMS och med tanke på deras funktionella betydelse under upprätt postural aktivitet och promenader, har CMR av TA och SOL utvärderats i olika kohorter och uppgifter11,12,13,14 ,15,16,17,18,19,20,21,22,23 ,24,25,26,27,28,29,30,31,32 .
I motsats till bedömningen av CMR i övre-extremitet muskler33, har ingen universell TMS-protokollet fastställts för bedömningen av CMR i nedre-extremitet muskler. På grund av avsaknaden av ett etablerat protokoll och den stora metodologiska variationen över de tidigare studierna (t.ex. typ av spolen, användning av neuronavigation, tonic aktivering, testning sida och muskel, använda och beräkning av CMR åtgärder, etc. ), tolkning av resultat över studier och kohorter kan vara besvärliga, komplicerade och felaktig. Eftersom åtgärderna som är funktionellt relevanta i olika motoriska aktiviteter, gör ett etablerat TMS protokoll som är specifika för nedre extremitet CMR bedömning att motor neuroforskare och rehabilitering forskare för att systematiskt bedöma CMR i dessa muskler över sessioner och olika kohorter.
Syftet med detta protokoll är därför att beskriva bilaterala bedömningen av TA och SOL CMR med enda puls TMS och neuronavigation system. I motsats till tidigare arbete syftar detta protokoll till att maximera noggrannheten i den experimentella procedurer, datainsamling och dataanalys genom att anställa metodologiska faktorer som optimerar giltigheten och varaktigheten av experimentet och standardisera CMR bedömning av dessa två nedre extremiteter muskler. Med tanke på att CMR av en muskel beror på om muskeln är helt avslappnad eller aktiveras delvis, beskriver det här protokollet hur de TA och SOL CMR kan bedömas under vila och tonic frivilliga aktiveringen (TVA). Följande avsnitt kommer att noggrant beskriva detta protokoll. Slutligen, representativa uppgifter presenteras och diskuteras. Protokollet beskrivs här härstammar från som i Charalambous et al. 201832.
Med tanke på det växande intresset för hur motoriska cortex bidrar till motorisk kontroll av benmusklerna under dynamiska uppgifter i olika kohorter, behövs ett standardiserat TMS-protokoll som beskriver en grundlig bedömning av dessa muskler. Därför, för första gången, detta protokoll ger standardiserade metodologiska förfaranden på bilaterala bedömning av två fotled antagonistiska muskler, SOL och TA, under två muskel stater (resten och TVA) med en enda puls TMS med neuronavigation.
