Hier presenteren we een protocol om magnetische resonantie (MR) beelden te verkrijgen van multiple sclerose (MS) patiëntenhersenen bij 7,0 Tesla. Het protocol omvat de voorbereiding van de opstelling inclusief de radiofrequentiespoelen, gestandaardiseerde interviewprocedures met MS-patiënten, positionering van het onderwerp in de MR-scanner en MR-gegevensverzameling.
Het algemene doel van dit artikel is om een state-of-the-art ultrahigh field (UHF) magnetische resonantie (MR) protocol van de hersenen bij 7,0 Tesla bij multiple sclerose (MS) patiënten te demonstreren. MS is een chronische inflammatoire, demyeliniserende, neurodegeneratieve ziekte die wordt gekenmerkt door witte en grijze stof laesies. Detectie van ruimtelijk en temporeel verspreide T2-hyperintense laesies door het gebruik van MRI bij 1,5 T en 3 T vormt een cruciaal diagnostisch hulpmiddel in de klinische praktijk om een nauwkeurige diagnose van MS vast te stellen op basis van de huidige versie van de McDonald-criteria van 2017. De differentiatie van MS-laesies van witte stoflaesies van andere oorsprong kan echter soms een uitdaging zijn vanwege hun lijkende morfologie bij lagere magnetische veldsterkten (meestal 3 T). Ultrahigh field MR (UHF-MR) profiteert van een verhoogde signaal-ruisverhouding en verbeterde ruimtelijke resolutie, beide de sleutel tot superieure beeldvorming voor nauwkeurigere en definitieve diagnoses van subtiele laesies. Vandaar dat MRI bij 7,0 T bemoedigende resultaten heeft laten zien om de uitdagingen van MS-differentiële diagnose te overwinnen door MS-specifieke neuroimaging markers te bieden (bijv. Centraal aderteken, hypointense rimstructuren en differentiatie van MS grijze stof laesies). Deze markers en andere kunnen worden geïdentificeerd door andere MR-contrasten dan T1 en T2 (T2*, fase, diffusie) en verbeteren aanzienlijk de differentiatie van MS-laesies van die welke voorkomen bij andere neuro-inflammatoire aandoeningen zoals neuromyelitis optica en Susac-syndroom. In dit artikel beschrijven we onze huidige technische aanpak voor het bestuderen van cerebrale witte en grijze stof laesies bij MS-patiënten bij 7,0 T met behulp van verschillende MR-acquisitiemethoden. Het up-to-date protocol omvat de voorbereiding van de MR-opstelling inclusief de radiofrequentiespoelen op maat voor UHF-MR, gestandaardiseerde screening, veiligheids- en interviewprocedures met MS-patiënten, patiëntpositionering in de MR-scanner en acquisitie van speciale hersenscans op maat voor het onderzoeken van MS.
Multiple sclerose (MS) is de meest voorkomende chronische inflammatoire en demyeliniserende ziekte van het centrale zenuwstelsel (CZS) die uitgesproken neurologische invaliditeit bij jongere volwassenen veroorzaakt en leidt tot langdurige invaliditeit1,2. Het pathologische kenmerk van MS is de accumulatie van demyeliniserende laesies die optreden in de grijze en witte stof van de hersenen en ook diffuse neurodegeneratie in de hele hersenen, zelfs in normaal lijkende witte stof (NAWM)3,4. MS-pathologie suggereert dat ontsteking weefselbeschadiging in alle stadia van de ziekte veroorzaakt, zelfs tijdens de progressieve stadia van de ziekte5. De eerste klinische manifestaties van MS gaan vaak gepaard met omkeerbare episodes van neurologische tekorten en worden een klinisch geïsoleerd syndroom (CIS) genoemd, wanneer ze alleen suggestief zijn voor MS6,7. Bij afwezigheid van een duidelijk CIS moet voorzichtigheid worden betracht bij het stellen van een MS-diagnose: de diagnose moet worden bevestigd door follow-up en het starten van langdurige ziektemodificerende therapieën moet worden uitgesteld, in afwachting van aanvullend bewijs8.
Magnetische resonantie beeldvorming (MRI) is een onmisbaar hulpmiddel bij het diagnosticeren van MS en het monitoren van ziekteprogressie9,10,11. MRI bij magnetische veldsterkten van 1,5 T en 3 T vertegenwoordigt momenteel een cruciaal diagnostisch hulpmiddel in de klinische praktijk om spin-spin relaxatietijd gewogen (T2)hyperintense laesies te detecteren en een nauwkeurige diagnose van MS vast te stellen op basis van de huidige versie van de McDonald-criteria van 20178. Diagnostische criteria voor MS benadrukken de noodzaak om de verspreiding van laesies in ruimte en tijd aan te tonen en alternatieve diagnoses uit te sluiten8,12. Contrastversterkte MRI is de enige methode om acute ziekte en acute ontsteking te beoordelen8maar toenemende bezorgdheid over mogelijke gadolinium hersenafzetting op lange termijn zou mogelijk de contrasttoepassing als een belangrijk diagnostisch hulpmiddel kunnen beperken13,14,17. Bovendien kan de differentiatie van MS-laesies van witte stoflaesies van andere oorsprong soms een uitdaging zijn vanwege hun op elkaar lijkende morfologie bij lagere magnetische veldsterkten.
Hoewel MRI zeker het beste diagnostische hulpmiddel is voor MS-patiënten, moeten MR-onderzoeken en protocollen de richtlijnen volgen van de Magnetic Resonance Imaging in MS-groep (MAGNIMS) in Europa18,19 of het Consortium of Multiple Sclerosis Centers (CMSC) in Noord-Amerika20 voor de diagnose, prognose en monitoring van MS-patiënten. Gestandaardiseerde kwaliteitscontrolestudies in overeenstemming met de nieuwste richtlijnen in verschillende ziekenhuizen en landen zijn ook cruciaal21.
MRI-protocollen op maat voor MS-diagnose en ziekteprogressiemonitoring omvatten meerdere MRI-contrasten, waaronder contrast dat wordt beheerst door de longitudinale ontspanningstijd T1,de spin-spin-ontspanningstijd T2, de effectieve spin-spin-ontspanningstijd T2* en diffusiegewogen beeldvorming (DWI)22. Harmonisatie-initiatieven leverden consensusrapporten op voor MRI in MS om te evolueren naar gestandaardiseerde protocollen die klinische vertaling en vergelijking van gegevens tussen sites23,24,25 vergemakkelijken. T2 -gewogenbeeldvorming is goed ingeburgerd en wordt vaak gebruikt in de klinische praktijk voor de identificatie van witte stof (WM) laesies, die worden gekenmerkt door hyperintense uiterlijk26,27. Hoewel het een belangrijk diagnostisch criterium is voor MS28,correleert de WM-laesiebelasting slechts zwak met klinische invaliditeit, vanwege het gebrek aan specificiteit voor de ernst van de laesie en de onderliggende pathofysiologie26,27,29. Deze waarneming heeft geleid tot verkenningen naar parametrische kartering van de transversale relaxatietijd T2 30. T2*-gewogen beeldvorming is steeds belangrijker geworden bij het afbeelden van MS. Het centrale aderteken in T 2 *gewogenMRI wordt beschouwd als een specifieke beeldvormingsmarker voor MS-laesies27,31,32,33. T2* is gevoelig voor ijzerafzetting34,35, die betrekking kan hebben op ziekteduur, activiteit en ernst36,37,38. T2* werd ook gemeld om hersenweefselveranderingen te weerspiegelen bij patiënten met kleine tekorten en vroege MS, en kan dus een hulpmiddel worden om de ontwikkeling van MS al in een vroeg stadium te beoordelen39,40.
Verbeteringen in MRI-technologie beloven veranderingen in het CZS van MS-patiënten beter te identificeren en clinici een betere gids te bieden om de nauwkeurigheid en snelheid van een MS-diagnose te verbeteren11. Ultrahigh field (UHF, B0≥7.0 T) MRI profiteert van een toename van de signaal-ruisverhouding (SNR) die kan worden geïnvesteerd in verbeterde ruimtelijke of temporele resoluties, beide de sleutel tot superieure beeldvorming voor nauwkeurigere en definitieve diagnoses41,42. Transmissieveld (B1+) inhomogeniteiten die een nadelig kenmerk zijn van de 1H-radiofrequentie die wordt gebruikt bij ultrahoge magnetische velden43 zou baat hebben bij meerkanaalstransmissie met behulp van parallelle transmissie (pTx) RF-spoelen en RF-pulsontwerpbenaderingen die de B1+ homogeniteit verbeteren en zo een uniforme dekking van de hersenen44vergemakkelijken .
Met de komst van 7,0 T MRI hebben we meer inzicht gekregen in demyeliniserende ziekten zoals MS met betrekking tot verhoogde gevoeligheid en specificiteit van laesiedetectie, centrale adertekenidentificatie, leptomeningeale verbetering en zelfs met betrekking tot metabole veranderingen45. MS-laesies zijn al lang aangetoond uit histopathologische studies om zich te vormen rond aderen en venules46. De periveneuze verdeling van laesies (centraal aderteken) kan worden geïdentificeerd met T2* gewogen MRI46,47,48 bij 3,0 T of 1,5 T, maar kan het best worden geïdentificeerd met UHF-MRI bij 7,0 T49,50,51,52. Anders dan het centrale aderteken, heeft UHF-MRI bij 7,0 T MS-specifieke markers verbeterd of blootgelegd, zoals hypointense velgstructuren en differentiatie van MS-grijze stoflaesies53,54,55,56. Een betere afbakening van deze markers met UHF-MRI belooft enkele van de uitdagingen van het onderscheiden van MS-laesies te overwinnen van die welke voorkomen bij andere neuro-inflammatoire aandoeningen zoals Susac-syndroom53 en neuromyelitis optica54, terwijl ook gemeenschappelijke pathogenetische mechanismen worden geïdentificeerd in andere aandoeningen of varianten van MS zoals Baló’s concentrische sclerose57,58.
Dit artikel erkent de uitdagingen en kansen van UHF-MRI voor de detectie en differentiatie van MS-laesies en beschrijft onze huidige technische aanpak om cerebrale witte en grijze stoflaesies bij MS-patiënten bij 7,0 T te bestuderen met behulp van verschillende beeldvormingstechnieken. Het up-to-date protocol omvat de voorbereiding van de MR-opstelling inclusief de radiofrequente (RF) spoelen op maat van de UHF-MR, gestandaardiseerde screening, veiligheids- en interviewprocedures met MS-patiënten, patiëntpositionering in de MR-scanner en acquisitie van hersenscans gewijd aan MS. Het artikel is bedoeld om beeldvormingsexperts, basisonderzoekers, klinische wetenschappers, translationele onderzoekers en technologen met alle niveaus van ervaring en expertise, variërend van stagiairs tot geavanceerde gebruikers en toepassingsexperts, te begeleiden op het gebied van UHF-MRI bij MS-patiënten, met als uiteindelijk doel synergetisch technologieontwikkeling en klinische toepassing te verbinden tussen disciplinaire domeinen.
Het hier gepresenteerde protocol beschrijft een reeks MRI-sequenties met verschillende contrasten die meestal worden gebruikt bij het onderzoeken van MS-patiënten op 7,0 T. Samen met opkomende technologische ontwikkelingen vormen ze de basis voor verkenningen naar meer geavanceerde toepassingen in metabole of functionele beeldvorming.
