缺血性心脏病是全球范围内导致发病和死亡的主要原因，仅在美国就影响至少2050万人1,2,3。若缺乏充分的灌注，持续的缺氧将导致心肌细胞（CM）死亡和心肌功能障碍。心肌缺血主要通过经皮介入、溶栓治疗或冠状动脉旁路移植术进行血运重建治疗。然而，血运重建是一把双刃剑，可能因活性氧物质的释放而造成额外损伤4,5。尽管再灌注策略已取得巨大成功，但在缺血损伤期间或再灌注后限制心肌细胞丢失的方法仍难以实现。尽管许多挽救梗死组织的尝试在缺血-再灌注动物模型中证明有效，但这些疗法在人类临床试验中大多未能显示出显著益处，因此迫切需要建立人类模型以评估保护或补充心肌细胞的治疗方法6,7,8,9,10,11,12。
用于模拟心脏损伤的人类研究平台包括原代组织和人诱导多能干细胞（hiPSC）来源的系统13,14。由于原代组织受限于样本可获得性，hiPSC系统已成为心脏疾病建模的主要人类平台，具有遗传可操作性和可扩展性的显著优势15,16,17,18,19。传统的二维hiPSC来源心肌细胞（hiPSC-CM）培养体系受限于所得心肌细胞的不成熟性、缺乏复杂的组织结构以及无法重现人类心脏中完整的细胞多样性15,20。为克服这些局限，工程化三维心脏组织（EHT）的应用日益广泛20,21,22,23,24,25,26。EHT技术包括将预先分化的 cardiac 细胞混合形成类球体24，或将已分化的 cardiac 细胞接种至支架或模具中21,22,27。尽管EHT能更有效地模拟异质性细胞相互作用，且相较于二维培养具有相对更高的成熟度，但其构建和维持过程对定制支架或生物反应器的需求目前限制了其可扩展性21,27,28。
自组装心脏类器官（hCOs），或称心形类器官（cardioids），是一种新型且强大的 体外 人类模型系统20,25,26,29人源心脏类器官（hCOs）具有多项优势。首先，它们可包含多种细胞类型，如内皮细胞、成纤维细胞和心外膜细胞，而无需采用多种专门的定向分化方案。26,29,30其次，它们的大部分分化过程发生在三维悬浮培养中，无需使用特殊的模具或组织基质25,26,29,30最后，hCOs具有典型的球形形态和中央腔，便于进行形态学分析。因此，hCOs在保留部分EHTs组织复杂性的同时，具备与二维培养系统相似的可扩展性。hCOs已被用于药物毒性测试、患者特异性疾病建模以及心脏发育过程的模拟研究20,26,30,31,32,33,34,35,36,37,38,39尽管前景广阔，类器官相比工程化心脏组织（EHTs）仍存在若干局限性，例如无法直接测量其收缩力输出。此外，目前所描述的类器官模型中缺乏内源性免疫细胞群体，尽管已有研究成功将巨噬细胞引入类器官。然而，类器官作为一种新兴的人源模型系统，正迅速发展和完善20,26,30,31,32,33,34,35,36,37,38,39.
迄今为止，利用人源心脏类器官（hCOs）构建心脏损伤模型的研究仍较为有限。已有研究报道了冷冻损伤模型，但其损伤程度变异度高，且需对每个hCO进行单独损伤处理，限制了模型的可扩展性29。本文描述了一种用户友好的实验方案，用于生成并通过对缺氧-复氧（H/R）损伤诱导自组装hCOs。与其他hCOs生成方案相比，本方案采用简化的分化培养基，并整合了Hofbauer等29提出的2D-3D中胚层分化策略。H/R损伤操作可在hCOs开始搏动后的任意时间进行，通常发生在分化第8天（D8）至第11天（D11）之间。H/R处理时间可在6小时至24小时之间梯度调整，以实现从轻微到近乎完全心肌细胞丢失的不同损伤程度。此外，hCOs在H/R损伤后通常会形成富含波形蛋白（Vimentin+）细胞的胶原核心结构，其严重程度亦随H/R持续时间延长而增加。该高通量、可扩展的H/R损伤hCO模型，有望满足开发和测试促进损伤后心肌细胞恢复或再生新疗法所需的人源模型需求。

A Hypoxia-Reoxygenation Injury Model in Self-Assembling Human Cardioids

若不及时恢复血流，缺血性心脏病将导致心肌细胞不可逆地丧失、瘢痕形成以及心脏功能下降。目前尚缺乏能够在人体组织中模拟缺氧-复氧损伤并评估潜在治疗干预手段的有效方法。人源心脏类器官（hCOs）是一种新兴的模型系统，但由于其体积微小、需要悬浮培养，以及分化效率存在差异，初学者在开展hCOs相关研究时可能面临挑战。本实验方案提供了一套用户友好的操作指南，用于生成和维持由人诱导多能干细胞（iPSC）衍生的自组装hCOs，以及对hCOs进行处理和组织学分析的方法；同时介绍了一种通过缺氧-复氧诱导hCOs损伤的策略，以模拟人类缺血/再灌注损伤中出现的纤维化和细胞凋亡现象。本方案为将hCOs作为缺血-再灌注损伤动物模型的补充工具提供了指导，也可用于识别和验证可能促进人类心肌功能恢复的因素。&#160;

按照上述方案，应可获得结构良好的人源心脏类器官（hCOs），其通常在分化第7至12天（D7-12）开始出现搏动。当hCOs发育超过第25天后，可能需要电刺激起搏才能观察到自发性收缩。缺氧/复氧（H/R）损伤可在hCOs开始搏动后的任意时间进行——我们通常在15天龄（D15）或更晚阶段的hCOs上实施损伤实验。
损伤确认 
与人类心肌梗死类似，hCOs 的缺氧/复氧（H/R）损伤会导致细胞死亡。临床上，可通过检测细胞死亡后释放到循环中的蛋白质（如乳酸脱氢酶（LDH）、心肌肌钙蛋白、肌红蛋白或肌酸激酶）来评估心肌损伤程度。同样，在 hCOs 发生 H/R 损伤后，可轻松检测其培养基中 LDH 的释放情况。图 4A 展示了使用商业化的基于微孔板的生物发光法检测 D21-D28 hCOs 在 H/R 损伤后 24 小时 LDH 释放的代表性结果。与未受损的 hCOs 相比，在经历 6 小时缺氧和 24 小时复灌注后，细胞外 LDH 活性显著升高（1518 ± 454.9 RLU 对比 854...

我们的研究致力于探索帮助心脏在损伤后再生的新方法。为此，我们构建了可自组装的心脏类器官，并开展缺氧-复氧研究。该方案可用于研究损伤应答、促进再生的治疗干预措施，并可用于患者特异性的疾病建模。首先，吸除D1心肌中胚层诱导培养基，加入足量的细胞解离溶液，完全覆盖人诱导性多能干细胞来源的心肌中胚层细胞。随后将培养板置于37摄氏度的培养箱中孵育5分钟。使用移液器向每孔加入与解离液体积相等的D2心肌中胚层培养基，轻柔吹打数次以使贴壁细胞脱落，将细胞悬液转移至15毫升锥形管中。使用离心机，在室温下以300G离心3分钟，使细胞沉淀。将沉淀重悬于1毫升D2心肌中胚层定向培养基中，该培养基含有5 µM IWP-2和50 µg/mL抗坏血酸，溶剂为RB minus。在微量离心管中，将10 µL台盼蓝与10 µL细胞悬液混合。使用首选的计数方法对细胞进行计数，然后将细胞用添加了2 µM thiazovivin的D2心脏中胚层定向培养基稀释至每毫升100,000个细胞的浓度。将细胞悬液转移至适当大小的试剂储液槽中。随后，使用多通道移液器将100 µL细胞悬液加入超低吸附96孔板的每个孔中，使每孔终浓度为10,000个细胞。在室温下以500G离心5分钟，以诱导人源心脏类器官在悬浮培养中形成。观察每个孔中是否形成一个大而边界清晰的细胞团。如果细胞团边界不清晰，需重复离心。24小时后，每孔准备含有5 µM IWP-2和50 µg/mL抗坏血酸的RB减培养基10毫升。使用多通道移液器，向每个孔中加入100 µL D3培养基，无需移除原有培养基。次日，从每个孔中移除100 µL D3培养基。为降低吸液风险，将培养板倾斜至45度角，并将多通道移液器垂直（90度角）紧贴培养板侧壁。将移液器牢固地贴住孔壁，缓慢吸液。移除培养基后，加入100 µL含5 µM IWP-2的RB减液D4培养基。使用剪去尖端的1,000微升移液枪，将人源心脏类器官转移至1.7毫升微量离心管中。让类器官沉降至管底，轻轻吸除多余培养基。加入磷酸盐缓冲液洗涤类器官一次。等待数秒使其沉降，并轻轻去除多余的 PBS。接着加入 4% 多聚甲醛以固定类器官，将离心管置于室温下 30 分钟进行固定。然后将所需数量的固定后类器官转移至 1.7 毫升微量离心管中。移除固定液，并用含有0.1% Tween 20的PBS洗涤类器官。将抗体在染色液中稀释至所需浓度，配制一抗溶液。使用移液器向每个微量离心管中加入50至100微升一抗溶液，确保类器官完全被覆盖。孵育后按前述方法进行洗涤，然后将样本置于摇床上，用染色液稀释的二抗在4°C下孵育24小时。孵育结束后，按前述方法进行洗涤。随后，使用剪去尖端的移液枪头，将类器官转移至带正电荷的玻片上。轻轻吸去含 Tween 20 的 PBS，加入数滴封片剂以覆盖类器官。小心地将盖玻片盖在类器官上，使其展平。随后进行荧光显微镜观察。将固定的类器官在4 °C条件下于30%蔗糖溶液中孵育过夜后，转移至塑料模具中，并向模具内加入少量包埋剂（OCT化合物），用量以刚好覆盖类器官为宜。使用精细镊子或移液器吸头，轻轻调整类器官在包埋剂中的位置，使其分散开且远离模具边缘。尽可能将类器官置于模具底部，以最大化单个冷冻切片中所含类器官的数量。将模具放入含有干冰层的泡沫塑料保温箱中，以冷冻最佳切割温度包埋剂的初始层。等待数分钟，待包埋剂完全硬化后，将模具移至实验台面。继续添加最佳切割温度包埋剂，直至模具完全填满。在将模具放回干冰中完成冷冻之前，需等待第一层完全融化。冷冻完成后，将模具储存在负80摄氏度环境中，或直接进行冷冻切片以获得6微米厚的切片。使用移液器将每块96孔人源心脏类器官板中的5毫升缺氧培养基转移至细胞培养瓶中。将烧瓶放入缺氧室中，并用由5%二氧化碳和95%氮气组成的无氧气体冲洗缺氧室三分钟。将缺氧室置于37°C的培养箱中至少一小时，然后开始诱导缺氧。使用多通道移液器，从人源心脏类器官板的所有孔中移除约150 µL培养基。然后，使用 200 微升移液器小心吸出每个孔中剩余的培养基，注意不要扰动类器官。向每个孔中加入 50 微升缺氧培养基。将人源心脏类器官培养板放入缺氧舱中。用由5%二氧化碳和95%氮气组成的无氧气体冲洗培养 chamber 三分钟。将缺氧 chamber 置于37°C的培养箱中六小时。为启动复氧，将人源心脏类器官从缺氧 chamber 中取出。使用多通道移液器，向每个孔中加入 150 微升预热的 RB+ 培养基。最后，将培养板置于 37 摄氏度的培养箱中，进行至少三小时的再氧合，然后再进行后续检测。在经历六小时缺氧和 24 小时再灌注后，与未损伤的对照组相比，受损的人类心脏类器官中细胞外乳酸脱氢酶活性显著升高。经低氧处理的心脏类器官表现出TUNEL阳性的心肌细胞核，且损伤类器官中TUNEL阳性心肌细胞核的比例显著高于未损伤对照组。随着再氧合前低氧时间的延长，瘢痕形成呈时间依赖性增加。再灌注24小时后，69%的未损伤人源心脏类器官仍有搏动，而所有损伤类器官均无搏动。未受损的人类心脏类器官在五个感兴趣区域均表现出同步的钙瞬变，其峰值出现时间一致且波形相似。受损的人类心脏类器官则未表现出协调性收缩，而是在各感兴趣区域呈现不规则的自发性钙瞬变。该方案可使研究人员在iPSC来源的人类心脏类器官中模拟心肌梗死。确保可重复性损伤的最大技术挑战在于完全去除现有的培养基。我们未来的研究旨在鉴定能够促进人类心脏类器官模型中组织再生的因素。
