Dieser Artikel beschreibt neue Ansätze zur Messung und Stärkung funktional spezifischer neuronaler Bahnen mit transkranieller magnetischer Stimulation. Diese fortschrittlichen nichtinvasiven Methoden zur Hirnstimulation können neue Möglichkeiten für das Verständnis von Beziehungen zwischen Gehirn und Verhalten und die Entwicklung neuer Therapien zur Behandlung von Hirnerkrankungen bieten.
Das Verständnis von Interaktionen zwischen Hirnbereichen ist wichtig für die Untersuchung von zielgerichtetem Verhalten. Funktionelle Neuroimaging der Gehirnkonnektivität hat wichtige Einblicke in grundlegende Prozesse des Gehirns wie Kognition, Lernen und motorische Kontrolle geliefert. Dieser Ansatz kann jedoch keine kausalen Beweise für die Einbeziehung von Hirninteressengebieten liefern. Transkranielle magnetische Stimulation (TMS) ist ein leistungsfähiges, nichtinvasives Werkzeug zur Untersuchung des menschlichen Gehirns, das diese Einschränkung überwinden kann, indem es die Gehirnaktivität vorübergehend verändert. Hier stellen wir die jüngsten Fortschritte mit einer gekoppelten, dualen TMS-Methode mit zwei Spulen hervor, die in verschiedenen Aufgabenkontexten kortikokortikale Wechselwirkungen im menschlichen Motorsystem kausal sonden. Darüber hinaus beschreiben wir ein tmS-Protokoll mit zwei Standorten, das auf kortikalgepaarter assoziativer Stimulation (cPAS) basiert, die die synaptische Effizienz in zwei miteinander verbundenen Hirnbereichen vorübergehend verbessert, indem wiederholte Paare kortikaler Reize mit zwei Spulen angewendet werden. Diese Methoden können ein besseres Verständnis der Mechanismen, die der kognitiv-motorischen Funktion zugrunde liegen, sowie eine neue Perspektive auf die Manipulation bestimmter neuronaler Bahnen in einer gezielten Weise, um Gehirnkreise zu modulieren und das Verhalten zu verbessern. Dieser Ansatz kann sich als ein wirksames Werkzeug erweisen, um anspruchsvollere Modelle von Gehirn-Verhaltens-Beziehungen zu entwickeln und die Diagnose und Behandlung vieler neurologischer und psychiatrischer Störungen zu verbessern.
Nichtinvasive Hirnstimulation ist ein vielversprechendes Bewertungsinstrument und Behandlung für viele neurologische Erkrankungen, wie Parkinson,-Alzheimer-Krankheit, und Schlaganfall1,2,3,4. Es gibt anhäufende Beweise, die den Zusammenhang zwischen den Verhaltenserscheinungen neurologischer Erkrankungen und Anomalien der kortikalen Erregbarkeit, Neuroplastizität, kortikokortikalen und kortikosubkortikalen Konnektivität5,6. Daher können Grundkenntnisse über die Dynamik des Gehirnnetzwerks und die Plastizität bei neurologischen Erkrankungen unschätzbare Einblicke in die Diagnose, das Fortschreiten und die Reaktion auf die Therapie liefern. Funktionelle Magnetresonanztomographie(fMRT) ist ein nützliches Werkzeug, um die komplexen Beziehungen zwischen Gehirn und Verhalten in gesunden und kranken Gehirnnetzwerken zu verstehen und hat das Potenzial, die Behandlung auf der Grundlage einer Netzwerkperspektive7,8,9zu verbessern. F MRTist jedoch korrelationaler Natur und kann weder einen kausalen Zusammenhang zwischen Gehirnfunktion und Verhalten bieten noch funktionelle Konnektivität manipulieren, um abnormale neuronale Schaltkreise im Zusammenhang mit Verhaltensstörungen bei Patienten10,11,12wiederherzustellen. Transkranielle magnetische Stimulation (TMS) kann sowohl kausal messen und modulieren menschliche Gehirnfunktion und Verhalten in Gesundheit und Krankheit3,13,14,15.
TMS ist eine sichere, nichtinvasive Methode zur Stimulierung des menschlichen Gehirns16,17und kann verwendet werden, um Plastizität zu induzieren und zu messen18. Diese Methode kann unser Verständnis von kausalen Zusammenhängen zwischen einzelnen Gehirnbereichen und Verhalten fördern10,11,12,19und ihre spezifischen funktionellen Interaktionen mit anderen Knoten eines Gehirnnetzwerks20,21,22,23. Bisher konzentrierten sich die meisten Studien auf das menschliche Motorsystem, da TMS im Handbereich des Motorkortex (M1) motorisch evozierte Potenziale (MEP) als physiologische Auslesungen für Veränderungen im Zusammenhang mit dem Motorverhalten erzeugen kann.24, die Untersuchung verschiedener hemmender und erregender Schaltkreise auf Systemebene im menschlichen Gehirn25. Jüngste Fortschritte mit einem Konditionierungstest TMS Ansatz mit zwei Spulen zeigen, dass es möglich ist, funktionelle Wechselwirkungen zwischen verschiedenen kortikalen Bereichen zu messen. Im Motorsystem zeigen dual-siteTMS-Experimente, dass sich Eingänge aus kortikalen Bereichen, die mit M1 verbunden sind, mit Aufgabenanforderungen, Alter oder Krankheit ändern können.14,26. Die Seminale Arbeit von Ferbert und Kollegen hat herausgefunden, dass die Anwendung eines Konditionierungsreizes auf M1 vor einem Testreiz des anderen M1 zu einer Hemmung der MEP-Amplitude führen kann, einem Phänomen, das als Kurzintervall-Interhemisphärische Hemmung (SIHI) bekannt ist.28. Eine Reihe von TMS-Studien mit diesem Ansatz haben auch gezeigt, dass M1 stark mit dem kontralateralen M1, ventralen prämotorischen Kortex (PMv), dorsalem prämotorischen Kortex (PMd), ergänzender Motorfläche (SMA), Pre-SMA, primärem sensorischen Kortex (S1), dorsolateraler präfrontaler Kortex (DLPFC) und posterior parietal cortex (PPC) im Ruhezustand27,28,29,30,31,32,33,34,35,36,37,38,39,40,41,42. Interessanterweise ist die Wirkung der Stimulation aus diesen kortikalen Bereichen auf die motorische erpresstechnische Erregung anatomisch, zeitlich und funktionell spezifisch für die laufende Hirnaktivität während der Vorbereitung einer Bewegung (staats- und kontextabhängig43,44,45,46,47,48,49,50,51,52,53,54,55,56,57,58,59,60,61,62,63,64,65,66,67,69). Jedoch, nur sehr wenige Studien mit dual-site TMS haben Muster der funktionellen kortikokortikalen Konnektivität mit motorischen und kognitiven Beeinträchtigungen bei Patienten mit Hirnerkrankungen charakterisiert70,71,72. Dies bietet Möglichkeiten, neue Methoden zur Beurteilung und Behandlung von motorischen und kognitiven Störungen zu entwickeln.
Mit dieser Technik wurde auch festgestellt, dass wiederholte Paare von kortikalen TMS, die auf kortikale Bereiche angewendet werden, die mit M1 verbunden sind, wie kontralaterale M168,69,70, PMv76,77,78, SMA71und PPC80,81,82 Veränderungen der synaptischen Effizienz in spezifischen neuronalen Bahnen auf der Grundlage des Hebbian-Prinzips der assoziativen Plastizität83 induzieren können. ,84,85,86 und verbessern die Verhaltensleistung72,73,74. Dennoch haben nur wenige Studien diesen Ansatz verwendet, um Schaltung und Plastizität Dysfunktion bei neurologischen Erkrankungen2,75,76,77,78,79,80,81,82,83,84,90,91,92, 93,94,95,96. Es bleibt zu zeigen, ob die Stärkung funktionell spezifischer neuronaler Bahnen mit TMS die Aktivität in dysfunktionalen Schaltkreisen wiederherstellen kann, oder ob die prospektive Stärkung intakter Schaltkreise die Widerstandsfähigkeit97 in Gehirnnetzwerken erhöhen kann, die die motorische und kognitive Funktion über die gesamte Lebensdauer und bei Krankheiten unterstützen. Das fehlende grundlegende Verständnis der neuronalen Mechanismen, die neurologischen Störungen zugrunde liegen, und die Auswirkungen der Stimulation auf miteinander verbundene dysfunktionale Gehirnnetzwerke schränken die aktuelle Behandlung ein.
Trotz seiner Fähigkeit, TMS hat noch ein Standard-Teil des Armamentarium der Neurowissenschaften und klinische Werkzeuge für das Verständnis von Gehirn-Verhalten-Beziehungen, Pathophysiologie von Hirnerkrankungen, und die Wirksamkeit der Behandlung. Um sein Potenzial auszuschöpfen und seine groß angelegte Anwendung zu unterstützen, ist die Standardisierung von TMS-Methoden daher wichtig, da es wahrscheinlicher ist, die Strenge zukünftiger TMS-Experimente und die Reproduzierbarkeit in unabhängigen Laboratorien zu erhöhen. In diesem Artikel wird erläutert, wie TMS zum Messen und Bearbeiten funktionaler Interaktionen verwendet werden kann. Hier beschreiben wir diese Technik im Motorsystem (z.B. Parieto-Motorweg44) durch Messung von TMS-basierten Ausgangsmessungen (z. B. MdEP), bei denen die Methode am besten verstanden wird. Es ist jedoch wichtig zu beachten, dass dieses Protokoll auch an die zielweise funktionelle Kopplung anderer subkortikaler85,Kleinhirn86,87und kortikaler Bereiche angepasst werden kann. 73,74,88 Darüber hinaus können Neuroimaging-Techniken wie EEG89,90,91 und fMRI92,93 verwendet werden, um die TMS-induzierten Veränderungen der Aktivität und Konnektivität zu bewerten26,94. Abschließend schlagen wir vor, dass die Untersuchung der funktionellen Einbeziehung der kortikalen Verbindung auf Schaltungsebene mit diesen TMS-Methoden sowohl in der Gesundheit als auch bei Krankheiten die Entwicklung gezielter Diagnosen und innovativer Therapien auf der Grundlage ausgefeilterer Netzwerkmodelle von Hirnverhaltensbeziehungen ermöglicht.
Die hier beschriebene dual-site TMS-Methode kann verwendet werden, um funktionelle Wechselwirkungen zwischen verschiedenen kortikalen Bereichen zu untersuchen, die mit dem primären Motorkortex verbunden sind, während ein Teilnehmer im Ruhezustand ist oder eine zielorientierte Aktion plant. Während die Bildgebung des Gehirns korrelativ ist, können Grundkenntnisse aus TMS-Methoden mit zwei Standorten kausale Gehirnverhaltensbeziehungen aufdecken, die mit Veränderungen in kortikalen Schaltkreisen verbunden sind.
