Nós descrevemos um "padrão ouro" para a avaliação da tolerância ortostática (OT), utilizando o teste de inclinação com a pressão corpo combinado inferior negativo (LBNP). Isto pode ser combinado com os não-invasivos avaliações de controle reflexo cardiovascular. Respostas normais e anormais estão definidas.
Tolerância ortostática (OT) refere-se à capacidade de manter a estabilidade cardiovascular quando na posição vertical, contra os efeitos hidrostáticos de gravidade, e, consequentemente, para manter a perfusão cerebral e prevenir a síncope (desmaio). Estão disponíveis várias técnicas para avaliar OT e os efeitos do stress sobre a circulação gravitacional, tipicamente através da reprodução de um evento de pré-síncope (desmaio quase episódio) num ambiente laboratorial controlado. A hora e / ou o grau de tensão necessária para provocar esta resposta, fornece a medida da OT. Qualquer técnica usada para determinar OT deve: permitir distinção entre pacientes com intolerância ortostática (de várias causas) e controles assintomáticos; ser altamente reprodutível, permitindo a avaliação de intervenções terapêuticas; evitar procedimentos invasivos, que são conhecidos por prejudicar OT 1.
No final de 1980 de cabeça ereta teste de inclinação foi utilizada pela primeira vez para o diagnóstico de síncope 2. Since então, tem sido utilizado para avaliar a OT em pacientes com síncope de causa desconhecida, assim como em indivíduos saudáveis para estudar posturais reflexos cardiovasculares 2-6. Protocolos de inclinação compreendem três categorias: inclinação passiva; inclinação passiva acompanhado por provocação farmacológica, e de inclinação passiva com a pressão corpo combinada inferior negativo (LBNP). No entanto, os efeitos do teste de inclinação (e outras modalidades de testes de estresse ortostáticos) são muitas vezes pouco reprodutível, com baixa sensibilidade e especificidade para o diagnóstico de intolerância ortostática 7.
Tipicamente, a inclinação passiva inclui 20-60 min de estresse ortostático continuou até o início da pré-síncope em pacientes 2-6. No entanto, a principal desvantagem deste processo é a sua incapacidade para invocar pré-síncope em todos os indivíduos submetidos a ensaio, e sensibilidade baixa correspondente 8,9. Assim, vários métodos foram exploradas para aumentar a tensão ortostática e melhorar a sensibilidade.
Provocação farmacológica tem sido utilizada para aumentar o desafio ortostática, utilizando por exemplo a isoprenalina 4,7,10,11 ou nitrato sublingual 12,13. No entanto, a principal desvantagem destas abordagens são aumentos na sensibilidade à custa de diminuições inaceitáveis na especificidade 10,14, com uma elevada taxa de resposta positiva imediatamente após a administração de 15. Além disso, procedimentos invasivos associados com algumas provocações farmacológicos aumentar significativamente a taxa de falsos positivos 1.
Outra abordagem é combinar teste de inclinação passiva com LBNP, proporcionando um forte estresse ortostático sem procedimentos invasivos ou drogas efeitos colaterais, utilizando a técnica pioneira pelo professor Roger Hainsworth na década de 1990 16-18. Esta abordagem provoca pré-síncope em quase todos os assuntos (permitindo o reconhecimento dos sintomas em pacientes com síncope), ao discriminar entre patients com controles de síncope e saudável, com uma especificidade de 92%, sensibilidade de 85% e repetitividade de 1,1 ± 0,6 min 16,17. Isto permite o diagnóstico e avaliação não só fisiopatológico 19-22, mas também a avaliação de tratamentos para a intolerância ortostática, devido à sua elevada repetibilidade 23-30. Por estas razões, defendemos este deve ser o "padrão ouro" para testes de estresse postural, e, consequentemente, este será o método descrito neste trabalho.
Esta técnica é altamente reprodutível, tem a capacidade de discriminar respostas normais e anormais, com elevada sensibilidade e especificidade, e pode provocar a pré-síncope em todos os indivíduos, o que permite a identificação dos sintomas em pacientes com síncope recorrente.
