Nous décrivons un «gold standard» pour évaluer la tolérance orthostatique (OT) en utilisant le test d'inclinaison avec la pression du corps combiné inférieur négative (PNMI). Ce peut être combiné avec l'évaluation non invasive de contrôle réflexe cardiovasculaire. Réponses normales et anormales sont définies.
Tolérance orthostatique (OT) se réfère à la capacité de maintenir la stabilité cardio-vasculaire et en position verticale, contre les effets hydrostatiques de la gravité, et donc de maintenir la perfusion cérébrale et de prévenir la syncope (évanouissement). Diverses techniques sont disponibles pour évaluer les OT et les effets du stress gravitationnel sur la circulation, généralement en reproduisant un événement presyncopal (quasi-évanouissement épisode) dans un environnement de laboratoire contrôlé. L'heure et / ou le degré de contrainte nécessaire pour provoquer cette réaction fournit la mesure de OT. Toute technique utilisée pour déterminer OT doit: permettre la distinction entre les patients présentant une intolérance orthostatique (de causes diverses) et les sujets témoins asymptomatiques; être hautement reproductible, permettant l'évaluation des interventions thérapeutiques; éviter des procédures invasives, qui sont connues pour altérer OT 1.
Dans les années 1980 test d'inclinaison verticale de tête a d'abord été utilisée pour le diagnostic de la syncope 2. Since lors, il a été utilisé pour évaluer OT chez les patients ayant eu une syncope de cause inconnue, ainsi que chez des sujets sains pour étudier posturaux réflexes cardiovasculaires 2-6. Protocoles de basculement comprennent trois catégories: inclinaison passive; inclinaison passive accompagnée par une provocation pharmacologique, et inclinaison passive avec la pression du corps combinée inférieure négative (PNMI). Cependant, les effets des essais d'inclinaison (et d'autres orthostatiques modalités de stress tests) sont souvent peu reproductibles, avec une faible sensibilité et la spécificité pour diagnostiquer l'intolérance orthostatique 7.
En règle générale, une inclinaison passive comprend 20-60 min de stress orthostatique a continué jusqu'au début de la pré-syncope chez les patients 2-6. Toutefois, le principal inconvénient de cette procédure est d'invoquer son incapacité présyncope chez tous les individus qui subissent le test, et correspondant faible sensibilité 8,9. Ainsi, différentes méthodes ont été explorées pour augmenter le stress orthostatique et améliorer la sensibilité.
Provocation pharmacologique a été utilisé pour augmenter le défi orthostatique, en utilisant par exemple isoprénaline 4,7,10,11 ou le nitrate de 12,13 sublinguale. Toutefois, le principal inconvénient de ces approches sont des augmentations de la sensibilité au prix d'une diminution inacceptable de la spécificité 10,14, avec un taux de réponse positive élevée immédiatement après l'administration 15. En outre, des procédures invasives associées à certaines provocations pharmacologiques augmenter considérablement le taux de faux positifs 1.
Une autre approche consiste à combiner le test d'inclinaison passive avec LBNP, offrant un fort stress orthostatique, sans procédures invasives ou effets secondaires des médicaments, en utilisant la technique mise au point par le professeur Roger Hainsworth dans les années 1990 16-18. Cette approche provoque présyncope dans presque tous les sujets (permettant la reconnaissance des symptômes chez les patients souffrant de syncope), tandis que la discrimination entre patients avec syncope contrôles et en bonne santé, avec une spécificité de 92%, la sensibilité de 85% et une répétabilité de 1,1 ± 0,6 min 16,17. Cela permet non seulement le diagnostic et l'évaluation physiopathologique 19-22, mais aussi l'évaluation des traitements de l'intolérance orthostatique en raison de sa grande répétabilité 23-30. Pour ces raisons, nous soutenons ce devrait être le «gold standard» pour les tests de stress orthostatique et, par conséquent ce sera la méthode décrite dans le présent document.
Cette technique est hautement reproductible, a la capacité de discriminer les réponses normales et anormales avec une sensibilité et une spécificité élevées, et peuvent provoquer des présyncope dans toutes les matières, ce qui permet la reconnaissance des symptômes chez les patients présentant des syncopes récidivantes.
