Näthinnan har framträdande likheter med hjärnan och utgör därmed ett unikt fönster för att studera vaskulatur och neuronala struktur i hjärnan icke-invasivt. Det här protokollet beskriver en metod för att studera demens med retinal imaging tekniker. Denna metod kan potentiellt stöd i diagnos och riskbedömning av demens.
Näthinnan erbjuder en unik ”fönster” för att studera patofysiologiska processer av demens i hjärnan, eftersom det är en förlängning av det centrala nervsystemet (CNS) och delar framträdande likheter med hjärnan i form av embryologiska ursprung, anatomiska funktioner och fysiologiska egenskaper. Vaskulära och neuronala strukturen i näthinnan nu kan enkelt visualiseras och icke-invasivt med retinal imaging tekniker, inklusive fundus fotografi och optisk koherenstomografi (OCT) och kvantifieras halvautomatiskt med hjälp datorstödd analysprogram. Studera associationerna mellan vaskulär och neuronala förändringar i näthinnan och demens kan förbättra vår förståelse av demens och potentiellt stöd i diagnos-och riskbedömning. Detta protokoll syftar till att beskriva en metod för att kvantifiera och analysera retinala blodkärlen och neuronala struktur som är potentiellt förknippas med demens. Detta protokoll också ger exempel på retinala förändringar hos patienter med demens, och diskuterar tekniska problem och nuvarande begränsningar av retinal imaging.
På grund av ökningar av medellivslängden, demens har blivit ett stort medicinskt problem, bidrar till betydande sociala och ekonomiska hälsa belasta globalt1,2,3,4,5. Idag, utvecklar en person i Förenta staterna Alzheimers sjukdom (AD), den vanligaste formen av demens, var 66 s6. Det har uppskattats att 115 miljoner personer fram till år 2050, kommer att påverkas av AD7.
Näthinnan erbjuder en unik ”fönster” för att studera demens på grund av dess liknande anatomiska och fysiologiska egenskaper med hjärnan. När det gäller kärlsystemet, retinal arterioler och venoler, mäta 100 till 300 µm i diameter, dela liknande funktioner med cerebral små kärl, som slutet arterioler utan anastomoser, barriärfunktion och auto-förordning8, 9. När det gäller neuronala struktur, retinal ganglieblockerande celler (RGCs) dela typiska egenskaper med nervceller i det centrala nervsystemet (CNS) 10. RGCs kopplas tydligt med hjärnan som de bildar synnerven och projektet visuella signalerna från näthinnan till den laterala geniculate kärnan och den superior colliculus. Synnerven, liknar många neuronala fibrer i CNS, är myeliniserade av oligodendrocyter och ensheathed i meningeal lager. Noterbart är en förolämpning mot synnerven kan resultera i liknande respons som observerats i andra CNS axoner, såsom retrograd och anterograd degeneration av axon, ärrbildning, myelin destruktion, sekundära degeneration och en onormal nivå av neurotrofa faktorer och signalsubstanser11,12,13,14. Utseendet på visuella symptom hos vissa AD-patienter kan också förklaras av de robusta associationerna mellan näthinnan och hjärnan15,16. Det har därför föreslagits att näthinnan kan avspegla de patologiska processerna av demens i hjärnan och retinal imaging kan användas för att studera demens.
Den retinala blodkärlen och neuronala struktur kan nu visualiseras icke-invasivt med retinal imaging tekniker. Till exempel retinal ögonbotten fotografier kan fångas med hjälp av fundus kameror, och egenskaperna hos den retinala kärl (t.ex. fartyg kaliber, tortuosity och fraktal dimension) kan då kvantifieras med datorstödd analys program. Dessutom parametrar av retinal neuronala struktur (till exempel tjocklek ganglion cell-inner plexiform lagret [GC-IPL] och retinala nervfiber skikt [RNFL]) kan också mätas med hjälp av optisk koherenstomografi (OCT) och kvantifieras med hjälp av inbyggt algoritmer för analys.
Med tanke på betydelsen av retinal imaging att studera demens, syftar detta protokoll till att beskriva en metod för imaging och analysera retinala blodkärlen och neuronala struktur i vivo som använder retinal imaging tekniker. Detta protokoll också ger exempel på retinala förändringar hos patienter med demens, och diskuterar tekniska problem och nuvarande begränsningar av retinal imaging.
Det här protokollet beskriver förfarandena att kvantifiera neuronala och vaskulära förändringar i näthinnan i vivo. Såsom näthinnan delar liknande embryologiska ursprung, anatomiska funktioner och fysiologiska egenskaper med hjärnan, kan dessa retinala förändringar avspeglar liknande förändringar av kärlsystemet och neuronala struktur i hjärnan.
