Nethinden deler fremtrædende ligheder med hjernen og udgør således en enestående vindue at studere vaskulatur og neuronal struktur i hjernen ikke-invasivt. Denne protokol beskriver en metode til at studere demens ved hjælp af retinale Billeddannende teknikker. Denne metode kan potentielt støtte i diagnosticering og risiko vurdering af demens.
Nethinden tilbyder en unik “vindue” for at studere patofysiologiske processer af demens i hjernen, som det er en udvidelse af det centrale nervesystem (CNS) og deler fremtrædende ligheder med hjernen i form af embryologiske oprindelse, anatomiske funktioner og fysiologiske egenskaber. Den vaskulære og neuronal struktur i nethinden kan nu være visualiseret let og ikke-invasivt bruge retinal imaging teknikker, herunder fundus fotografering og optisk kohærens tomografi (OCT) og kvantificeret semi-automatisk ved hjælp af computerstøttet analyse programmer. At studere sammenhængen mellem vaskulære og neuronale ændringer i nethinden og demens kunne forbedre vores forståelse af demens og, potentielt, støtte i diagnosticering og risikovurdering. Denne protokol har til formål at beskrive en metode til kvantificering og analyse af retinale kar og neuronal struktur, som er potentielt forbundet med demens. Denne protokol også giver eksempler af retinale forandringer hos forsøgspersoner med demens, og diskuterer tekniske problemer og nuværende begrænsninger af retinale billeddannelse.
Som følge af stigninger i middellevetiden, demens er blevet en større medicinske problem, bidrager til betydelig social og økonomisk sundhed belaster globalt1,2,3,4,5. I dag, udvikler en person i USA Alzheimers sygdom (AD), den mest almindelige form for demens, hver 66 s6. Det anslås at inden år 2050, 115 millioner mennesker påvirkes af AD7.
Nethinden tilbyder en unik “vindue” for at studere demens på grund af dens lignende anatomiske og fysiologiske egenskaber med hjernen. Vaskulaturen, retinal arterioler og vener, måling 100-300 µm i diameter, har lignende træk med cerebral små fartøjer, som ende arterioler uden anastomoser, barriere funktion, og auto-forordning8, 9. med hensyn til neuronal struktur, retinal ganglionære celler (RGCs) del typiske egenskaber med neuroner i det centrale nervesystem (CNS) 10. RGCs er tydeligt forbundet med hjernen som de danner de optiske nerve og projekt visuelle signaler fra nethinden til de laterale geniculate kerner og den overlegne colliculus. Synsnerven, svarende til mange neuronal fibre i CNS, er myelinerede af oligodendrocytes og er ensheathed i meningeal lag. Især en fornærmelse mod den optiske nerve kan resultere i tilsvarende svar observeret i andre CNS axoner, såsom retrograd og anterograd degeneration af axon, scar dannelse, myelin ødelæggelse, sekundære degeneration og en unormal niveau af neurotrope faktorer og neurotransmittere11,12,13,14. Udseendet af visuelle symptomer i nogle AD patienter kan også forklares ved de robuste sammenslutninger mellem nethinden og hjernen15,16. Som et resultat, er det blevet foreslået at nethinden kan afspejle de patologiske processer af demens i hjernen og nethinde billedbehandling kan bruges til at studere demens.
Den retinale kar og neuronal struktur kan nu visualiseres ikke-invasivt ved hjælp af retinale Billeddannende teknikker. For eksempel, retinal fundus fotografier kan blive fanget ved hjælp af fundus kameraer, og Karakteristik af den retinale Vaskulaturen (fx fartøj kaliber, tortuosity og fraktal dimension) kan derefter kvantificeres ved hjælp af computer-assisteret analyse programmer. Derudover parametre af den retinale neuronal struktur (som tykkelsen af ganglion celle-indre plexiform lag [GC-IPL] og retinal nerve fiber lag [RNFL]) kan også måles ved hjælp af optisk kohærens tomografi (OCT) og kvantificeres ved hjælp af indbygget analyse algoritmer.
I betragtning af betydningen af retinale imaging til at studere demens, denne protokol har til formål at beskrive en metode til imaging og analysere retinale kar og neuronal struktur i vivo ved hjælp af retinale Billeddannende teknikker. Denne protokol også giver eksempler af retinale forandringer hos forsøgspersoner med demens, og diskuterer tekniske problemer og nuværende begrænsninger af retinale billeddannelse.
Denne protokol beskriver procedurer af kvantificere neuronal og vaskulære forandringer i nethinden i vivo. Som nethinden deler samme embryologiske oprindelse, anatomiske funktioner og fysiologiske egenskaber med hjernen, kan disse retinale forandringer afspejler lignende ændringer af vaskulatur og neuronal struktur i hjernen.
