Her præsenterer vi en protokol om brugen af intraoperativ ultralyd i rygmarvskirurgi, især i tilfælde af intradurale læsioner og læsioner i den ventrale rygmarvskanal, når der anvendes en bageste tilgang.
Siden 1980’erne har der været flere rapporter om brugen af intraoperativ ultralyd som et nyttigt supplement til rygmarvskirurgi. Men med fremkomsten af nyere banebrydende billeddannelsesmetoder er brugen af intraoperativ ultralyd i rygsøjlekirurgi stort set faldet i ugunst. På trods af dette fortsætter intraoperativ ultralyd med at give flere fordele i forhold til andre intraoperative teknikker såsom magnetisk resonansbilleddannelse og computertomografi, herunder at være mere omkostningseffektiv, effektiv og nem at betjene og fortolke. Derudover er det fortsat den eneste metode til realtidsvisualisering af blødt væv og patologier. Dette papir fokuserer på fordelene ved at anvende intraoperativ ultralyd, især i tilfælde af intradurale læsioner og læsioner ventral til den tekale sæk, når de nærmer sig bagud.
Ultralyd er et af de mest almindelige diagnostiske værktøjer inden for medicin, især til visualisering af patologi i maven, lemmerne og nakken. Imidlertid er dets anvendelse til at undersøge kraniale og spinale læsioner i øjeblikket ikke meget udbredt. I 1978 var Reid den første til at rapportere brugen af ultralyd til at visualisere cervikal ledning cystisk astrocytom1. Her blev scanninger udført med patientens hals bøjet for at muliggøre åbning af det intralaminærvindue. Fire år senere, i 1982, rapporterede Dohrmann og Rubin brugen af ultralyd intraoperativt til at visualisere det intradurale rum hos 10 patienter2. Patologier identificeret med intraoperativ ultralyd blandt de 10 patienter omfattede syringomyelia, rygmarvscyster og intramedullære og ekstramedullære tumorer. De demonstrerede yderligere brugen af intraoperativ ultralyd til at guide katetre og sonder til biopsi af tumorer, dræning af cyster og ventrikulær shuntkateterplacering3. Dette muliggjorde overvågning i realtid og præcis positionering af sonder / katetre, hvilket reducerede unøjagtighed og fejl i placeringen. Efter disse indledende rapporter har flere andre offentliggjort brugen af intraoperativ ultralyd til styring af rygmarvscystedræning, intramedullær og ekstramedullær tumorresektion og syringo-subarachnoid shuntkateterplacering 4,5,6,7,8,9,10 . Derudover har det vist sig også at øge hastigheden af fuldstændig resektion af intraaksiale solide hjernetumorer og spinal intradurale tumorer11,12. Intraoperativ ultralyd har også vist sig at være nyttig til intraoperativ kirurgisk planlægning før manipulation af vævet og efterfølgende visualisering af tilstrækkelig neurale elementdekompression hos patienter med rygsøjlefrakturer 7,9,13,14,15.
Med fremkomsten af nyere intraoperativ teknologi, der muliggør klarere visualisering af blødt væv, såsom magnetisk resonansbilleddannelse (MRI) og computertomografi (CT), er intraoperativ ultralyd blevet mindre almindelig og en mindre begunstiget intraoperativ billeddannelsesmodalitet blandt neurokirurger i dag16. Imidlertid kan intraoperativ ultralyd have fordele i forhold til disse nyere teknologier i visse operative tilfælde (tabel 1). Intraoperativ ultralyd har vist sig at demonstrere bedre blødt vævsvisualisering af intradurale strukturer sammenlignet med intraoperativ CT (iKT) eller keglestråle CT (cbCT) 9,17. Mens intraoperativ MR (iMRI) er nyttig, hvor den er tilgængelig på grund af den højere bløddelsopløsning, den giver, er den dyr, tidskrævende og giver ikke realtidsbilleder6, 16,18. Et eksempel er i tilfælde af en intradural masse ventral til den tekale sæk, som kirurgen ikke er i stand til direkte at visualisere. Derudover, på trods af at det er operatørafhængigt, er intraoperativ ultralyd fra vores erfaring ret simpelt at bruge og kan let læses uden en radiolog.
Intraoperativ ultralyd i rygmarvskirurgien er stort set faldet i ugunst med fremkomsten af nyere teknologi, men det fortsætter med at give flere fordele i forhold til de andre tilgængelige billeddannelsesmetoder såsom MR og CT 6,9,16,17,18.
