Her presenterer vi en protokoll for å studere i Patofysiologien ved proliferativ diabetisk retinopati med pasient-avledede, inngrep forbrukeravgift, fibrovascular vev for tredimensjonale innfødt vev karakterisering og ex vivo kultur. Dette ex vivo kultur modell er også mottakelig for testing eller utvikle nye behandlinger.
En av årsakene til blindhet i yrkesaktiv alder voksne diabetisk retinopati (DR) er den vanligste mikrovaskulær komplikasjonen av diabetes. Ingen gjeldende dyr modeller av diabetes og oksygen-indusert retinopati utvikle fulltone progressive endringer manifestert i menneskelig proliferativ diabetisk retinopati (PDR). Derfor forståelse av sykdommen patogenesen og patofysiologi har støttet seg hovedsakelig på bruk av histologiske seksjoner og glasslegemet prøver tilnærminger som bare gir stabil informasjon på involvert sykdomsfremkallende faktorer. Økende bevis indikerer at dynamisk celle-celle og celle-ekstracellulær matrix (EFM) interaksjoner i konteksten av tredimensjonale (3D) microenvironments er avgjørende for den mekanistiske og funksjonelle studier mot utviklingen av nye behandling strategier. Derfor hypotesen vi at patologisk fibrovascular vevet inngrep forbrukeravgift fra øynene med PDR kan utnyttes til å pålitelig rakne cellulære og molekylære mekanismer for denne ødeleggende sykdommen og teste potensialet for romanen klinisk intervensjoner. Mot dette formål utviklet vi en ny metode for 3D ex vivo kultur av inngrep forbrukeravgift pasient-avledede fibrovascular vev (FT), som vil tjene som en relevant modell av menneskelig PDR patofysiologi. FTs deles opp i explants og innebygd i fibrin matrise for ex vivo kultur og 3D karakterisering. Hele-mount immunofluorescence opprinnelige FTs og sluttpunktet kulturer kan av vevssammensetning og flercellet prosesser, fremheve betydningen av 3D vev-nivå karakterisering av avdekke relevante funksjoner i PDR patofysiologi. Denne modellen tillater samtidig vurdering av molekylære mekanismer, mobilnettet/vev prosesser og behandlingstiltak i kompleks sammenheng med dynamisk biokjemiske og fysiske interaksjoner i PDR vev arkitekturen og microenvironment. Siden denne modellen viser PDR patofysiologi, vil det også være mottakelig for testing eller utvikle nye behandlinger.
DR er en alvorlig okulær komplikasjoner av diabetes, en sykdom som har nådd enorme proporsjoner i de siste tre tiårene1. Tyve år etter diagnose, nesten hver pasient med type 1 diabetes og 60% av pasienter med type 2 diabetes stede tegn på retinopati, gjør diabetes sådan en av årsakene til blindhet i arbeider age voksne2. Ifølge nivået på mikrovaskulær degenerasjon og iskemiske skader, er DR klassifisert i ikke-proliferativ DR (ikke-PDR) og proliferativ DR (PDR). Sluttstadiet sykdommen, PDR, er preget av ischemia – og betennelse-indusert neovascularization og fibrotiske svar på vitreoretinal grensesnittet. I ubehandlet forhold, vil disse prosessene føre til blindhet glasslegemet blødning, netthinnen fibrose, tractional Netthinneavløsning og neovascular glaukom3,4. Til tross for nylige fremskritt mål gjeldende behandlingstilbud bare DR stadier, inkludert diabetiker macula ødem og PDR, retinal skade har allerede fulgte. Videre nytte en stor andel av DR pasienter ikke fra gjeldende behandling armamentarium, som angir et presserende behov for bedre behandling4,5,6.
Flere andre jegn vivo sykdom/utviklingsmessige modeller og diabetiker dyremodeller har vært utviklet ennå, men ingen av dem viser hele omfanget av patologisk funksjoner i menneskelig PDR7,8. Videre indikerer økende bevis at behandlingstiltak er tett koblet til ECM sammensetningen samt romlige arrangement og samspill mellom mobilnettet og acellular microenvironment9. Vi er derfor å utvikle en klinisk relevante modell av menneskelig PDR ved å benytte FT patologisk materialet som er vanlig forbrukeravgift fra øynene under vitrectomy som en del av kirurgisk behandling av PDR10.
Dette manuskriptet beskriver protokollen for 3D ex vivo kultur og karakterisering av de kirurgisk-forbrukeravgift, PDR pasient-avledede patologisk FT. Metoden beskrevet her har vært brukt i en fersk publikasjon som vellykket dekonstruksjon av innfødte 3D PDR vev landskapet og recapitulation av funksjonene i PDR patofysiologi inkludert angiogenic og fibrotiske svar av unormale vaskulære strukturer11. Denne modellen har også avdekket romanen funksjoner som ikke kan lett nytes fra tynne histologiske seksjoner, som romlig begrenset apoptose og spredning samt vaskulær Holme formasjon11. Glasslegemet væske har blitt brukt av andre 3D endothelial spheroid kulturer å evaluere sin angiogenic potensielle og effekten av angiostatic molekyler12. Kombinert med en i vitro 3D lymfatisk endothelial celle (LEC) spheroid spirende analysen bruker PDR glasslegemet som stimulerende, viste vår modell bidrag av både løselig vitreal faktorer så vel som lokale stikkordene i det neovascular vev som men dårlig forstått LEC engasjement i PDR patofysiologi3,11. I forvaltningen av PDR er vitreoretinal kirurgi en rutinemessig utført ennå utfordrende prosedyre. Som kirurgisk instrumentations og teknikker ser kontinuerlig utvikling og raffinement, tidsriktig og konservative fjerning av fibrovascular proliferativ ikke bare forbedrer visjon resultatet, men gir også uvurderlig vev materiale for den undersøkelse av PDR patofysiologi og behandling svar i komplekse translasjonsforskning aspekter av det levende menneskelige vev microenvironment.
Vurderer betydningen av relevante vev microenvironment for pålitelig funksjonelle cellen og molekylære mekanistisk resultater er det viktig å finne passende eksperimentelle modeller som gir dette vev-miljøet. Her beskrives ex vivo PDR kultur modell for fibrin-embedded FTs lar etterforskningen av mekanismer for PDR patofysiologi i innfødte, komplekse og flercellet sammenheng PDR klinisk prøvene.
