Dit protocol laat zien hoe de functionele connectiviteit van de rusttoestand in de menselijke prefrontale cortex kan worden gemeten met behulp van een op maat gemaakt diffuus correlatiespectroscopie-instrument. Het rapport bespreekt ook praktische aspecten van het experiment en gedetailleerde stappen voor het analyseren van de gegevens.
Om een uitgebreid begrip van het menselijk brein te verkrijgen, is het gebruik van cerebrale bloedstroom (CBF) als een bron van contrast gewenst omdat het een belangrijke hemodynamische parameter is die verband houdt met cerebrale zuurstoftoevoer. Rusttoestand lage frequentie schommelingen op basis van oxygenatie contrast is aangetoond dat correlaties tussen functioneel verbonden regio’s te bieden. Het gepresenteerde protocol maakt gebruik van optische diffuse correlatie spectroscopie (DCS) om de bloedstroom-gebaseerde rusttoestand functionele connectiviteit (RSFC) in het menselijk brein te beoordelen. Resultaten van CBF-gebaseerde RSFC in de menselijke frontale cortex geven aan dat intra-regionale RSFC aanzienlijk hoger is in de linker- en rechtercortices in vergelijking met interregionale RSFC in beide cortices. Dit protocol moet van belang zijn voor onderzoekers die gebruik maken van multi-modale beeldvormingstechnieken om de menselijke hersenfunctie te bestuderen, vooral in de pediatrische bevolking.
Wanneer de hersenen zich in een rusttoestand bevinden, toont het een hoge synchronisatie van spontane activiteit in functioneel verwante gebieden aan, die zich dichtbij of van een afstand kunnen bevinden. Deze in-sync regio’s staan bekend als functionele netwerken1,2,3,4,5,6,7,8,9. Dit fenomeen werd voor het eerst ontdekt door een functionele magnetische resonantie imaging (fMRI) studie met behulp van bloed zuurstof niveau-afhankelijke (BOLD) signalen die oxygenatie niveaus van het hersenbloed5,,10, ook bekend als ruststaat functionele connectiviteit (RSFC). Afwijkingen in RSFC zijn geassocieerd met hersenaandoeningen zoals autisme11, Alzheimer12, en depressie13. Dus, RSFC is een waardevol instrument voor het bestuderen van patiënten met aandoeningen die moeite hebben met het uitvoeren van taakgebaseerde beoordelingen. Echter, veel patiënten, zoals jonge autistische kinderen, zijn slechte kandidaten voor beoordeling door fMRI, omdat het vereist nog steeds binnen in een besloten ruimte voor langere tijd14,15. Optische beeldvorming is snel en draagbaar; zo is het geschikt voor een meerderheid van de patiënten, met name de pediatrische bevolking16,17,18,19,20,21,22,23,24. Met behulp van deze voordelen, functionele nabij-infrarood spectroscopie (fNIRS), die hemoglobine concentratie en zuurstofverzadiging parameters in de hersenen kan kwantificeren, wordt gebruikt om RSFC te meten bij de mens (met inbegrip van de pediatrische bevolking4,8,25 en patiënten met autisme11).
Optische diffuse correlatiespectroscopie (DCS), een relatief nieuwe optische techniek, kan de cerebrale bloedstroom kwantificeren, wat een belangrijke parameter is die zuurstoftoevoer associeert met metabolisme6,17,26,27,28,29. Het optische stroomcontrast dat door DCS wordt gekwantificeerd, blijkt een hogere gevoeligheid in de hersenen te hebben in vergelijking met oxygenatiecontrast30. Het gebruik van DCS-afgeleide CBF-parameters voor de beoordeling van RSFC is dus voordelig.
DCS is gevoelig voor bewegende bloedcellen. Wanneer het verspreiden van fotonen verstrooiing van bewegende bloedcellen, dit zorgt ervoor dat de intensiteit van gedetecteerd licht te fluctueren in de tijd. DCS meet een op tijd gebaseerde intensiteitautocorrelatiefunctie en de vervalsnelheid is afhankelijk van de optische parameters en de bloedstroom. Deze waarden worden uiteindelijk gebruikt om de cerebrale bloedstroom index (CBFi) te verkrijgen. Met sneller bewegende bloedcellen, de intensiteit autocorrelatie functie vervalt sneller. Daarom kan informatie over beweging diep onder het weefseloppervlak worden afgeleid (bijvoorbeeld in de hersenen) uit metingen van het verspreiden van lichtfluctuaties in de tijd27,31,32,33,34,35. DCS is een techniek die complementair is aan de alom bekende fNIRS die bloedoxygenatie meet17,36. Aangezien zowel fNIRS als DCS optische hersenbeeldvormingstechnieken zijn met een hoge temporele resolutie in het bereik van milliseconden, zijn de optische beeldvormingsopstellingen veel minder gevoelig voor bewegingsartefacten dan fMRI. Ze zijn ook met succes gebruikt voor functionele beeldvorming van de hersenen in pediatrische populaties, waaronder zeer jonge zuigelingen16. Voorheen werden oppervlakkige bloedstroommetingen gebruikt om RSFC te beoordelen in preklinische studies bij muizen37. Hier worden bloedstroomparameters gebruikt om RSFC bij negen gezonde volwassenen te kwantificeren als een proof-of-concept studie38,39.
In deze studie wordt gebruik gemaakt van een commercieel FD-fNIRS systeem en een aangepast DCS systeem(zie Tabel van Materialen). De DCS die in eigen huis werd gebouwd bestaat uit twee 785 nm, 100 mW, lange coherentielengte continugolflasers die zijn gekoppeld aan een FC-connector en acht single-photon telmachines (SPCM) aangesloten op een auto-correlator. Een aangepaste software grafische gebruikersinterface (GUI) is ook speciaal gemaakt voor dit systeem om de fotontellingen, autocorrelatiecurven en semi-kwantitatieve bloedstroom van elk SPCM-kanaal in real-time weer te geven en op te slaan. De onderdelen in dit systeem worden vaak gebruikt voor DCS16,17,31,32,40,42,43,44, en de verkregen resultaten zijn ook in eigen beheer geverifieerd en gebruikt in een recente studie39.
Om te bepalen of CBF zoals gemeten door DCS nauwkeurig gedetecteerd RSFC, twee gebieden van de hersenen met bekende RSFC eigenschappen werden onderzocht. Functionele connectiviteit tussen DLFC-regio’s en tussen DLFC en IFC wordt verondersteld te bestaan57,58,59.
