Abstract

Introduction
生物相容性可以定义为材料整合组织并诱导有利的治疗反应的能力,不受局部和全身损伤1,2,3。生物相容性评估对于开发任何用于医疗用途的材料至关重要。因此,该协议为每个旨在开发新生物材料或研究现有生物材料新应用的研究人员提供了一种系统和全面的方法。
体外细胞毒性测试被广泛用作生物相容性评估的第一阶段,使用原代细胞培养物或细胞系。这些结果构成了潜在临床应用的第一个指标。除了对生物材料开发至关重要外,该测试还具有强制性要求,以符合EUA和欧盟监管机构(FDA和CE认证)的现行市场引入法规4,5,6,7,8。此外,生物医学研究中的标准化测试在可重复性和比较类似生物材料或设备的不同研究结果方面提供了显着优势9。
国际标准化组织 (ISO) 指南被多个独立的商业、监管和学术实验室广泛使用,以准确和可重复的方式测试材料。ISO 10993-5 是指体外细胞毒性评估,ISO 10993-12 报告用于取样制备10,11。对于生物材料检测,提供了三类,根据材料类型,接触组织和治疗目标进行选择:提取物,直接接触和间接接触8,11,12,13。通过用生物材料富集细胞培养基来获得提取物。对于直接接触测试,将生物材料直接放置在细胞培养物上,并且在间接接触中,通过屏障(例如琼脂糖凝胶11)与细胞进行孵育。适当的对照是强制性的,至少应进行三次独立实验5,8,10,11,14。
模拟或夸大临床状况以确定细胞毒性潜力至关重要。在提取物测试的情况下,材料的表面积; 中等体积;培养基和材料pH值;材料溶解度、渗透压和扩散比;搅拌、温度、时间等提取条件对培养基富集的影响5.
该方法允许对几种固体和液体药物制剂的细胞毒性进行定量和定性评估。可以进行几种测定,例如中性红色摄取试验,菌落形成试验,MTT测定和XTT测定5,10,14。
大多数已发表的细胞毒性评估研究使用更简单的测定方法,即MTT和XTT,它们提供的信息有限。评估生物相容性不仅应涉及细胞毒性的评估,还应涉及给定测试材料的生物活性2,正如本协议所认可的那样。在有正当理由和记录的情况下,应使用其他评估标准。因此,该协议旨在提供全面的指南,详细说明了生物材料细胞毒性评估的一组方法。此外,还描述了不同细胞过程的评估,即细胞死亡的类型、细胞形态、特定蛋白质合成中的细胞功能和特定的组织产生。

Protocol
1. 颗粒制备
- 通过在PVC板上执行已知尺寸的圆形孔来制备聚氯乙烯(PVC)模具。
注意:PVC成型件可以制成不同的尺寸。计算PVC模具的接触面,使用公式A = h(2πr)+ 2πr2 (r:圆柱体的半径;h:圆柱体的高度)。 - 根据制造商的说明准备要测试的生物材料,并尽可能接近实验开始。
注意:对于糊状/糊状制剂生物材料的制备,用混合刮刀手动混合足量的基础浆料和催化剂。对于基于液体和粉末配方的其他材料,应按照制造商的说明或适合新材料的说明进行手动刮刀或振动机械混合。对于液体材料,此步骤不是必需的。在步骤 2 中启动协议。 - 用刮刀将生物材料放在模具上,让它们凝固适当的时间。
注意:生物材料的凝固时间和凝固条件必须遵循制造商的说明或适合新材料的说明。 - 凝固后,从PVC模具中取出生物材料的颗粒并将其放入容器中(可以使用6孔板或培养皿)。
- 通过将颗粒放在紫外线 (UV) 灯下每侧 20 分钟来对颗粒进行消毒。
2. 获取生物材料的提取物
注意:所有程序应在严格的无菌条件下进行。
- 通过根据1.1中描述的公式计算颗粒表面积来确定所需的颗粒数量。
注意:作为参考值,通过每毫升培养基添加 9 个颗粒 (r 3 mm x h 1.5 mm) 来实现 250 mm2/mL11,15 的接触表面积。 - 准备可溶性提取物(富含生物材料的提取物)。
- 将沉淀放入 50 mL 管中,并加入相应的细胞培养基。将试管以37°恒定旋转的方式放入培养箱中24小时。
注意:使用适合细胞培养物的细胞培养基。 - 24小时后,从培养箱中取出试管。此时,提取物的浓度对应于1/1或100%。
- 通过依次向细胞培养基中加入等体积的条件培养基来稀释提取物。
注意:不应对培养基进行pH调节。- 将 1 mL 培养基加入 1 mL 100% 提取物中,以获得 50% 提取物。将 1 mL 培养基加入 1 mL 50% 提取物中,以获得 25% 提取物,依此类推(图 1)。
注意:使用与每种化合物相关的浓度。

3. 使用生物材料提取物进行细胞孵育
- 根据实验所需的细胞数量,准备细胞悬液并将其接种在足够的细胞容器(例如多孔板)中。
- 从具有80%至90%汇合度的所需细胞的烧瓶开始。
- 弃去细胞培养基,用磷酸盐缓冲盐溶液(PBS)洗涤,并用胰蛋白酶-EDTA(75 cm2 细胞培养瓶为1至2mL)分离细胞。
- 加入细胞培养基(75 cm 2 细胞培养瓶为2 至 4 mL),将细胞悬液转移到管中并以 200 x g 离心 5 分钟。
- 将沉淀悬浮在已知体积的细胞培养基中。
注意:本协议设计用于贴壁细胞培养物;然而,可以进行简单的调整以用于悬浮细胞培养物。 - 计数血细胞计数器中的细胞并计算细胞悬液的细胞浓度。
- 将测定量的细胞悬液悬浮在培养基中并转移到多孔培养皿中。作为接种密度的参考值,请考虑 5 – 20 x 105 个细胞/cm2。
注意:必须根据细胞类型和细胞特性(即细胞倍增时间)计算分配的细胞数。
- 孵育细胞24小时以使细胞粘附。
- 在此期间之后,将可溶性提取物施用到培养板中。
- 吸出细胞培养基。

- 孵育板24小时或更长时间。

4.代谢活性的评估
- 用生物材料的提取物孵育细胞后,从平板中吸出培养基并洗涤每个孔PBS。
- 在每个孔中,放置足够体积的0.5mg/mL 3-(4,5-二甲基噻唑-2-基)-2,5-二苯基四氮唑溴化物(MTT),在PBS中制备,pH 7.4。
- 将板在37°C的黑暗中孵育4小时或过夜。
- 为了溶解获得的甲臜晶体,向每个孔中加入足够体积的0.04M盐酸异丙醇溶液,并搅拌板30分钟。
注意:根据孔的大小调整MTT和异丙醇的量。 - 如有必要,通过上下移液直到看不到晶体,搅拌和均质每个孔的内容物。
- 在分光光度计中,用620nm参考滤光片量化570nm波长处的吸光度。
- 为了计算代谢活性,将处理细胞的吸光度除以对照培养物的吸光度。要获得百分比值,请乘以 100。
5. 细胞死亡评估
注意:要执行此评估,每个条件至少应使用106 个细胞。
- 根据所评估的条件,使用正确识别的离心管。
- 与生物材料的提取物一起孵育细胞后,将培养基收集到相应的管中。
- 分离细胞并将细胞悬液添加到相应的管中。
- 通过以120× g 离心5分钟来浓缩细胞悬液。
- 用PBS清洗颗粒。通过以1,000× g 离心5分钟来除去PBS。
- 加入 1 mL PBS 并将细胞沉淀转移到鉴定的细胞术管中。
- 通过以1,000× g 离心5分钟来除去PBS。
- 用 100 μL 结合缓冲液(0.01 M HEPES、0.14 mM NaCl 和 0.25 mM CaCl2)孵育16,让细胞静置约 15 分钟以进行细胞膜回收。
- 在室温下在黑暗中加入 2.5 μL 荧光标记的膜联蛋白-V 和 1 μL 碘化丙啶 15 分钟。
- 孵育后,加入 400 μL PBS 并在细胞仪上分析。对于信息的分析和量化,请使用适当的软件。
- 以活细胞、细胞凋亡、晚期细胞凋亡/坏死和坏死的百分比表示结果。
6. 形态学评估
- 选择适合多孔板内的适当尺寸的灭菌玻璃盖玻片。
- 使用无菌镊子将每张载玻片放入孔中。
- 将足够浓度的细胞悬浮液分配到孔中,并在37°C的培养箱中在95%空气和5%CO 2的潮湿气氛中过夜。
- 如前所述,将细胞培养物暴露于提取物中。
- 吸出培养基并用PBS洗涤。
- 让盖玻片在室温下干燥,然后加入足够体积的May-Grünwald溶液以覆盖盖玻片;孵育 3 分钟。
- 除去染料,用蒸馏水洗涤1分钟。
- 取出水并加入足够体积的吉姆萨溶液以覆盖盖玻片;孵育 15 分钟。
- 用流水清洗盖玻片。
- 将盖玻片转移到载玻片上。

7. 通过逆转录聚合酶链反应(RT-PCR)评估细胞功能
注意:要执行此评估,每个条件至少应使用 2x106 个单元格。例如,碱性磷酸酶作为牙母细胞活性评估的感兴趣基因提出。其他感兴趣的基因见 表1。
- 如上所述对细胞进行铺板。
注意:可能需要根据所研究生物材料的细胞类型和细胞毒性调整接种的细胞浓度。 - 如上所述,用可溶性提取物孵育。
- 如前所述分离细胞以获得悬浮液。
- 用PBS洗涤细胞两次;对于该离心机,在室温下以200×g离心5分钟。
- 通过将沉淀悬浮在 1 mL RNA 纯化溶液(例如 NZYol)中、剧烈搅拌和连续移液来裂解细胞。
- 将样品在室温下孵育5分钟。
- 加入 200 μL 氯仿并用手摇动试管 15 秒。
- 在室温下孵育3分钟。
- 在4°C下以12,000× g离心裂解物15分钟。在离心过程中,样品中产生两相,将RNA留在水性(上层)相中。
- 将水相移入新试管中,加入 500 μL 冷异丙醇以沉淀 RNA。
- 将样品在室温下孵育10分钟,并在4°C下以12,000× g 离心10分钟。
- 除去上清液,用1mL的75%乙醇洗涤沉淀,在4°C下以7,500× g 离心5分钟。
- 在室温下干燥沉淀直至乙醇蒸发。
- 悬浮在不含RNase的水中。
- 使用吸收分光光度法在260nm和280nm波长下定量和确定样品的纯度。确定RNA纯度并使用纯度比(A260/280)在2.0左右的样品。
- 将样品储存在-80°C。
- 按照制造商的方案17 继续执行 RT-PCR。

8. 通过蛋白质鉴定评估细胞功能
注意:根据研究目标,选择要评估的特定蛋白质。例如,牙本质唾液酸蛋白(DSP)作为牙母细胞活性评估中感兴趣的蛋白质呈现。其他感兴趣的蛋白质见 表1。
- 如前所述,在盖玻片中培养细胞并暴露于提取物中。
- 用PBS洗涤细胞培养物。
- 在室温下用3.7%多聚甲醛固定30分钟。
- 用PBS清洗两次。
- 在PBS中用0.5%的Triton透化15分钟。
- 在PBS中用0.3%过氧化氢封闭过氧化物酶5分钟。
- 用PBS清洗两次。
- 用0.5%牛血清白蛋白(BSA)洗涤两次。
- 用2%BSA封闭细胞培养物45分钟。
- 用0.5%BSA在PBS中洗涤。
- 根据所选蛋白质与一抗在室温下孵育培养物60分钟。
注意:本协议使用一抗DSP(M20)抗体(1:100)和二抗多克隆兔抗山羊免疫球蛋白/ HRP(1:100)。 - 在PBS中用0.5%BSA洗涤五次。
- 在室温下与二抗孵育90分钟。
注意:使用PBS中的0.5%BSA进行抗体稀释。 - 用 0.5% BSA 在 PBS 中洗涤 5 次,每次洗涤 1 分钟。
- 将培养物与底物和显色剂混合物以 20 μL 显色原/mL 底物的浓度孵育 25 分钟。
- 用0.5%BSA在PBS中洗涤两次。
- 用苏木精复染15分钟。
- 用0.037mol/L的氨水和蒸馏水洗涤5分钟,以除去多余的染料。
- 将盖玻片安装在载玻片上。使用甘油作为封片剂。
- 晾干过夜。
- 在显微镜下看。以所选放大倍率拍摄照片。
9. 通过茜素红S测定进行矿化评估
- 制备浓度为40mM18的茜素红S溶液。在黑暗中搅拌溶液进行均质化12小时。
注意:要制备100 mL茜素红S溶液,请将1.44 g茜素粉(分子量:360 g / mol)溶解在超纯水中,避光。对于该溶液,pH值至关重要,应在4.1和4.3之间。 - 如上所述,用可溶性提取物孵育细胞培养物。
- 用PBS洗涤细胞培养物三次。
- 在室温下用4%多聚甲醛固定15分钟。
- 用PBS清洗三次。
- 在黑暗中用茜素红染色溶液在37°C下染色20分钟。
- 染色后,用PBS清洗板以去除多余的染料。
- 在显微镜下看。以所选放大倍率拍摄照片。
- 向每个孔中加入由10%(w / v)乙酸和20%(w / v)甲醇组成的提取溶液,并在室温下搅拌40分钟。
- 在分光光度计19上测量490nm波长处的吸光度。

Representative Results
这里的代表性结果是指牙科生物材料的研究。提取方法允许在暴露于牙科材料后获得细胞毒性谱和细胞功能,关于对代谢活性(图2),细胞活力,细胞死亡谱和细胞形态(图3)以及特异性蛋白质表达(图4)的影响。
MTT测定用于以直接的方式快速了解材料的细胞毒性。可以在两种或多种材料之间进行比较(图2);即使在低浓度(6.25%)和中等浓度(50%)下,代谢活性的严重降低也表明毒性较高(图2a)。同时,细胞毒性较小的物质仅存在较轻或无还原(图2b)。不同时间点之间的比较可以确定更直接的细胞毒性作用或在后期阶段。
对细胞活力的影响提供了有关活细胞减少的重要信息,这可能会损害组织在破坏性影响后的恢复能力。活细胞百分比的测定可以比较材料细胞毒性;在相同浓度下,更多的细胞毒性物质诱导更高的细胞死亡(图3a和3b)。减少超过30%至关重要,并定义了具有低生物相容性风险的材料(图3a)。该信息与细胞死亡图谱一起完成(图3a和3b)。在代表性结果中,细胞毒性较强的物质的特征在于细胞活力的急剧下降以及晚期细胞凋亡和坏死细胞死亡特征(图3a),而细胞毒性较小的物质呈现较少的细胞死亡和更多的凋亡和晚期凋亡特征(图3b)。
从细胞形态学评估(图3c)获得的信息补充了细胞活力评估。细胞典型形态的变化可表明细胞凋亡或坏死特征16。此外,可以从该协议中获得其他信息,例如观察材料颗粒(红色箭头, 图3C)。
受提取物暴露影响的特定标志物对细胞功能至关重要,可以通过多种技术进行评估,如免疫组织化学、PCR、流式细胞术、印迹或比色测定(表1)。暴露于提取物后DSP表达的代表性结果如图 4a所示,可以看出某些材料(硅酸三钙水泥)刺激细胞增加蛋白质表达。相反,其他(氢氧化钙水泥)促进蛋白质表达的显着降低,与活力损失无关。在这两种情况下,提取物的浓度都会直接影响蛋白质的表达。
在成牙细胞表型的MDPC-23细胞系中,矿化沉积物的形成是特征。矿化矿床鉴定和定量协议允许评估这种类型的特殊细胞的特定功能。在所呈现的案例中,观察到一旦观察到矿化沉积物的增加,除了细胞毒性较小外,硅酸三钙水泥会刺激细胞功能(图4b)。相反,细胞毒性更大的氢氧化钙水泥由于细胞损伤和死亡导致矿物质沉积减少(图4b)。除了定性评估外,还可以进行定量测定(图4c)。

提取培养基包括含或不带血清的细胞培养基、生理盐溶液、二甲基亚砜或纯化水,根据生物材料的化学特性11,33选择。针对细胞培养研究,首选使用细胞培养基,因为它避免了进一步的处理步骤。提取条件应根据实验模型进行调整。在本协议中显示的代表性结果中,补充有FBS的DMEM培养基在37±1°C下使用24±2小时。
一些生物材料可能会在提取培养基中留下残留物,这可能会对细胞培养产生负面影响。虽然应避免过滤和离心,但有可能在使用前让颗粒沉淀。另一个问题是提取后pH值可能会发生变化。由于不建议进行进一步调整11,因此必须测量,注册提取物的pH值,并且必须在必要时在实验设计中包括额外的对照以分离pH效应。

制备适当密度的细胞培养物至关重要,特别是在高重复率的细胞培养物上31。根据所用细胞的推荐接种密度范围,如果计划长时间孵育,则必须降低初始接种密度以避免与过度汇合相关的问题。此外,高细胞毒性材料可能需要更高的初始接种密度。
除了提取物方法的优点外,对于与细胞粘附性评估相关的材料,它并不是最佳选择。在这种情况下,必须进行直接接触研究38,39,40,41。虽然这是一种全面的方法,但重要的是要记住,它是一种体外评估,并不能完全反映体内条件42。
