Optical coherence tomography (OCT) is a high resolution imaging technique that allows analysis of tissue specific optical properties providing the means for tissue differentiation. We developed needle based OCT, providing real-time imaging combined with on-the-spot tumor differentiation. This publication describes a method for percutaneous, needle based OCT of renal masses.
Optical coherence tomography (OCT) is the optical equivalent of ultrasound imaging, based on the backscattering of near infrared light. OCT provides real time images with a 15 µm axial resolution at an effective tissue penetration of 2-3 mm. Within the OCT images the loss of signal intensity per millimeter of tissue penetration, the attenuation coefficient, is calculated. The attenuation coefficient is a tissue specific property, providing a quantitative parameter for tissue differentiation.
Until now, renal mass treatment decisions have been made primarily on the basis of MRI and CT imaging characteristics, age and comorbidity. However these parameters and diagnostic methods lack the finesse to truly detect the malignant potential of a renal mass. A successful core biopsy or fine needle aspiration provides objective tumor differentiation with both sensitivity and specificity in the range of 95-100%. However, a non-diagnostic rate of 10-20% overall, and even up to 30% in SRMs, is to be expected, delaying the diagnostic process due to the frequent necessity for additional biopsy procedures.
We aim to develop OCT into an optical biopsy, providing real-time imaging combined with on-the-spot tumor differentiation. This publication provides a detailed step-by-step approach for percutaneous, needle based, OCT of renal masses.
De afgelopen decennia hebben laten zien een gestage toename in de incidentie van niertumoren 1,2. Tot nu toe renale massabehandeling beslissingen gemaakt voornamelijk op basis van MRI en CT imaging kenmerken leeftijd en comorbiditeit. Echter deze diagnostische methoden en klinische parameters missen de finesse om de maligne potentie van een renale massa echt detecteren. Een biopsie of punctie voldoende weefsel voor pathologische evaluatie (diagnostische) bepaald doel tumordifferentiatie met zowel gevoeligheid als specificiteit in het traject van 95-100% 3. Daarom biopsie steeds meer aanvaard bij de evaluatie van verdachte niertumoren 4,5. Echter, biopten onvoldoende weefsel diagnose of gewone nierparenchym (niet-diagnostische) optreden met een snelheid van 10-20% in totaal, en zelfs tot 30% kleine niertumoren (<4 cm, SRM) vaststellen, vertragen het diagnostisch proces vanwege de frequente noodzaak voor aanvullendebiopsieprocedures 3,5.
Optische coherentie tomografie (OCT) is een nieuwe beeldvormende modaliteit die het potentieel heeft om de voornoemde obstakels in de renale massa differentiatie overwonnen heeft. Op basis van de terugverstrooiing van nabij infrarood licht, oktober geeft beelden met een 15 urn axiale resolutie van een effectieve penetratie van 2-3 mm (figuur 1, 2). Het verlies van signaalintensiteit per millimeter weefselpenetratie, een resultante van weefselspecifieke lichtverstrooiing, uitgedrukt als de demping coëfficiënt (μ oktober: -1 mm) zoals beschreven door Faber et al. 6. Histologische kenmerken kunnen worden gecorreleerd oktober waarden verschaffen een kwantitatieve parameter voor weefseldifferentiatie (figuur 3) μ.
Tijdens carcinogenese, kwaadaardige cellen vertonen een toenemend aantal grotere en meer onregelmatig gevormde kernen met een hogere brekingsindex en actievere mitochondria. Door deze overexpressie van celbestanddelen, een verandering in μ oktober te verwachten bij het vergelijken kwaadaardige tumoren benigne tumoren of aangetast weefsel 7.
Onlangs hebben we de mogelijkheid van oppervlakkige oktober te maken tussen goedaardige en kwaadaardige niertumoren 8,9. Bij 16 patiënten, intra-operatieve oktober metingen van tumorweefsel werd verkregen met een extern geplaatste oktober probe. De bedieningsarm omvat oktober metingen van aangetast weefsel in dezelfde patiënten. Normaal weefsel toonde een significant lagere mediane verzwakkingscoëfficiënt opzichte van kwaadaardig weefsel, bevestigt het potentieel van oktober voor tumor differentiatie. Deze kwantitatieve analyse werd op soortgelijke wijze rang andere soorten kwaadaardig weefsel toegepast zoals urotheelcarcinoom 10,11 en vulvaire epitheliale neoplasie differentiatie 12.
