De ziekte van Dupuytren (DD) is een fibroproliferatieve ziekte van de palm van de hand. Hier presenteren we een protocol om de cultuur resectiepreparaten van DD in een driedimensionale (3D) cultuur systeem. Een dergelijke tijdelijke kweeksysteem maakt behoud van de 3D structuur en moleculaire eigenschappen van het fibrotische weefsel.
Organ fibrosis or “scarring” is known to account for a high death toll due to the extensive amount of disorders and organs affected (from cirrhosis to cardiovascular diseases). There is no effective treatment and the in vitro tools available do not mimic the in vivo situation rendering the progress of the out of control wound healing process still enigmatic.
To date, 2D and 3D cultures of fibroblasts derived from DD patients are the main experimental models available. Primary cell cultures have many limitations; the fibroblasts derived from DD are altered by the culture conditions, lack cellular context and interactions, which are crucial for the development of fibrosis and weakly represent the derived tissue. Real-time PCR analysis of fibroblasts derived from control and DD samples show that little difference is detectable. 3D cultures of fibroblasts include addition of extracellular matrix that alters the native conditions of these cells.
As a way to characterize the fibrotic, proliferative properties of these resection specimens we have developed a 3D culture system, using intact human resections of the nodule part of the cord. The system is based on transwell plates with an attached nitrocellulose membrane that allows contact of the tissue with the medium but not with the plastic, thus, preventing the alteration of the tissue. No collagen gel or other extracellular matrix protein substrate is required. The tissue resection specimens maintain their viability and proliferative properties for 7 days. This is the first “organ” culture system that allows human resection specimens from DD patients to be grown ex vivo and functionally tested, recapitulating the in vivo situation.
Ziekte van Dupuytren (DD), een goedaardige fibroproliferatieve ziekte veroorzaakt permanente buiging van de vingers door de vorming van nodules en koorden in de palm van de hand. Hoewel de verspreiding van de ziekte is vooral hoog onder blanken van Noord-Europa, de onderliggende genetische etiologie van de ziekte is nog onbekend 1. Het belangrijkste kenmerk van DD de overproductie van extracellulaire matrix (ECM) eiwitten (bijvoorbeeld collageen), die een taai vezelig weefsel bezetten de ruimte tussen de pezen en de huid van de handpalm en vingers vormen permanent verstoren van de fijne bewegingen van de hand 2, 3. De terugkeer van de ziekte suggereert onderliggende genetische veranderingen als oorzaak van fibrose 1, 4. Een effectieve behandeling kan direct richten oncontroleerbare fibrotische mechanismen op cellulair en moleculair niveau.
Onze recente werkzaamheden fibrose heeft geleid tot de ontwikkeling van eenroman 3D-cultuur systeem dat op korte termijn de cultuur van de menselijke fibrotische weefsel mogelijk maakt met het potentieel van de dopingcontrole. Dit systeem heeft bijgedragen tot het beperken benadering 2D fibroblastculturen overwinnen en een rol voor de gedeeltelijke neerregulatie bereikt door exon skipping van TGFp route activatie bij het mediëren fibrose 5 definiëren.
We hebben een werkwijze cultuur ex vivo humane resectiepreparaten van DD patiënten om de interactie tussen myofibroblasten en het omliggende ECM 5, 6 kunnen onderzoeken. De studie van DD bindweefsel fibrose en andere fibrotische ziekten gebaseerd op histopathologische analyse van de uitgesneden chirurgische exemplaren, isolatie van fibroblasten uit het weefsel en de vestiging van primaire culturen of celsorteren procedures. Deze benaderingen zijn vrij statisch omdat zij geen exogene manipulatie van de ziekte eigenschappen of therapeutische interventie door de experimentator mogelijk. Daarnaast primaire cell culturen hebben de neiging zich aan te passen aan de omstandigheden cultuur en hun genexpressie eigenschappen verschillen wezenlijk van de in vivo situatie bij elke passage, zelfs tijdens de vroege passages (onder passage 3 en 6) 7, 8. We zijn erin geslaagd om het afval chirurgisch materiaal in stand te houden ex vivo kweekcondities gedurende een tijdsperiode die studie van de patiënt-specifieke kenmerken en screening van anti-fibrotische of anti-inflammatoir geneesmiddel verbindingen toestaat.
Het systeem is gebaseerd op een nitrocellulosemembraan dat contact van het weefsel met het medium, maar niet met de kunststof maakt dus het voorkomen van de wijziging van het weefsel na bevestiging, zoals eerder waargenomen wanneer kweken DD fibroblasten en andere celtypen 9. Nr collageengel of andere ECM eiwitten substraat vereist, aangezien de DD weefsel zelf produceert grote hoeveelheden van deze eiwitten. Dit is voordelig voor het onderhoud van natieve ECM micromilieu en omzet since matrix substraten belangrijke regulator van weefselarchitectuur en functie 10, 11. Zo ECM eiwitten zoals fibronectine, laminine en collageen, kunnen voor- achterzijde polariteit van fibroblasten zoals eveneens getoond apicale-basale polariteit in epitheliale cellen 12, 13 beïnvloeden . gepolariseerde cellen hebben asymmetrische verdeling van extracellulaire moleculen die celmigratie en genexpressie, bijvoorbeeld α1β1 integrine toegankelijkheid van het membraan beïnvloedt cel adhesie aan type I collageen 14 bepaalt. Aangezien een primaire doelstelling van deze 3D-model was de natieve micro behouden, werd geen kunstmatige ECM matrix substraat.
Kortom: resectiepreparaten gelijkelijk gesneden in een steriele omgeving en op nitrocellular membranen. Indien behandeling toegediend via injectie vereist het weefsel worden geïnjecteerd nadat deze op het membraan geplaatst. Als de behandeling niet behoeft te worden toegediend via injectie wordt de verbinding toegevoegd aan het kweekmedium (Dulbecco's gemodificeerd Eagle's medium (DMEM) met 1% foetaal kalfsserum (FCS), 1% penicilline-streptomycine (P / S)). De kweken worden gedurende maximaal tien dagen waarna het weefsel gefixeerd in 4% paraformaldehyde (PFA), behandeld met 30% sucrose-oplossing, ingebed in OCT verbinding en bewaard bij -80 ° C, zoals eerder beschreven 5.
De meest kritische stappen van het kweken van ex vivo humane bindweefsel zijn het onmiddellijke gebruik van het weefsel na operatieve verwijdering van de levensvatbaarheid te waarborgen; het weefsel moet in medium of zoutoplossing allen tijde; onderhouden van een steriele cultuur; dwarsdoorsneden van weefsel moet maximaal 200 urn dikte; instellen van de ex vivo kweken systeem optimaal wanneer het weefsel in contact met het medium, maar niet geheel onder water.
