The flow mediated dilation (FMD) test is the most commonly utilized, non-invasive, ultrasound assessment of endothelial function in humans. Although the FMD test has been related with the prediction of future cardiovascular disease and events, it is a physiological assessment with many inherent confounding factors that need to be considered.

Assim, uma correcta avaliação da função endotelial em humanos representa uma técnica importante que poderia ajudar a compreender a etiologia de várias patologias cardiovasculares, com o objectivo final de melhorar a eficácia do tratamento e prevenção da doença.

O endotélio é uma monocamada de células que sintetize numerosas substâncias vasoactivas, como o óxido nítrico (NO), prostaciclinas, endotelinas, factor de crescimento celular endotelial, interleucinas, e inibidores de plasminogénio 2. Tais factores que contribuem para a função do endotélio para regular a fluidez do sangue, o tónus vascular, a agregação de plaquetas, a permeabilidade de componentes do plasma e da parede do vaso inflamação 2-4. Além disso, o NO desempenha um papel anti-aterogênico fundamental na promoção da vasodilatação e manutenção da integridade endotelial. NO regula o tônus dos vasos e diâmetro através controlar o equilíbrio entre o fornecimento de oxigênio para os tecidos e seus 3,5 demanda metabólica. Existem múltiplos endógeno, exógeno, e factores estimuladores mecânicos que induzem a NO-sintase endotelial (eNOS) que sintetiza NO a partir de L-arginina 6,7. O estímulo mecânico mais notável é a tensão de cisalhamento. Parede de tensão de corte contribui para uma maior ativação de eNOS, resultando em produção de NO e posterior relaxamento da musculatura lisa 4. A disfunção do endotélio

O grau de disfunção do endotélio tem sido relacionada com a gravidade de várias doenças tais como hipertensão 10, 11 aterosclerose, acidente vascular cerebral isquémico 12, 13 diabetes, pré-eclampsia 14 ou doenças renais 15 entre outros. Deste modo, existe grande interesse, não só para avaliar as alterações na função endotelial ao longo do tempo, mas também na sequência de intervenções terapêuticas. Diferentes métodos têm sido utilizados paraavaliação clínica da função endotelial tanto invasiva (cateterismo cardíaco e pletismografia de oclusão venosa 3,16) e não-invasiva (mediada fluxo de dilatação, tonometria arterial e pulso análise contorno radial 4,17,18) em circulações coronárias e periféricas 19.

Dilatação mediada por fluxo (FMD) é um não-invasiva, a avaliação ultra-sónica da função endotelial e tem sido correlacionada com o desenvolvimento de problemas de saúde vascular. Desde a sua criação, em 1989, 20, a febre aftosa tem sido amplamente utilizada como um método confiável, in vivo para avaliar a função endotelial predominantemente mediado em humanos 19,21,22. De facto, o teste da febre aftosa artéria braquial foi associada com outras técnicas invasivas 23 e numerosas investigações descreveram uma forte relação inversa entre a febre aftosa e lesões cardiovasculares 24,25 de tal forma que individuos com mais de patologia vascular exibem uma FMD inferior 25.
