Abstract

Introduction
全身性硬化症(SSc)は、複雑で不均一な自己免疫疾患である。血管障害、レイノーの現象、皮膚および内臓の線維症として現れる可能性があります 1.SScは、限定された皮膚、びまん性皮膚、正中硬化症(皮膚関与なし)、およびSScオーバーラップ症候群1のサブタイプに分類される。
SScはメンデリアの方法では遺伝しませんが、遺伝的要因は病気に対する感受性に影響を与えるように見えます。入射率は民族間で異なり、疾患の家族歴を持つ個人の間で増加します2,,3.環境リスク因子も存在するように見え、シリカまたは有機溶媒への高い暴露がSSc4の発生を4増加するように見える。SScの世界的な普及率は約10,000の1 1です。男性よりも多くの女性がSScの影響を受け、報告された女性:男性比は3:1から8:1の間で、病気の発生率が最も高い年齢層は45〜54歳です。
肺はSSc 6患者において2番目に最も一般的に影響を受ける内臓器官である。SSc には、間質性肺疾患(ILD)と肺高血圧症の2 つの主な症状があります。ILDは、通常、繊維性です;それはSSc患者のおよそ80%で起こり、疾患11、88の限られた形態よりもびまん性皮性強皮症でより一般的である。肺高血圧症は、孤立した肺動脈性高血圧症(SScで13〜35%の有病率を有するPAH)または左心室の関与/拡張障害またはILD/低酸素血症に起因する肺高血圧症として現れる可能性がある7。抗体プロファイルは、SSc-ILD患者とSSc-PAH患者の間で異なります。例えば、抗Scl-70抗体の存在はSSc-ILD8と関連しているが、8一方、抗セントロメア抗体はPAH9を有さない患者よりもPAHを有するSSc患者においてより一般的である。
SSc-ILDの症状には、呼吸困難、咳、胸痛、運動制限などがあります。ILD は SSc 10,11,12,の罹患率に大きく貢献しています。その結果、SSc-ILD患者では、SSc-ILD患者では、それぞれ$31,285-55,446対$18,513-23,268の医療費が高いことが報告されています。
SSc-ILDはSSc患者の死亡率の主要な原因であり、このグループ10,14,の死亡の30〜35%を占める。SSc-ILD患者の生存期間の中央値は、5~8年10年、15年と報告されている。比較すると、SScを有する全人口の約76%が、発症後10年以上生存している。SSc-ILDにおける死亡率の有意な予測変数には、年齢、強制バイタル容量(FVC)、一酸化炭素(DLCO)の肺のベースライン拡散能力、高解像コンピュータ断層撮影(HRCT)上の疾患の程度、肺高血圧症の存在およびケルブ・フォン・デン・ルングレン6(KL-6)抗原17、18,18が含まれる。
早期診断は、治療を最小限の遅延で投与できるようにするために重要であり、進行性表現型の患者では、疾患の進行が遅くなる可能性がある。しかし、咳、呼吸困難、疲労の非特異的な症状は、心疾患や筋骨格系の関与など、SScの他の側面と間違えられる可能性があるため、SSc-ILDの診断は困難です。ILDの診断の評価には、臨床提示、歴史、喫煙状態、肺機能、画像診断、および場合によっては肺生検が含まれる。SSc-ILD診断の肯定には、しばしば組み合わせて使用されるいくつかの調査が必要です。最も頻繁に使用される評価には、肺機能検査とHRCT20、21、22、2321,22,が含まれます。20胸部X線撮影や放射線遠温画像撮影などの他の撮像方法(例えば、磁気共鳴画像[MRI]、肺超音波)も22を採用してもよい。肺機能試験は、ILDの重症度を評価し、その経過を監視するために使用されます。しかし、肺機能検査の使用だけでは、SSc-ILD24,25,25の診断には限られた用途があります。胸部のHRCTは、SSc-ILD19の鑑別診断を促進する最も敏感な非侵襲的手段と見なされる。ベースラインHRCTの結果は、時間の経過に伴う変化と同様に、肺疾患の将来の経過および治療26に対する潜在的な反応を予測するために使用することができる。
HRCTによる放射線への暴露は、定期的なスクリーニング27、28,28の制限因子として考えられる場合がある。スライス数を制限することは、放射リスクを低減する潜在的な方法であり、また、電圧または電流29、30、31,のいずれかを減少させることによって線29,量を減少させることができる。31あるいは、異なる評価方法が考慮されてもよい。例えば、MRIはILD患者22の評価とフォローアップのためのいくつかの可能性を持っているように見える。呼吸同期を伴うT2加重MRI画像を用いた1つの研究では、HRCTは「ゴールドスタンダード」評価と並行して行われた。ILD32の存在を決定するために、100%の感度および60%の特異性がMRIで報告された。ILDの検出と分類におけるMRIとHRCTの間の同様の一致が、別の研究33で報告された。有望な結果にもかかわらず、MRIは現在研究方法論であり、一般化された臨床使用の準備はまだ整っていません。
ここでは、SScにおける肺関与の診断、予後の決定、および画像化方法および結果の解釈を改善する可能性のある将来の発展を探るため、HRCTに焦点を当てたイメージング結果の解釈の実践的な概要を提供する。代表的なケースからのHRCT画像が論文に含まれています。
Protocol
1. HRCTスキャン
- 胸部36の体積HRCT取得スキャンを行う。造影剤は36、37は37必要ない。
- 表 136,37に示すパラメータを使用して、以下の取得を取得します。
- 肺アピセスから肺ベースへの腹筋刺激スキャン(容積)を取得する。
- 肺アピセスから肺ベースまでの2cmから、サピーヌの呼気スキャン(10~20mmの隙間で順次)を取得します。

- 完全なインスピレーション35、36で吸気スキャンを36取得します。
- 最も薄いコリメーション、最短回転時間、最高ピッチを使用して、動きのない画像が36を取得するようにします。推奨されるスキャンパラメータの詳細は、表 137に記載されています。
- 容積測定スキャンの最適な品質を得るために、高空間分解能再構成35,36,36の薄い断面(<2 mm)画像を得る。
- 取得直後にスキャンを確認し、モーションアーティファクトが存在するか、または不十分なインスピレーションが発生した場合に繰り返します37.
2. 報告

Representative Results
診断
HRCT上のSSc-ILDの主な特徴は、一般的に、周縁的な地上ガラスの不透明性および広範な牽引性気管支拡張症を有する非特異的間質性肺炎(NSIP)パターンを含む(図1および図2)。グラウンドガラスの不透明性は広い病因を有し、多くの場合、非特異的な40、41、42,41,42である。中央優位または胸下の予備を伴う周辺分布は、NSIPを非常に示唆している(図3)。
典型的には、HRCT画像におけるILDパターンには、牽引性気管支拡張症/気管支拡張症(線維性形態のNSIPと一致する)をもたらす建築歪みを伴う網状化が含まれる。確かに牽引気管支拡張症および牽引気管支性拡張症は、多くの場合、SSc-ILDの主要な特徴である(図4)43。43その他の知見には、ハニカム(図5;SScの限られた形態でより一般的)、眼間中隔肥厚および眼内線、およびミクロノドゥル40、44,44が含まれる。ハニカムは、厚く、よく定義された壁31を有する典型的に一貫した直径(〜3〜10mm)のクラスター化された嚢胞性空域を指す。ハニカムおよび牽引気管支拡張症は、HRCT上の通常の間質性肺炎(UIP)の重要な特徴である。このパターンは、最も一般的には特発性肺線維症(IPF)と関連しているが、このプロトタイプは進行性表現型を有するILDを線維化し、SSc-ILD10患者に10見られることがある。最近、結合組織疾患関連ILD(SSc-ILDを含む)およびHRCTのUIPパターンを有する患者ではいくつかの徴候が確認されているが、IPFを有する患者では同定されていない。これらは、直線エッジサイン(すなわち、コロナ画像上の肺の横縁に沿って実質的な延長を伴わない頭蓋突起面に鋭い境界を持つ肺塩基への線維症の単離)、ハニカム優勢(または活気に満ちた)サイン(>肺の線維部分の70%)、および前側上部葉(i.e.e.) 上葉の前葉の前側面内の線維症の濃度は、上葉の他の側面の相対的な控え、およびそれに付随する低ローブ関与)45。直線エッジサインは、SSc-ILD10の主要なCTパターンであるNSIP病理46にも関連付けられている。
拡張空気充填食道は、SSc(図6)47、48、49の患者およびSSc-ILD48,47、48,の患者において頻47繁に観察される。,4849拡張食道がSSc-ILDとIPFを区別するのに役立たない可能性のある上限は認められませんが、拡張食道は食道運動障害の発生率が増加するため、65歳以上の患者では解釈が難しい場合があります。リンパ節の短軸が10mmを超える中隔リンパ節障害(通常反応性)は、SSc-ILD47,50,50の患者においてもしばしば観察される。隣接する上昇大動脈より大きい肺動脈サイズは、肺高血圧症が共存することを示唆している(図6)、線維性肺疾患51、52、5352,53を有しない患者においても。51統合の領域は、重ね合わせ感染、吸引、肺炎、出血または悪性腫瘍を組織することを示唆している。小結節はSSc-ILD7における肺癌のリスク増加のために監視されなければならない。SSc-ILD患者に生じる最も一般的な原発癌は腺癌77,5454である。
SSc-ILD は、IPF15, 55,と多くの臨床的、機械学的、および病理学的類似点を共有しています。しかし、一部の放射線機能では、これら 2 つの ILD15, 45,の分化が可能です。SSc-ILDでは、IPFと比較して、地盤ガラスの不透明度および線維化の割合が低いほど粗い。SScにおけるUIPの場合、蜂巣は線維性肺組織の70%以上で観察され得る─ 活気に満ちた蜜蜂の巣のサイン5656,57。57さらに、四隅の符号 (前葉記号とも呼ばれる) は、IPF よりも SSc-ILD ではかなり一般的です。これは、二国間の前側上葉および後方性下葉58を局所的または不釣り合いに関与する炎症および/または線維症のパターンである。
胸部X線写真は、最初にILDを検出することがあります。しかし、信頼性の高い診断のためのコントラスト解像度が十分ではありません。SSc-ILD患者からの胸部X線写真では、最も頻繁なパターンは基礎優勢な網状分解59である。さらなる特徴は、目に見える気管支拡張症、体積損失および蜜蜂の巣を含んでいてもよい。HRCTと同様に、拡張空気で満たされた食道の存在は、SSc-ILD47の診断を支援するのに役立つかもしれません。
予後
いくつかの異なるイメージングの所見は、SSc-ILDにおける予後に関連していることが示されている。死亡リスクは、HRCTに対して少なくとも20%の疾患程度の患者においてより高いことが報告されている(10年生存率は、それぞれ43%対67%であり、疾患範囲が20%を下回る患者では20%の閾値を下回る)60。同様に、HRCT上の高い線維化スコア(網状化および蜜蜂の巣の程度に基づく)は、死亡率の増加61と関連している。大きな食道の直径は、ILDの重症度の増加およびDLCO48の減少に関連している。肺密度および肺動脈径は、肺高血圧症62のリスクを予測するために使用される可能性がある。コンピュータ化された定量的CTパラメータを利用して、肺機能の低下または死亡のリスクを特定することもできます。ある研究は、HRCTから定量化されたILDの程度が、12ヶ月63日のFVCの減少を予測するために使用できることを示唆した。別の研究では、定量的な胸部CTパラメータは、臨床予測モデル64と一致した死亡リスク結果を提供した。その明らかな可能性にもかかわらず、イメージングベースのバイオマーカーは、現在、個々の患者における臨床有用性が確立されていないので、集団レベルで最もよく考慮されている。
治療応答
シクロホスファミドおよびミコフェノールトモフェチルは、SSc-ILD患者に控えめな利益をもたらす。画期的な強皮症肺研究Iでは、シクロホスファミド治療はプラセボ65と比較して線維症の進行を遅くすることにつながった。さらに最近では、強皮症肺研究IIは、シクロホスファミド66と比較してミコフェノール酸モフェチルと同様の有効性および改善された耐用性を報告した。しかし、SSc-ILD患者に対する治療オプションの改善が依然として必要である。現在調査中の治療法には、モノクローナル抗体(例えばリツキシマブ、アビツズマブ)、抗線維化剤(例えば、ニンテダニブ、ピルフェニドン)、直接トロンビン阻害剤ダルビガトラン、プロテアソーム阻害剤ボルテゾミブ、および造血幹細胞移植19、67,67が含まれる。

Discussion
HRCTは現在、SSc-ILDを診断および評価するための決定的なイメージング方法ですが、電離放射線を使用し、比較的高価です。胸部X線写真は代わりに行われるかもしれないが、これらはHRCTと同じ程度に鑑別診断を促進するものではなく、通常の胸部X線写真はILDの可能性を排除しない。おそらく、胸部X線写真の最良の使用は、HRCTスキャンの間の進行性疾患を監視することと、症状の急性悪化の設定において、感染性肺炎などの複雑な疾患の排除を監視することです。
HRCTの知覚限界は放射線被ばくである。先に述べたように、CTスキャンを行う新しい方法は、放射線被ばくを31に減らすことを可能にするかもしれないし、さらに、現在のCTスキャナは、ほぼ胸部X線検査レベルへの放射線被ばくを減らすために将来的に可能性を提供する高度な技術の配列を提供する。あるいは、MRIや肺超音波のような画像化方法は、将来的に患者を放射線にさらすことを避けるために使用される可能性がある32,71,,72,,73.イメージングの利用に伴うリスク・ベネフィットの考慮事項はありますが、診断および患者管理におけるCTの利点は、潜在的なリスクをはるかに上回ると考えています。
イメージングデータ、特にHRCTは、SSc-ILDの診断を可能にする最も重要な情報を提供します。HRCTスキャンのパターンと特徴の詳細な検討は、通常、侵襲的な生検手順の必要性を避けるという利点を持つ他の肺疾患とSSc-ILDを区別するのに十分である。
HRCTスキャンの視覚的評価は、ある程度の主観性と観察者間変動の可能性を導入する。HRCTスキャン解釈のコンピュータベースの方法は、精度を向上させる可能なアプローチとして調査されています63,,74.例えば、肺線維症の評価または疾患の程度の評価に対する定量的アプローチは、治療応答68、70、75,70,75を評価するために使用され得る。しかし、これらの方法は、この時点で毎日の臨床現場で広く使用されていません。
