Ontwikkeling van grote bijwerkingen cardiovasculaire voorvallen, die van invloed zijn cardiovasculaire prognose na coronaire angioplastie, worden beïnvloed door de omvang van coronaire schade en vasculaire reparatie. Het gebruik van nieuwe coronaire cellulaire en oplosbare biomarkers, reactief op vasculaire schade en reparatie, zijn nuttig om de ontwikkeling van MacEs en prognose te voorspellen.
Grote bijwerkingen (MACEs) hebben een negatieve invloed op de cardiovasculaire prognose van patiënten die coronaire angioplastie ondergaan als gevolg van coronaire ischemische letsel. De omvang van coronaire schade en de mechanismen van vasculaire reparatie zijn factoren die van invloed zijn op de toekomstige ontwikkeling van MACE’s. Intrinsieke vasculaire kenmerken zoals de plaque kenmerken en coronaire slagader complexiteit hebben aangetoond prognostische informatie voor MACEs. Het gebruik van intracoronaire circulerende biomarkers is echter als een handige methode voor de vroege identificatie en prognose van MACE’s geposteerd, omdat ze beter aansluiten bij dynamische mechanismen waarbij coronaire schade en herstel betrokken zijn. Bepaling van coronaire circulerende biomarkers tijdens angioplastie, zoals het aantal subpopulaties van mononucleaire voorlopercellen (Mp’s) en de concentratie van oplosbare moleculen die ontstekingen, celhechting en reparatie reflecteren, beoordeling van toekomstige ontwikkelingen en de prognose van MACE’s 6 maanden na coronaire angioplastie. Deze methode wordt benadrukt door zijn translationele aard en betere prestaties dan perifere bloed circulerende biomarkers met betrekking tot de voorspelling van MACE’s en het effect ervan op de cardiovasculaire prognose, die kan worden toegepast voor risicogelaagdheid van patiënten met coronaire hartziekte die angioplastie ondergaat.
Coronaire angioplastie en stenting vormen een bergingsprocedure voor patiënten met coronaire hartziekten (CAD). Echter, grote bijwerkingen cardiovasculaire voorvallen (MacEs), met inbegrip van cardiovasculaire dood, hartinfarct, coronaire restenose, en episodes van angina of decompenseren hartfalen, kan maanden na coronaire interventie optreden, waardoor ongeplande bezoeken aan het ziekenhuis. Maces komen wereldwijd vaak voor en hun morbisterfte is hoog1.
Coronaire ischemische schade veroorzaakt vroege vasculaire respons en herstellende mechanismen waarbij mobilisatie van MpCs als gevolg van hun differentiatie vermogen en / of angio-herstellend potentieel, evenals de productie van oplosbare moleculen zoals intercellulaire hechting moleculen (ICAMs), matrix metalloproteinases (MMP’s), en reactieve zuurstofsoorten, als gevolg van celverkleving, weefsel remodelleren, en oxidatieve stress. Hoewel intrinsieke vasculaire kenmerken zoals plaquekenmerken en coronaire hartcomplexiteit zijn gebruikt om MACEs te voorspellen, hebben sommige studies gesuggereerd dat biomarkers met betrekking tot de mechanismen van letsel en herstel die zich voordoen in het coronaire endotheel zeer nuttig kunnen zijn voor de vroege identificatie en prognose van cardiovasculaire voorvallen bij patiënten met CAD die aan coronaire angioplastie2,3,4,5worden voorgelegd.
Voortdurende interesse in het begrijpen van de mechanismen die ten grondslag liggen aan CAD letsel en reparatie heeft onderzoekers gemotiveerd om intracoronaire circulerende biomarkers te bestuderen, omdat coronaire bemonstering nauwer vasculaire schade weerspiegelt enherstel 6. Echter, karakterisering van coronaire biomarkers in menselijke studies is schaars7,8,9. Daarom was het doel van deze studie om een methode te beschrijven om de hoeveelheid coronaire circulerende MpCs en oplosbare moleculen te bepalen, als gevolg van zowel vasculairletsel als herstel, en om aan te tonen of deze biomarkers geassocieerd zijn met MACEs en de klinische prognose van CAD-patiënten die coronaire angioplastie ondergingen. Deze methode is gebaseerd op het gebruik van vasculaire, circulerende MpCs en oplosbare moleculen verkregen door bemonsteringslocaties die het dichtst bij de schade aan het vat staan. Het kan ook nuttig zijn voor klinische studies voor onderste ledematen ischemie, beroerte, vasculitis, veneuze trombose, en andere verwondingen waarbij vasculaire letsel en reparatie.
Bloedafname van de aangetaste kransslagader kan moeilijk zijn. Soms is de kransslagader nauwelijks toegankelijk. In dit geval kan bemonstering van de veneuze sinus een alternatief zijn. We hebben validatietests uitgevoerd waarbij circulerende biomarkers in kransslagader vergeleken met veneuze sinus, zonder significante verschillen.
