Het doel van dit protocol is om een dynamische co-cultuur van menselijke macrofagen en myofibroblasten in buisvormige elektrospunsteigers uit te voeren om materiaalgestuurde weefselregeneratie te onderzoeken, met behulp van een bioreactor die het ontkoppelen van schuifspanning en cyclische rek mogelijk maakt.
Het gebruik van resorbeerbare biomaterialen om regeneratie direct in het lichaam te induceren is een aantrekkelijke strategie vanuit een translationeel perspectief. Dergelijke materialen induceren een ontstekingsreactie bij implantatie, die de oorzaak is van daaropvolgende resorptie van het materiaal en de regeneratie van nieuw weefsel. Deze strategie, ook bekend als in situ tissue engineering, wordt nagestreefd om cardiovasculaire vervangingen zoals weefsel-engineered vasculaire grafts te verkrijgen. Zowel de ontstekings- als de regeneratieve processen worden bepaald door de lokale biomechanische signalen op de steiger (d.w.z. rek- en schuifspanning). Hier beschrijven we in detail het gebruik van een op maat ontwikkelde bioreactor die op unieke wijze het ontkoppelen van rek- en schuifspanning op een buisvormige steiger mogelijk maakt. Dit maakt een systematische en gestandaardiseerde evaluatie mogelijk van het ontstekings- en regeneratieve vermogen van buisvormige steigers onder invloed van goed gecontroleerde mechanische belastingen, wat we aantonen op basis van een dynamisch co-cultuurexperiment met menselijke macrofagen en myofibroblasten. De belangrijkste praktische stappen in deze aanpak – de bouw en het opzetten van de bioreactor, de voorbereiding van de steigers en het zaaien van cellen, het aanbrengen en onderhouden van rek- en afschuifstroom en het oogsten van monsters voor analyse – worden in detail besproken.
Cardiovasculaire weefseltechniek (TE) wordt nagestreefd als een alternatieve behandelingsoptie voor de momenteel gebruikte permanente cardiovasculaire prothesen (bijv. vasculaire grafts, hartklepvervangingen), die suboptimaal zijn voor grote cohorten van patiënten1,2,3,4. Veelgevraagde toepassingen zijn weefselgeïnjeneerde vasculaire grafts (TEVGs)5,6 en hartkleppen (TEHVs)7,8. Meestal maken cardiovasculaire TE-methodieken gebruik van resorbeerbare biomaterialen (natuurlijk of synthetisch) die dienen als een leerzame steiger voor het nieuwe weefsel dat moet worden gevormd. De vorming van nieuw weefsel kan volledig in vitro worden ontworpen, door de steiger met cellen te zaaien en te cultiveren in een bioreactor voorafgaand aan implantatie (in vitro TE)9,10,11, of direct in situ, waarbij de synthetische steiger wordt geïmplanteerd zonder voorkweek om de vorming van nieuw weefsel rechtstreeks in het lichaam te induceren (in situ TE)12,13,14. Voor zowel in vitro als in situ cardiovasculaire TE-benaderingen is succesvolle functionele regeneratie dominant afhankelijk van zowel de immuunrespons van de gastheer op de geïmplanteerde constructie als de juiste biomechanische belasting.
Het belang van biomechanische belasting voor cardiovasculaire TE wordt algemeen erkend15. In het geval van cardiovasculaire implantaten worden de cellen die de steiger bevolken blootgesteld aan cyclische rek- en schuifspanningen die ontstaan als gevolg van de hemodynamische omgeving. Talrijke studies hebben het stimulerende effect van (cyclische) rek op de vorming van matrixcomponenten, zoals collageen16, 17,18,19,glycosaminoglycanen (GAGs)20, en elastine21,22, door verschillende celtypen gerapporteerd. Huang et al. toonden bijvoorbeeld aan dat biaxiale stretch de afzetting en organisatie van collageen en elastine in in vitro TEVGs verhoogde met behulp van een vasculaire bioreactor23. Hoewel de nadruk meestal ligt op rek als dominante belasting, maken deze studies vaak gebruik van door debiet aangedreven bioreactoren waarin het monster ook wordt blootgesteld aan afschuifstroom. Hoewel er relatief weinig bekend is over de geïsoleerde invloed van afschuifspanningen op weefselvorming en ontsteking in 3D, zijn er enkele gegevens beschikbaar. Zo toonden Hinderer et al. en Eoh et al. aan dat afschuifstroom, naast een 3D steigermicrostructuur, belangrijk was voor de vorming van volwassen elastine door menselijke vasculaire gladde spiercellen in een in vitro modelsysteem24,25. Al met al illustreren deze bevindingen de relevantie van zowel cyclische rek- als afschuifspanning voor cardiovasculaire TE.
Een andere belangrijke bepalende factor voor het succes of falen van TE-implantaten is de immuunrespons van de gastheer op de geïmplanteerdeent 26. Dit is met name belangrijk voor materiaalgestuurde TE-strategieën in situ, die eigenlijk afhankelijk zijn van de acute ontstekingsreactie op de steiger om de daaropvolgende processen van cellulaire instroom en endogene weefselvorming en remodellering op gang te brengen27. De macrofaag is een kritische initiator van functionele weefselregeneratie, wat is aangetoond door meerdere studies28,29,30. Analoog aan wondgenezing wordt de regeneratie van weefsel geregeld door paracrinesignalering tussen macrofagen en weefselproducerende cellen zoals fibroblasten en myofibroblasten31,32,33. Naast het coördineren van nieuwe weefseldepositie zijn macrofagen betrokken bij de actieve resorptie van vreemd steigermateriaal34,35. Als zodanig is de in vitro macrofaagrespons op een biomateriaal geïdentificeerd als een voorspellende parameter voor het in vivo succes van implantaten36,37,38.
De macrofaagrespons op een geïmplanteerde steiger is afhankelijk van steigerontwerpkenmerken zoals materiaalsamenstelling en microstructuur35,39,40. Naast steigereigenschappen wordt ook de macrofaagrespons op een steiger en hun overspraak met myofibroblasten beïnvloed door hemodynamische belastingen. Cyclische stretch bleek bijvoorbeeld een belangrijke modulator te zijn van macrofaagfenotype41,42,43,44 en de afscheiding van cytokinen43,44,45,46 in 3D-elektrospunsteigers. Battiston et al. toonden met behulp van een co-kweeksysteem van macrofagen en vasculaire gladde spiercellen aan dat de aanwezigheid van macrofagen leidde tot verhoogde niveaus van elastine en GAG’s en dat matige niveaus van cyclische stretch (1,07-1,10) de afzetting van collageen I en elastine47stimuleerden . In eerdere werken hebben we aangetoond dat schuifspanning een belangrijke determinant is voor monocytwerving in 3D-elektrospunsteigers48,49, en dat zowel schuifspanning als cyclische rek de paracrinesignalering tussen menselijke monocyten en mesenchymale stromale cellenbeïnvloeden 50. Fahy et al. toonden aan dat de afschuifstroom de afscheiding van pro-inflammatoire cytokinen door menselijke monocyten verhoogde51.
Al met al toont het bovenstaande bewijs aan dat een adequaat begrip van en controle over hemodynamische belastingen cruciaal is voor cardiovasculaire TE, en dat het belangrijk is om de ontstekingsreactie te overwegen om dit te bereiken. Talrijke bioreactoren zijn eerder beschreven voor de in vitro52,53,54,55,56,57,58 of ex vivo59,60,61 kweek van cardiovasculaire weefsels. Al deze systemen zijn echter ontworpen om de fysiologische hemodynamische belastingsomstandigheden zoveel mogelijk na te bootsen. Hoewel dit zeer waardevol is voor het creëren van cardiovasculaire weefsels in vitro of het handhaven van ex vivo-culturen, maken dergelijke systemen geen systematisch onderzoek naar de individuele effecten van individuele signalen mogelijk. Dit komt omdat de toepassing van zowel cyclische rek- als schuifspanning in deze bioreactoren wordt aangedreven door dezelfde drukstroom, die ze intrinsiek verbindt. Hoewel microsystemen die nauwkeurige multi-cue mechanische manipulatie mogelijk maken, zijn beschreven voor 2D-substraten62 of 3D-hydrogelopstellingen63,64, maken dergelijke opstellingen het niet mogelijk om elastomeer 3D-biomateriaalsteigers op te zetten.
Hier presenteren we de toepassing van een buisvormig bioreactorsysteem dat op unieke wijze de ontkoppeling van schuifspanning en cyclische rek mogelijk maakt en helpt om hun individuele en gecombineerde effecten mechanistisch te onderzoeken. Dit systeem maakt het mogelijk om een breed scala aan weefselgeënskundig vasculaire grafts te testen (bijv. synthetische of natuurlijke oorsprong, verschillende micro-architectuur, verschillende porositeiten). Om de toepassing van afschuifspanning en rek effectief los te ontkoppelen, zijn de belangrijkste concepten die de bioreactor gebruikt (1) scheiding van de controle van schuifspanning en rek met behulp van verschillende pompsystemen en (2) stimulatie van de steigers op een ‘inside-out’ manier met rekengestuurde afmetingen. Debiet wordt aangebracht op het buitenoppervlak van de buissteiger door het gebruik van een stromingspomp, terwijl de omtrek van de steiger wordt veroorzaakt door het uitzetten van een siliconenbuis waarop de steiger is gemonteerd door het gebruik van een aparte stampomp. De afmetingen van de siliconenbuis en de glazen buis die de constructie bevat, worden zorgvuldig gekozen en gevalideerd met behulp van computationele vloeistofdynamicasimulaties, om ervoor te zorgen dat de schuifspanning op de steiger (als gevolg van stroming) en de omtrekspanning (als gevolg van buisuitbreiding) elkaar niet significant beïnvloeden. Dit inside-out ontwerp heeft verschillende praktische redenen. Als stretch wordt toegepast door de lichtgevende vloeistofdruk (vergelijkbaar met fysiologische belasting), vereist dit inherent dat het monsterontwerp lekvrij is. Bovendien zou de druk die nodig is om het monster uit te rekken volledig worden bepaald door de stijfheid van het monster, die in de loop van de tijd tussen de monsters en binnen een monster kan variëren, waardoor het moeilijk is om de rek te controleren. Deze bioreactor monteert het weefsel ontworpen transplantaat rond een siliconenbuis en maakt het mogelijk om muurschaarspanning (WSS) aan te brengen op de buitenwand van de graft en zet de ent van binnenuit onder druk. Op deze manier kunnen gelijke belastingsomstandigheden tussen monsters en binnen monsters in de loop van de tijd worden gegarandeerd, en bovendien mogen de monsters lekken, zoals gebruikelijk is voor poreuze vasculaire steigers19. Deze inside-out bioreactor is specifiek bedoeld voor systematisch onderzoek naar de effecten van afschuiven en/of rekken, in plaats van de engineering van een native-achtig bloedvat in vitro, waarvoor traditionele vasculaire bioreactoropstellingen geschikter zijn. Zie figuur 1A–B voor de ontwerptekeningen van de bioreactor en de bijbehorende tabel 1 voor een functionele beschrijving en motivering achter de belangrijkste componenten van de bioreactor.
Het gebruik van de bioreactor wordt aangetoond op basis van een reeks recente studies van onze groep waarin we de individuele en gecombineerde invloeden van schuifspanning en cyclische rek op ontsteking en weefselvorming in resorbeerbare elektrospunsteigers voor in situ cardiovasculair weefsel19,43,44onderzochten . Daartoe gebruikten we menselijke macrofagen en myofibroblasten in mono- of in co-cultuur om de verschillende fasen van de regeneratieve cascade in situ te simuleren. We hebben aangetoond dat cytokinesecretie door menselijke macrofagen duidelijk wordt beïnvloed door zowel cyclische rek- als schuifspanning, die de matrixdepositie en organisatie door menselijke myofibroblasten in deze steigers beïnvloedt, zowel via paracrinesignalering als direct contact19,43,44. Met name deze studies toonden aan dat in het geval van gecombineerde toepassing van afschuifspanning en rek, de effecten op weefselvorming en ontsteking worden gedomineerd door een van de twee belastingen, of dat er synergetische effecten van beide belastingen zijn. Deze bevindingen illustreren de relevantie van het ontkoppelen van beide belastingen om een beter inzicht te krijgen in de bijdrage van de mechanische omgeving aan TE-processen. Dit begrip kan worden toegepast om steigerontwerpparameters systematisch te optimaliseren in relevante hemodynamische belastingsregimes. Bovendien kunnen de mechanistische gegevens uit dergelijke goed gecontroleerde omgevingen dienen als input voor numerieke modellen die worden ontwikkeld om het verloop van in situ weefselremodellering te voorspellen, zoals onlangs gerapporteerd voor TEVGs65 of TEHVs66, om de voorspellende capaciteit verder te verbeteren.
De hierin beschreven bioreactor maakt een systematische evaluatie mogelijk van de bijdragen van de individuele en gecombineerde effecten van schuifspanning en cyclische rek op ontsteking en weefselregeneratie in buisvormige resorbeerbare steigers. Deze aanpak maakt het ook mogelijk om een grote verscheidenheid aan analyses uit te voeren op vasculaire constructies, zoals geïllustreerd in de sectie representatieve resultaten.
