This article describes a rat model of electrically-induced ventricular fibrillation and resuscitation by chest compression, ventilation, and delivery of electrical shocks that simulates an episode of sudden cardiac arrest and conventional cardiopulmonary resuscitation. The model enables gathering insights on the pathophysiology of cardiac arrest and exploration of new resuscitation strategies.
A rat model of electrically-induced ventricular fibrillation followed by cardiac resuscitation using a closed chest technique that incorporates the basic components of cardiopulmonary resuscitation in humans is herein described. The model was developed in 1988 and has been used in approximately 70 peer-reviewed publications examining a myriad of resuscitation aspects including its physiology and pathophysiology, determinants of resuscitability, pharmacologic interventions, and even the effects of cell therapies. The model featured in this presentation includes: (1) vascular catheterization to measure aortic and right atrial pressures, to measure cardiac output by thermodilution, and to electrically induce ventricular fibrillation; and (2) tracheal intubation for positive pressure ventilation with oxygen enriched gas and assessment of the end-tidal CO2. A typical sequence of intervention entails: (1) electrical induction of ventricular fibrillation, (2) chest compression using a mechanical piston device concomitantly with positive pressure ventilation delivering oxygen-enriched gas, (3) electrical shocks to terminate ventricular fibrillation and reestablish cardiac activity, (4) assessment of post-resuscitation hemodynamic and metabolic function, and (5) assessment of survival and recovery of organ function. A robust inventory of measurements is available that includes – but is not limited to – hemodynamic, metabolic, and tissue measurements. The model has been highly effective in developing new resuscitation concepts and examining novel therapeutic interventions before their testing in larger and translationally more relevant animal models of cardiac arrest and resuscitation.

Dierlijke modusls van een hartstilstand spelen een cruciale rol in de ontwikkeling van nieuwe therapieën reanimatie door het verstrekken van inzichten over de pathofysiologie van een hartstilstand en reanimatie en het bieden van praktische middelen te conceptualiseren en testen van nieuwe interventies voordat ze kunnen worden getest bij mensen 3. De rat-model van de gesloten kist reanimatie (CPR), hier beschreven heeft een belangrijke rol gespeeld. Het model werd in 1988 ontwikkeld door Irene von Planta – een research fellow bij de tijd – en haar medewerkers 4 in het laboratorium van wijlen professor Max Harry Weil MD, Ph.D. aan de Universiteit van Health Sciences (omgedoopt tot Rosalind Franklin University of Medicine and Science in 2004) en wordt veelvuldig gebruikt op het gebied van reanimatie voornamelijk door fellows van professor Weil en hun stagiairs.
Het model simuleert een episode van een plotselinge hartstilstand met reanimatie geprobeerd door conventionele reanimatie technieken en omvat dus induction van ventriculaire fibrillatie (VF) door middel van een elektrische stroom naar de rechter ventrikel endocardium en levering van gesloten borst CPR door een pneumatisch aangedreven zuiger apparaat terwijl gelijktijdig leveren beademing met zuurstof verrijkt gas. Beëindiging van VF wordt bereikt door transthoracale levering van elektrische schokken. De rat-model biedt een evenwicht tussen de ontwikkelde modellen in grote dieren (bijvoorbeeld, varkens) en modellen ontwikkeld in kleinere dieren (bijvoorbeeld muizen) waardoor verkenning van nieuwe concepten in het onderzoek een goed gestandaardiseerde, reproduceerbare en efficiënte manier met toegang tot een robuuste inventaris van relevante metingen. Het model is met name nuttig in de vroege stadia van het onderzoek naar nieuwe concepten van de effecten van confounders te verkennen en te onderzoeken voordat het uitvoeren van studies in grotere diermodellen die duurder zijn, maar van grotere translationeel impact.
Een Medline voor alle peer-reviewed artikelen rapportage alsimilar rat model met VF als het mechanisme van een hartstilstand en een vorm van gesloten kist reanimatie onthulden totaal 69 extra oorspronkelijke studies met behulp van het model, omdat het voor het eerst werd gepubliceerd in 1988 4. De onderzoeksgebieden zijn onder pathofysiologische aspecten van reanimatie 5-17, factoren uitkomsten 18-30, de rol van farmacologische interventies onderzoeken vasopressormiddelen 31-43, buffermiddelen 44, 45 inotrope middelen, middelen ter myocardiale en cerebrale bescherming 46-70, en ook de effecten van mesenchymale stamcellen 71-73.
Het model en protocol beschreven in dit artikel wordt momenteel gebruikt bij de reanimatie Instituut. Toch zijn er meerdere mogelijkheden om "aanpassen" het model op basis van de beschikbare individuele onderzoekers en de doelstellingen van de studies mogelijkheden.
Kritische stappen in het protocol
