This manuscript describes the creation of defined engineered cardiac tissues using surface marker expression and cell sorting. The defined tissues can then be used in a multi-tissue bioreactor to investigate mechanisms of cardiac cell therapy in order to provide a functional, yet controlled, model system of the human heart.
Human cardiac tissue engineering can fundamentally impact therapeutic discovery through the development of new species-specific screening systems that replicate the biofidelity of three-dimensional native human myocardium, while also enabling a controlled level of biological complexity, and allowing non-destructive longitudinal monitoring of tissue contractile function. Initially, human engineered cardiac tissues (hECT) were created using the entire cell population obtained from directed differentiation of human pluripotent stem cells, which typically yielded less than 50% cardiomyocytes. However, to create reliable predictive models of human myocardium, and to elucidate mechanisms of heterocellular interaction, it is essential to accurately control the biological composition in engineered tissues.
To address this limitation, we utilize live cell sorting for the cardiac surface marker SIRPα and the fibroblast marker CD90 to create tissues containing a 3:1 ratio of these cell types, respectively, that are then mixed together and added to a collagen-based matrix solution. Resulting hECTs are, thus, completely defined in both their cellular and extracellular matrix composition.
Here we describe the construction of defined hECTs as a model system to understand mechanisms of cell-cell interactions in cell therapies, using an example of human bone marrow-derived mesenchymal stem cells (hMSC) that are currently being used in human clinical trials. The defined tissue composition is imperative to understand how the hMSCs may be interacting with the endogenous cardiac cell types to enhance tissue function. A bioreactor system is also described that simultaneously cultures six hECTs in parallel, permitting more efficient use of the cells after sorting.
Cardiale tissue engineering is sterk gevorderd in de afgelopen tien jaar, met meerdere groepen publiceren van resultaten van volledig functioneel, het verslaan van weefsels, vervaardigd van zowel muizen hartspiercellen 1-6 en, meer recentelijk, menselijke stamcel-afgeleide cardiomyocyten 7-12. Het hartweefsel vakgebied dat wordt aangedreven door twee primaire en wezen onafhankelijk doelen: 1) om exogene transplantaten die kunnen worden getransplanteerd in falende harten functioneren 4-6 verbeterd; en 2) de ontwikkeling in vitro modellen voor het bestuderen van de fysiologie en ziekte, of screeningsmethoden voor therapeutische ontwikkeling 2,7.
Drie-dimensionale (3-D) celkweek is essentieel voor de ontwikkeling van nieuwe generatie screeningsmethoden, als de 3-D matrix geeft een natuurlijker cardiale micro dan traditionele 2-D monolaag celcultuur beschouwd; inderdaad een aantal aspecten van celbiologie totaal andere 2-D versus 3-D kweken 13,14 </sup>. Bovendien zijn ontworpen hartweefsel opgebouwd uit volledig gedefinieerde componenten: een extracellulaire matrix, en een celpopulatie. Voor traditionele gemanipuleerde menselijke cardiale weefsels, terwijl de extracellulaire matrix samenstelling (gewoonlijk fibrine 9 of collageen 7,8,10) streng wordt gecontroleerd, de invoercel samenstelling minder goed gedefinieerd, door het hele mengsel van cellen van een gerichte cardiale differentiatie van beide embryonale stamcellen (ESC 7,9) of geïnduceerde pluripotente stamcellen (iPSC 10,12) worden toegevoegd aan de weefsels. Afhankelijk van de specifieke cellijn en de efficiëntie van de differentiatie protocol, kan de resulterende percentage hartspiercellen variëren van minder dan 25% tot meer dan 90%, de specifieke hartspierfenotype (dwz ventricular-, atriaal of gangmaker-achtig) kan ook variëren, zelfs de niet-cardiomyocyt fractie kan zeer heterogeen 15,16 en de looptijd van de gedifferentieerde cardiale m veranderenyocytes 17.
Recente hartweefsel engineering heeft getracht de ingang celpopulatie controleren, met ofwel een cardiale reporter humane embryonale stamcellijn 8 of celoppervlak markers 18 wordt gebruikt om de hartspiercel component van de differentiatie isoleren. Hoewel aanvankelijk een weefsel uit slechts cardiomyocyten lijkt het ideaal zijn, is dit in feite niet het geval; hECTs uitsluitend uit cardiomyocyten niet comprimeren tot functionele weefsels met bepaalde groepen vinden van een 3: 1 verhouding van cardiale myocyten: fibroblasten die de hoogste twitch kracht 8. Via verschillende celselectie methoden, waaronder oppervlakte merkers voor levende celsortering, kan men hECTs met gedefinieerde celpopulaties maken. Terwijl markers van niet-cardiale stromacellen enige tijd beschikbaar CD90 19,20 zijn, zoals de vermeende fibroblast marker zijn oppervlaktemerkers van cardiale myocyten moeilijker geweestte identificeren. SIRPα was een van de eerste cardiale oppervlakte merkers geïdentificeerd voor menselijke cardiale myocyten 18 en blijkt zeer selectief voor de cardiale lijn zijn. Onlangs hebben we gevonden dat dubbel met gescheiden SIRPα + en CD90 – cellen geeft nagenoeg zuiver cardiomyocyten met de CD90 + populatie vertoont een fibroblast-achtige fenotype (Josowitz, ongepubliceerde waarnemingen). Op basis van deze bevindingen verzameld, hierin beschrijven we creëren hECTs met een 3: 1 combinatie SIRPα + / CD90 – CD90 + cardiomyocyten en fibroblasten.
De mogelijkheid om een volledig gedefinieerd menselijk hartweefsel ingenieur is niet alleen essentieel voor het creëren van robuuste screening instrumenten, maar ook voor het ontwikkelen van modellen voor opkomende cel- en gen-gebaseerde cardiale therapieën te onderzoeken. Vooral veel celtherapie voor hartfalen, gebruik celtypen waaronder mesenchymale stamcellen (MSC) 21 </sup>, cardiale stamcellen 22 en beenmerg mononucleaire cellen 23-25, getest in klinische studies. Hoewel veel van de eerste resultaten zijn veelbelovend 21,23,25, de aanvankelijke voordeel vermindert vaak tijd 26-29. Een soortgelijke trend is gemeld bij muizen gemanipuleerde cardiale weefsels, die een belangrijke functionele voordelen te geven als gevolg van MSC suppletie, maar het voordeel is bij langdurige cultuur 1 niet volgehouden. Grondslag liggen aan de sub-optimale prestaties is onze beperkte kennis van de mechanismen die celtherapie. Een dieper begrip van hoe therapeutische cellen hun gunstige invloed, alsmede mogelijke negatieve gevolgen van myocyten-nonmyocyte interacties uit te oefenen, zou de ontwikkeling van verbeterde therapieën waardoor klinisch significante en blijvende voordelen, met minimale bijwerkingen voor patiënten met hartfalen mogelijk te maken.
Hier beschrijven we het gebruik van gedefinieerde hECTs te interrogat mechanismen van cel-gebaseerde therapie. De gecontroleerde weefsel samenstelling is van essentieel belang om specifieke factoren die van invloed zijn cardiomyocyt prestaties te identificeren. Direct vullen hECTs de therapeutische celtype van belang (bijvoorbeeld MSC), kunnen de effecten op de cardiale myocyten prestaties onthullen, zoals we in ratten ECT 1 aangetoond.
De volgende meerdere stappen protocol begint met gerichte cardiale stamcel differentiatie, gevolgd door de vervaardiging van de multi-weefsel bioreactor, en afgesloten met een beschrijving van weefsel constructie en functionele analyse. Onze experimenten worden uitgevoerd met behulp van de NIH-goedgekeurde H7 menselijke embryonale stamcellen (hESC) lijn. Echter, de volgende protocollen ook getest met behulp van een extra hESC lijn en drie geïnduceerde pluripotente stamcellen (iPSC) lijnen met vergelijkbare resultaten. Wij hebben gevonden dat de efficiëntie bij cardiomyocyt differentiatie en succes in Hect fabricage afhankelijke cellijn kan zijn, met name for iPSC lijnen afgeleid van individuele patiënten. Door dit protocol worden twee 6 putjes bedekt met in totaal 1.680.000 hESCs (140.000 cellen per putje), die ongeveer 2,5 miljoen myocyten na differentiatie gedurende 20 dagen en sorteren, genoeg om zes gedefinieerde tissues oplevert.
De bouw van gedefinieerde menselijke gemanipuleerde cardiale weefsels (hect) kan een meer consistente en betrouwbare model van menselijke hartspiercel functie te geven. Kritisch, alle cellulaire en extracellulaire componenten in het systeem bekend en kunnen als gewenst, waardoor de verstorende invloed van andere onbekende celtypen gevolg van het differentiatieproces verwijderd worden gemanipuleerd.
