Resting-state functional-connectivity MRI has identified abnormalities in patients with a wide range of neuropsychiatric disorders, including epilepsy due to malformations of cortical development. Transcranial Magnetic Stimulation in combination with EEG can demonstrate that patients with epilepsy have cortical hyperexcitability in regions with abnormal connectivity.
Resting-state functional connectivity MRI (rs-fcMRI) is a technique that identifies connectivity between different brain regions based on correlations over time in the blood-oxygenation level dependent signal. rs-fcMRI has been applied extensively to identify abnormalities in brain connectivity in different neurologic and psychiatric diseases. However, the relationship among rs-fcMRI connectivity abnormalities, brain electrophysiology and disease state is unknown, in part because the causal significance of alterations in functional connectivity in disease pathophysiology has not been established. Transcranial Magnetic Stimulation (TMS) is a technique that uses electromagnetic induction to noninvasively produce focal changes in cortical activity. When combined with electroencephalography (EEG), TMS can be used to assess the brain’s response to external perturbations. Here we provide a protocol for combining rs-fcMRI, TMS and EEG to assess the physiologic significance of alterations in functional connectivity in patients with neuropsychiatric disease. We provide representative results from a previously published study in which rs-fcMRI was used to identify regions with abnormal connectivity in patients with epilepsy due to a malformation of cortical development, periventricular nodular heterotopia (PNH). Stimulation in patients with epilepsy resulted in abnormal TMS-evoked EEG activity relative to stimulation of the same sites in matched healthy control patients, with an abnormal increase in the late component of the TMS-evoked potential, consistent with cortical hyperexcitability. This abnormality was specific to regions with abnormal resting-state functional connectivity. Electrical source analysis in a subject with previously recorded seizures demonstrated that the origin of the abnormal TMS-evoked activity co-localized with the seizure-onset zone, suggesting the presence of an epileptogenic circuit. These results demonstrate how rs-fcMRI, TMS and EEG can be utilized together to identify and understand the physiological significance of abnormal brain connectivity in human diseases.
Transkraniel magnetisk stimulation (TMS) er et middel til ikke-invasivt at stimulere regioner af cortex via elektromagnetisk induktion. I TMS, er et stort, men rumligt begrænset magnetisk flux anvendes til at inducere et elektrisk felt i et mål kortikal område, og derved modulere aktiviteten af det underliggende nervevæv. Disse resultater tyder på, at TMS foranstaltninger af kortikal ophidselse kan tillade os at identificere endophenotypes for epilepsi. følsomheden og specificiteten af disse foranstaltninger er imidlertid begrænset, sandsynligvis fordi TMS-EMG kun kan vurderes med stimulation af motoriske kortikale kredsløb, og mange patienter med epilepsi har anfald foci udenfor den motoriske hjernebark.
Elektroencefalografi (EEG) giver mulighed for direkte at måle cerebral reaktion på TMS, og kan anvendes til at vurdere cerebral reaktivitet tværs store områder af neocortex. Undersøgelser integrerer TMS med EEG (TMS-EEG) har vist, at TMS producerer bølger af aktivitet, genlyd i hele cortex 9,10, og som er reproducerbar og pålidelig 11-13. Ved at evaluere udbredelsen af fremkaldte aktiviteti forskellige adfærdsmæssige tilstande og i forskellige opgaver, har TMS-EEG blevet brugt til kausalt sonde den dynamiske effektive konnektivitet af menneskelige hjerne netværk 10,14-16. TMS-EEG foranstaltninger har vist signifikante abnormiteter i sygdomme, der spænder fra skizofreni 17 til ADHD 18, og i forstyrrelser af bevidsthed, såsom vedvarende vegetativ tilstand 19. Desuden har flere grupper identificeret EEG korrelater til den parrede-puls TMS-EMG målinger, der er unormal hos patienter med epilepsi 20,21. Af særlig relevans har tidligere undersøgelser også foreslået, at unormal stimulering fremkaldte EEG aktivitet ses hos patienter med epilepsi 22-25.
Et andet middel til vurdering hjernens kredsløb er via hvilende-state funktionel tilslutning MRI (RS-fcMRI), en teknik, der evaluerer korrelationerne over tid i blodets iltning niveau afhængig (fed) signal fra forskellige hjerneområder 26. undersøgelser under anvendelse afrs-fcMRI har vist, at den menneskelige hjerne er organiseret i forskellige netværk af interagerende regioner 26-29, at neuropsykiatriske sygdomme kan opstå inden for specifikke store distribuerede neurale netværk identificeret ved rs-fcMRI 30, og at hjernen netværk identificeret via RS fcMRI er ofte unormal i neuropsykiatriske sygdomstilstande 31,32. Med hensyn til potentielle kliniske anvendelser, rs-fcMRI har flere fordele i forhold til konventionel opgavebaseret fMRI program 33, herunder mindre afhængighed af emnet samarbejde og bekymring over variabel ydelse. Derfor har der for nylig været en eksplosion af undersøgelser, RS-fcMRI ændringer i forskellige sygdomstilstande. Men en af begrænsningerne ved RS-fcMRI er problemer med at bestemme, om og hvordan korrelationer (eller anticorrelations) i BOLD signal vedrører de elektrofysiologiske interaktioner, der danner grundlag for neuronal kommunikation. Et beslægtet problem er, at det er oftenn uklart, om de rs-fcMRI ændringer set i forskellige sygdomstilstande har fysiologisk betydning. Især med hensyn til epilepsi, er det uklart, om abnormaliteter i RS-fcMRI skyldes udelukkende interictal epilepsilignende transienter, eller eksisterer uafhængigt af sådanne elektrofysiologiske abnormiteter; samtidige EEG-fMRI er nødvendig for at hjælpe evaluere mellem disse muligheder 34.
Som TMS kan anvendes til fremstilling af forbigående eller vedvarende ændringer i aktiveringer af forskellige kortikale områder, TMS undersøgelser tilvejebringer et middel til kausalt at vurdere betydningen af forskellige hvilende-state fMRI tilslutningsmuligheder mønstre. En metode er at bruge RS-fcMRI at guide terapeutiske stimulation indsats på forskellige sygdomstilstande; Det kunne forventes, at TMS målrettet til regioner, der er funktionelt er forbundet til områder, der vides at være involveret i forskellige sygdomstilstande er mere tilbøjelige til at være terapeutisk effektiv end TMS målrettet til regioner uden en sådan functional tilslutningsmuligheder, og faktisk flere undersøgelser har fundet foreløbige beviser for denne 35,36. En anden fremgangsmåde ville involvere anvendelse af TMS-EEG til kausalt vurdere den fysiologiske betydning af forskellige hvilende-state fcMRI mønstre. Konkret kan man teste hypotesen om, at regioner, der viser unormal funktionel konnektivitet i en bestemt sygdomstilstand skal vise en anden respons på stimulation hos patienter end hos raske forsøgspersoner, og at disse fysiologiske abnormiteter er til stede specifikt (eller primært) med stimulering af unormalt forbundet region.
For at illustrere ovenstående, giver vi et eksempel på en nylig undersøgelse, hvor rs-fcMRI, TMS og EEG blev kombineret til at udforske kortikale hyperexcitabilitet hos patienter med epilepsi på grund af den udviklingsmæssige hjerne abnormitet periventrikulær nodulær heterotopia (PNH) 37. Patienter med PNH nuværende klinisk med adolescent- eller voksen-debut epilepsi, læsning handicap, og normal intelligence, og har unormale knuder af grå stof støder op til de laterale ventrikler på Neuroimaging 38,39. Tidligere undersøgelser har vist, at disse periventrikulære knuder af heterotop grå substans er strukturelt og funktionelt forbundet til diskrete foci i neocortex 40,41, og at epileptiske anfald kan stamme fra neocorticale regioner, heterotopisk grå substans eller begge samtidigt 42, hvilket antyder, at epileptogenese i disse patienter er et kredsløb fænomen. Ved at bruge hvile-state fc-MRI til at guide TMS-EEG, viste vi, at patienter med aktiv epilepsi på grund af PNH har beviser for kortikal hyperexcitabilitet, og at denne hyperexcitabilitet synes at være begrænset til regioner med unormal funktionel forbindelse til de dybe knuder.
Protokollen udføres i to separate sessioner. Under den første session, er strukturelt og hvile-state blod-iltning niveau afhængige (BOLD) kontrast MRI-sekvenser erhvervet(Til patienter), eller blot strukturelle MRI sekvenser (for de raske kontroller). Mellem den første og anden sessioner, anvendes hvilende-state funktionel konnektivitet analyse for at fastlægge de kortikale mål for patienterne, og MNI koordinater for opnås disse mål. De tilsvarende kortikale mål (baseret på MNI koordinater) derefter fastlægges for hver raske kontrol emne. I den anden session, opnås TMS-EEG-data.
I eksemplet i dette papir blev funktionel-tilslutningsmuligheder MRI analyser udføres ved hjælp af en in-house software værktøjskasse og MRI-softwaren 43,44. Neuro-navigeret TMS blev udført med en transkraniel magnetisk stimulator med real-time MRI neuronavigation. EEG blev registreret med en 60-kanals TMS-kompatibelt system, som anvender en prøve-og-hold kredsløb for at undgå forstærker mætning af TMS. EEG-data blev analyseret ved hjælp af brugerdefinerede scripts og EEGLAB værktøjskasse 45 (version 12.0.2.4b) kører i MATLAB R2012b.
