Resting-state functional-connectivity MRI has identified abnormalities in patients with a wide range of neuropsychiatric disorders, including epilepsy due to malformations of cortical development. Transcranial Magnetic Stimulation in combination with EEG can demonstrate that patients with epilepsy have cortical hyperexcitability in regions with abnormal connectivity.
Resting-state functional connectivity MRI (rs-fcMRI) is a technique that identifies connectivity between different brain regions based on correlations over time in the blood-oxygenation level dependent signal. rs-fcMRI has been applied extensively to identify abnormalities in brain connectivity in different neurologic and psychiatric diseases. However, the relationship among rs-fcMRI connectivity abnormalities, brain electrophysiology and disease state is unknown, in part because the causal significance of alterations in functional connectivity in disease pathophysiology has not been established. Transcranial Magnetic Stimulation (TMS) is a technique that uses electromagnetic induction to noninvasively produce focal changes in cortical activity. When combined with electroencephalography (EEG), TMS can be used to assess the brain’s response to external perturbations. Here we provide a protocol for combining rs-fcMRI, TMS and EEG to assess the physiologic significance of alterations in functional connectivity in patients with neuropsychiatric disease. We provide representative results from a previously published study in which rs-fcMRI was used to identify regions with abnormal connectivity in patients with epilepsy due to a malformation of cortical development, periventricular nodular heterotopia (PNH). Stimulation in patients with epilepsy resulted in abnormal TMS-evoked EEG activity relative to stimulation of the same sites in matched healthy control patients, with an abnormal increase in the late component of the TMS-evoked potential, consistent with cortical hyperexcitability. This abnormality was specific to regions with abnormal resting-state functional connectivity. Electrical source analysis in a subject with previously recorded seizures demonstrated that the origin of the abnormal TMS-evoked activity co-localized with the seizure-onset zone, suggesting the presence of an epileptogenic circuit. These results demonstrate how rs-fcMRI, TMS and EEG can be utilized together to identify and understand the physiological significance of abnormal brain connectivity in human diseases.
La estimulación magnética transcraneal (TMS) es un medio para estimular de forma no invasiva regiones de la corteza a través de la inducción electromagnética. En TMS, un gran pero espacialmente restringida de flujo magnético se utiliza para inducir un campo eléctrico en una zona cortical de destino, y por lo tanto modular la actividad del tejido neural subyacente. Otro medio para evaluar los circuitos del cerebro es a través de la conectividad funcional en estado de reposo MRI (rs-fcMRI), una técnica que evalúa las correlaciones con el tiempo en el nivel de oxigenación de la sangre señal dependiente (BOLD) de diferentes regiones del cerebro 26. Para ilustrar lo anterior, se proporciona un ejemplo de un estudio reciente en el que se combinaron RS-fcMRI, TMS y EEG para explorar la hiperexcitabilidad cortical en pacientes con epilepsia debido al desarrollo anormalidad cerebral heterotopía nodular periventricular (PNH) 37. Los pacientes con HPN se presentan clínicamente con epilepsia necesidades de los adolescentes o adulto-inicio, la dificultad para leer, y lo normal intelligence, y tiene nódulos anormales de la sustancia gris adyacente a los ventrículos laterales en las neuroimágenes 38,39. Estudios anteriores han demostrado que estos nódulos periventriculares de la materia gris heterotópica son estructural y funcionalmente conectados a focos discretos en el neocórtex 40,41, y que las crisis epilépticas pueden proceder de regiones neocorticales, materia gris heterotópica, o ambos a la vez 42, lo que sugiere que la epileptogénesis en estos pacientes es un fenómeno circuito. Al utilizar en estado de reposo fc-RM para guiar TMS-EEG, hemos demostrado que los pacientes con epilepsia activa debido a PNH tienen evidencia de la hiperexcitabilidad cortical, y que esta hiperexcitabilidad parece estar limitada a regiones con conectividad funcional anormal de los nódulos profundos.
El protocolo se lleva a cabo en dos sesiones separadas. Durante la primera sesión, estructural y en estado de reposo sangre oxigenación nivel-dependientes (BOLD) secuencias de contraste de MRI se adquieren(Para pacientes), o sólo las secuencias de resonancia magnética estructural (para los controles sanos). Entre la primera y la segunda sesión, el análisis de la conectividad funcional en estado de reposo se utiliza para definir los objetivos corticales de los pacientes, y coordina el MNI de estos objetivos se obtienen. Los objetivos corticales equivalentes (en base a las coordenadas MNI) son identificadas para cada sujeto de control sano. En la segunda sesión, se obtienen los datos TMS-EEG.
En el ejemplo dado en el presente documento, los análisis de resonancia magnética funcional-conectividad se realizaron utilizando una caja de herramientas de software propio y el software RM 43,44. Neuro-navegado TMS se realizó con un estimulador magnético transcraneal con neuronavegación MRI en tiempo real. EEG se registró con un sistema compatible con TMS 60 canales, que utiliza un circuito de muestreo y retención para evitar la saturación del amplificador de TMS. Los datos del EEG se analizaron mediante scripts personalizados y la caja de herramientas EEGLAB 45 (versión 12.0.2.4b) que se ejecutan en MATLAB R2012b.
En estado de reposo MRI conectividad funcional se ha utilizado para identificar la conectividad de red en el cerebro humano, y para identificar alteraciones de conectividad que se producen en diferentes estados de enfermedad 26,31,32.
