Islet β cell death precedes development of type 1 diabetes, and detecting this process may allow for early therapeutic intervention. Here, we provide a detailed description of how to measure differentially methylated INS DNA species in human serum as a biomarker of β cell death.
The death of islet β cells is thought to underlie the pathogenesis of virtually all forms of diabetes and to precede the development of frank hyperglycemia, especially in type 1 diabetes. The development of sensitive and reliable biomarkers of β cell death may allow for early therapeutic intervention to prevent or delay the development of diabetes. Recently, several groups including our own have reported that cell-free, differentially methylated DNA encoding preproinsulin (INS) in the circulation is correlated to β cell death in pre-type 1 diabetes and new-onset type 1 diabetes. Here, we present a step-by-step protocol using digital PCR for the measurement of cell-free INS DNA that is differentially methylated at cytosine at position -69 bp (relative to the transcriptional start site). We demonstrate that the assay can distinguish between methylated and unmethylated cytosine at position -69 bp, is linear across several orders of magnitude, provides absolute quantitation of DNA copy numbers, and can be applied to samples of human serum from individuals with new-onset type 1 diabetes and disease-free controls. The protocol described here can be adapted to any DNA species for which detection of differentially methylated cytosines is desired, whether from circulation or from isolated cells and tissues, and can provide absolute quantitation of DNA fragments.
Typ 1 diabetes (T1D) är en autoimmun sjukdom som kännetecknas av destruktion av insulinproducerande ö-β-celler genom autoreaktiva T-celler 1. Diagnosen T1D typiskt görs vid mätning av hyperglykemi (blodglukos> 200 mg / dl) i en mager, ung individ, som kan uppvisa ketoacidos som bevis på insulinbrist. Vid tidpunkten för diagnos av T1D, finns det bevis för väsentlig förlust av β-cellfunktion och massa (50-90%) 2. I linje med dessa ansträngningar, har vår grupp och andra nyutvecklade biomarkörer analyser som mäter cirkulerande, epigenetiskt modifierade DNA-fragment som uppstår i första hand från att dö p-celler 6-9. I samtliga av de publicerade analyserna hittills har fokus varit på kvantifiering av den humana genen som kodar preproinsulin (INS), som visar större grad av ometylerade CpG platser i de kodande och promotorregioner jämfört med andra celltyper. Befrielsen av ometylerade INS DNA-fragment var hypotes som uppstår i första hand från att dö (nekrotiska, apoptotiska) p-celler. Våra senare studier har visat att i ungdomen, höjder både ometylerade och metylerade INS DNA vid position -69 bp (i förhållande till than transkriptions start site) observerades i nydebuterat T1D, och tillsammans tjänade som specifika biomarkörer för denna population 6. Dessa biomarkörer analyser innefattar isolering av cellfria DNA från serum eller plasma med hjälp av kommersiella spin kit, följt av en bisulfit omvandling av den isolerade DNA (för att konvertera icke-metylerade cytosiner till uraciler, lämnar metylerade cytosiner intakt).

Metylering av cytosiner genom DNA metyltransferaser möjliggör epigenetisk reglering av transkription på många gener. INS-genen hos människa är nästan uteslutande uttrycks i ö-β-celler, och det verkar finnas ett samband mellan frekvensen av metylering av cytosiner i INS-genen till att tysta av dess transkription 11.
