Dit artikel demonstreert het gebruik van laser doppler flowmetrie om te evalueren van het vermogen van de cerebrale circulatie te autoreguleren de bloedtoevoer tijdens verlagingen van de arteriële bloeddruk.
Bij het onderzoeken van de mechanismen van het lichaam voor het reguleren van cerebrale doorbloeding, een relatieve meting van de micro circulatoire bloedstroom kan worden verkregen met behulp van laser doppler flowmetrie (LDF). Deze paper toont een gesloten schedel preparaat dat het mogelijk maakt cerebrale bloedtoevoer te beoordelen zonder de schedel door te dringen of een kamer of cerebrale venster te installeren. Om autoregulatory mechanismen te evalueren, kan een model van gecontroleerde bloeddruk reductie via gegradeerde bloeding worden gebruikt, terwijl tegelijkertijd gebruik wordt maken van LDF. Dit maakt de real time tracking van de relatieve veranderingen in de bloedstroom in reactie op verlagingen van de arteriële bloeddruk geproduceerd door de terugtrekking van Circulerend bloed volume. Dit paradigma is een waardevolle benadering voor het bestuderen van cerebrale bloedstroom autoregulatie tijdens verlagingen van de arteriële bloeddruk en, met kleine wijzigingen in het Protocol, is ook waardevol als een experimenteel model van hemorragische shock. Naast het evalueren van autoregulatoire responsen, kan LDF worden gebruikt om de corticale bloedstroom te monitoren bij het onderzoeken van metabole, myogene, endotheel-, humorele of neurale mechanismen die de cerebrale doorbloeding reguleren en de impact van verschillende experimentele interventies en pathologische condities op cerebrale doorbloeding.
Autoregulatory mechanismen in de cerebrale circulatie spelen een cruciale rol bij het handhaven van de homeostase en de normale functie in de hersenen. Autoregulatie van de cerebrale doorbloeding wordt beïnvloed door meerdere factoren, waaronder hartslag, bloed snelheid, perfusiedruk, de diameter van de cerebrale resistentie slagaders en de microcirculatoire weerstand, die allemaal een rol spelen bij het handhaven van de totale cerebrale bloedstroom constante in de hersenen over het fysiologische bereik van systemische bloeddruk. Wanneer de arteriële druk toeneemt, deze mechanismen constrikte arteriolen en weerstand aders om te voorkomen dat gevaarlijke stijgingen van de intracraniële druk. Wanneer arteriële bloeddruk afneemt, lokale controlemechanismen verwijden de arteriolen te handhaven weefsel perfusie en O2 levering. Verschillende pathologische aandoeningen zoals hypercapnia, traumatisch of globaal hypoxisch hersenletsel, en diabetische microangiopathie1,2,3,4,5,6 kunnen het vermogen van de hersenen om de bloedstroom te autoregeren verstoren. Chronische hypertensie verschuift bijvoorbeeld het effectieve autoregulatoire bereik naar hogere druk7,8,9, en een hoog zout (HS) dieet interfereert niet alleen met normale endotheel-afhankelijke verwijding in de cerebrale microcirculatie10, maar ook het vermogen van autoregulatory mechanismen in de cerebrale circulatie te verwijden en handhaven weefsel perfusie wanneer de arteriële druk wordt verlaagd11. Cerebrale autoregulatie is ook aangetast bij Dahl-zoutgevoelige ratten wanneer ze een HS-dieet12hebben gevoed.
Tijdens verlagingen van de arteriële druk, verwijding van de cerebrale resistentie slagaders en arteriolen in eerste instantie retourneert cerebrale doorbloeding om waarden te beheersen ondanks de verminderde perfusiedruk. Naarmate de arteriële druk verder wordt verlaagd, blijft de cerebrale bloedstroom constant bij de lagere druk (plateaufase van de autoregulatoire respons) totdat de vasculatuur niet langer kan verwijden om de bloedstroom bij de lagere druk te handhaven. De laagste druk waarbij een orgaan de normale bloedtoevoer kan aanhouden, wordt de ondergrens van de autoregulatie (LLA) genoemd. Bij druk onder de LLA daalt de cerebrale bloedstroom aanzienlijk van de rust waarden en neemt deze af op een lineaire manier met elke reductie in arteriële perfusiedruk13,14. Een opwaartse verschuiving in de LLA, zoals waargenomen in hypertensie7,8,9, kan het risico en de ernst van ischemische letsel verhogen tijdens omstandigheden waar de arteriële perfusiedruk wordt verminderd (bijv. myocardinfarct, ischemische beroerte, of bloedsomloop shock).
Ldf heeft bewezen een uiterst waardevolle benadering te zijn om de bloedtoevoer in de microcirculatie onder verschillende omstandigheden te evalueren, waaronder autoregulatie van de bloedstroom in de cerebrale circulatie11,14,15. Naast het evalueren van autoregulatoire reacties, kan ldf worden gebruikt om de corticale bloedstroom te bewaken bij het onderzoeken van metabole, myogene, endotheel-, humorele of neurale mechanismen die de cerebrale doorbloeding reguleren en de impact van verschillende experimentele interventies en pathologische aandoeningen op de cerebrale bloedstroom10,16,17,18,19,20,21
LDF meet de verschuiving in gereflecteerde laserlicht in reactie op het aantal en de snelheid van bewegende deeltjes–in dit geval, rode bloedcellen (RBC). Voor studies van cerebrale vasculaire autoregulatie, arteriële bloeddruk wordt veranderd hetzij door de infusie van een alpha-adrenergic agonist te verhogen arteriële druk (omdat de cerebrale circulatie zelf is ongevoelig voor alpha-adrenergic vasoconstrictor agonisten)12,15 of via gecontroleerde bloed volume terugtrekking te verminderen van arteriële druk11,14. In de huidige studie wordt LDF gebruikt om de effecten van beoordeelde verlagingen van de bloeddruk op cerebrale autoregulatie in een gezonde rat aan te tonen. Hoewel open en gesloten schedel methoden zijn beschreven in de literatuur22,23,24,25, het huidige papier toont een gesloten schedel voorbereiding, waardoor cerebrale bloedtoevoer kan worden beoordeeld zonder het binnendringen van de schedel of het installeren van een kamer of cerebrale venster.
Evaluatie van weefsel bloedstroom reacties met laser doppler flowmetrie (LDF). Zoals hierboven vermeld, is het LDF-signaal evenredig aan het aantal en de snelheid van bewegende deeltjes, in dit geval RBC, in de microcirculatie.
