Esophagus

The esophagus is a muscular conduit approximately 25 cm long that helps transport food from the mouth to the stomach.
It courses through the mediastinum and penetrates the diaphragm through the esophageal hiatus.
The lumen of the esophagus is not circular in its normal state because it is typically compressed and only expands during swallowing.
The esophagus connects to the stomach at the cardial orifice, which is surrounded by the gastroesophageal or cardiac sphincter.
This minor thickening of circular smooth muscle primarily seals the stomach, opening only during swallowing.
Mucosal cells on either side of the sphincter protect the esophagus from gastric acid backflow.
The wall of the esophagus comprises four foundational layers of the alimentary canal. The esophageal mucosa is made up of nonkeratinized stratified squamous epithelium.
The submucosa contains esophageal glands that produce mucus to lubricate the esophagus, facilitating food passage.
The muscularis externa consists of a mix of skeletal and smooth muscle.
Finally, the fibrous adventitia, composed entirely of connective tissue, replaces the serosa layer.

The esophagus, a muscular conduit linking the pharynx and stomach, measures roughly 10 inches (25.4 cm) and sits behind the trachea. It remains collapsed when not swallowing. The esophagus follows a predominantly straight path through the thoracic mediastinum and enters the abdominal cavity through a diaphragmatic opening known as the esophageal hiatus.
The movement of edibles from the pharynx into the esophagus is facilitated by the upper esophageal sphincter, which is formed primarily by the cricopharyngeus muscle. The esophagus&#39;s proximal third is composed mainly of skeletal muscle, and the middle third is a mixture of smooth and skeletal muscle tissue; however, the skeletal muscles gradually disappear in the esophagus&#39;s final third. In the esophagus, peristaltic rhythmic contractions help propel the food bolus toward the stomach. Concurrently, the esophageal mucosa excretes substances that lubricate the esophagus and the food. The lower esophageal sphincter (also termed the gastroesophageal or cardiac sphincter) is the passage from the esophagus into the stomach. Sphincters function as valves, opening, and closing to regulate the flow through tubes. The lower esophageal sphincter relaxes, lets food go into the stomach, and then contracts to prevent the stomach&#39;s contents from refluxing into the esophagus. The diaphragm muscle encircles this sphincter, assisting in sealing off the sphincter when swallowing is not occurring. Incomplete closure of the lower esophageal sphincter can result in reflux of stomach contents into the esophagus, causing conditions like heartburn or gastroesophageal reflux disease (GERD).
The esophagus&#39;s mucosal layer consists of an epithelial lining with stratified squamous epithelium, which is not keratinized, and a stratum of basal and parabasal cells. This type of epithelium offers protection against food particle-induced wear and tear. The lamina propria of the mucosa houses glands that secrete mucus. The composition of the muscularis layer varies with its anatomical location: the upper third comprises skeletal muscle, the middle third is a mix of skeletal and smooth muscles, and the lower third consists only of smooth muscles. Contrary to the stomach and intestines, the outermost layer of the esophagus, known as the adventitia instead of the serosa, is not enveloped by a visceral peritoneum fold. This layer is mostly comprised of loose connective tissue.
The digestive functions of the esophagus include allowing the food bolus to pass from the laryngopharynx to the esophagus, peristalsis for propelling the bolus through the esophagus, relaxation of the lower esophageal sphincter to enable the bolus to move from the esophagus to the stomach and to prevent chyme from entering the esophagus, and mucus secretion for lubricating the esophagus to aid smooth passage of the bolus.

The esophagus is a muscular tube that moves food from the pharynx to the stomach. It is about 10 inches, or 25.4 cm, long and lies behind the trachea. When a person is not swallowing, the tube stays collapsed. It also runs mostly straight through the thoracic mediastinum and passes into the abdominal cavity through the esophageal hiatus in the diaphragm.

Food enters the esophagus through the upper esophageal sphincter, which is formed mainly by the cricopharyngeus muscle. The wall of the esophagus changes along its length. The proximal third is mostly skeletal muscle, the middle third has both smooth and skeletal muscle, and the skeletal muscle disappears in the final third. These muscle layers help create peristalsis, which is the rhythmic squeezing that pushes the food bolus toward the stomach.

The esophageal lining also helps food move smoothly. Its mucosa releases mucus that lubricates both the tube and the food. The lining is made of stratified squamous epithelium that is not keratinized, along with basal and parabasal cells. This structure protects the esophagus from wear and tear caused by food particles. Glands in the lamina propria also secrete mucus.

At the lower end, the lower esophageal sphincter, also called the gastroesophageal or cardiac sphincter, controls the opening into the stomach. A sphincter acts like a valve that opens and closes to regulate flow through a tube. This sphincter relaxes to let food enter the stomach and then contracts to keep stomach contents from flowing back into the esophagus. The diaphragm surrounds this area and helps seal it when swallowing is not happening.

If the lower esophageal sphincter does not close completely, stomach contents can reflux into the esophagus. This can cause heartburn or gastroesophageal reflux disease, often called GERD. The outer layer of the esophagus is called the adventitia, not the serosa. It is mostly loose connective tissue and is not covered by a visceral peritoneum fold.

Content elaboration: The digestive system consists of the gastrointestinal tract (alimentary canal) as well as various accessory organs including the teeth, tongue, salivary glands, liver, gallbladder and pancreas. The digestive system processes and supplies the molecules needed to sustain the living tissues within the body through the absorption of nutrients. Six major functions of the digestive system include secretion, ingestion, mechanical processing, enzymatic digestion, absorption and excretion. The lining of the digestive system protects surrounding tissues from the mechanical and enzymatic stresses of the digestive process. Processes of the digestive system include the mechanical and chemical breakdown of food into small molecules which are then absorbed by the digestive tract. Specific actions within the digestive system include mastication, peristalsis, segmentation and the release of hormones and enzymes necessary for digestion. The metabolic functions of the accessory organs play strategic roles in the breakdown of food products, the maintenance of glucose levels within the blood and the regulation of homeostasis in the body. Indigestible material is excreted as waste. Homeostatic imbalances are explored. These include, but are not limited to, conditions such as gallstones, heartburn, ulcers, dehydration, diarrhea, cirrhosis and cancers of the digestive system. Investigations are used to understand and explain the digestive system in a variety of inquiry and design scenarios that can incorporate evolutionary concepts, scientific reasoning, comparative analysis, communication skills and real-world applications.
