Betrouwbare en nauwkeurige uitkomst evaluatie is de sleutel voor de vertaling van preklinische therapieën in de klinische behandeling. De huidige paper beschrijft hoe drie klinisch relevante primaire uitkomstmaten van cardiale prestaties en de schade op te nemen in een varken acuut hartinfarct model.
Mortality after acute myocardial infarction remains substantial and is associated with significant morbidity, like heart failure. Novel therapeutics are therefore required to confine cardiac damage, promote survival and reduce the disease burden of heart failure. Large animal experiments are an essential part in the translational process from experimental to clinical therapies. To optimize clinical translation, robust and representative outcome measures are mandatory. The present manuscript aims to address this need by describing the assessment of three clinically relevant outcome modalities in a pig acute myocardial infarction (AMI) model: infarct size in relation to area at risk (IS/AAR) staining, 3-dimensional transesophageal echocardiography (TEE) and admittance-based pressure-volume (PV) loops. Infarct size is the main determinant driving the transition from AMI to heart failure and can be quantified by IS/AAR staining. Echocardiography is a reliable and robust tool in the assessment of global and regional cardiac function in clinical cardiology. Here, a method for three-dimensional transesophageal echocardiography (3D-TEE) in pigs is provided. Extensive insight into cardiac performance can be obtained by admittance-based pressure-volume (PV) loops, including intrinsic parameters of myocardial function that are pre- and afterload independent. Combined with a clinically feasible experimental study protocol, these outcome measures provide researchers with essential information to determine whether novel therapeutic strategies could yield promising targets for future testing in clinical studies.
Hartfalen met verminderde ejectiefractie (HFrEF) is goed voor ongeveer 50% van alle gevallen van hartfalen, die naar schatting 1-2% van de mensen in de westerse wereld 1. De meest voorkomende oorzaak is acuut myocardinfarct (AMI). Als acute mortaliteit na AMI aanzienlijk is gedaald door de toegenomen bewustwording en een betere behandeling opties, is de nadruk verschoven naar haar chronische gevolgen; de meest prominente wezen HFrEF 2,3. Samen met de toenemende kosten van de gezondheidszorg 4, de groeiende epidemie van hartfalen benadrukt de noodzaak voor nieuwe diagnostica en therapieën, die in een sterk translationeel varkens model van negatieve remodeling kan worden bestudeerd nadat AMI zoals eerder beschreven 5.
Zowel determinanten (bijvoorbeeld infarct grootte) en functionele evaluaties (bijvoorbeeld echocardiografie) van bijwerkingen remodeling worden vaak gebruikt voor de werkzaamheid testen van nieuwe therapieën, met vermelding van de noodzaak van een reliable en relatief goedkope methodes. Het doel van dit artikel is om deze behoefte door het introduceren belangrijke en betrouwbare meetinstrumenten op werkzaamheid testen in een varkensmodel van acuut myocardiaal infarct. Deze omvatten infarctgrootte (IS) met betrekking tot risicogebied (AAR), 3D transesophagale echocardiografie (TEE-3D) en gedetailleerde admittantie gebaseerde druk-volume (PV) loop acquisitie.
Infarctgrootte is de belangrijkste determinant van ongunstige remodeling en overleving na AMI 6. Hoewel tijdige reperfusie van ischemisch myocard kan omkeerbaar gewonden hartspiercellen te redden en te beperken grootte van het infarct, reperfusie zelf veroorzaakt extra schade door het genereren van oxidatieve stress en een onevenredige ontstekingsreactie (ischemie-reperfusie schade (IRI)) 7. Derhalve is IRI geïdentificeerd als een veelbelovend therapeutisch doelwit. Het vermogen van nieuwe therapieën voor de infarctgrootte te verminderen wordt gekwantificeerd door het bepalen van infarctgrootte in relatiehet risicogebied (AAR). AAR kwantificering is verplicht om te corrigeren voor de inter-individuele variabiliteit in de coronaire anatomie van diermodellen, zoals een grotere AAR leidt tot een grotere absolute grootte van het infarct. Sinds infarctgrootte is direct gerelateerd aan de prestaties van hart en myocardcontractiliteit, kunnen variaties in AAR beïnvloeden bestuderen uitkomstmaten ongeacht behandelingsmethoden 8.
Driedimensionale transesophageal echocardiografie (3D-TEE) is een veilige, betrouwbare en, belangrijker nog, klinisch toepasbare goedkope methode voor het meten van de hartfunctie niet-invasief. Overwegende transthoracale echocardiografie (TTE) beelden worden beperkt tot 2D parasternale lange en korte as uitzicht bij varkens 9, kan 3D-TEE worden gebruikt voor het volledige 3-dimensionale beelden van de linker hartkamer te verkrijgen. Daarom is het niet wiskundige benaderingen van de linker ventrikel (LV) volumes, zoals de regel van de gewijzigde Simpson 10 vereisen. De laatste lager dan dat van correctly schatten LV volumes na LV remodeling gevolg van het ontbreken van cilindrische geometrie 11. Bovendien 3D-TEE voorkeur boven epicardiale echocardiografie omdat het geen chirurgische ingrepen, die zijn waargenomen cardioprotectieve effecten in dit model 12 uitoefenen vereisen. Hoewel het gebruik van 2D-TEE voor de beoordeling van myocardiale functie eerder beschreven 13,14, beperkingen ten aanzien ventriculaire geometrie vergelijkbaar met die waargenomen in 2D-TTE en afhankelijk van de mate van LV remodeling. Dus hoe groter de infarct (en dus hoe groter de kans op hartfalen), hoe waarschijnlijker 2D metingen worden ontsierd door onjuiste geometrische aannames en des te groter de 3D technieken.
Desalniettemin zijn de meeste beeldvormende modaliteiten beperkt in hun vermogen om de intrinsieke functionele eigenschappen van het myocard te beoordelen. PV lussen, deze relevante aanvullende informatie en de verwerving ervan is daaromhieronder in detail beschreven.
Remodellering is grotendeels afhankelijk van myocardiale infarctgrootte en de kwaliteit van myocardiaal infarct repareren 6,26. Aan de voormalige op een gestandaardiseerde manier te beoordelen, de huidige manuscript biedt een elegante methode van in vivo infusie van Evans blauw gecombineerd met ex vivo TTC vlekken, die is gevalideerd en veelvuldig gebruikt 8,16,27,28.
