In this manuscript, experimental techniques, including blood preparation, confocal microscopy, and lysis rate analysis, to examine the morphological differences between normal and abnormal clot structures due to diseased states are presented.
Fibrin is an extracellular matrix protein that is responsible for maintaining the structural integrity of blood clots. Much research has been done on fibrin in the past years to include the investigation of synthesis, structure-function, and lysis of clots. However, there is still much unknown about the morphological and structural features of clots that ensue from patients with disease. In this research study, experimental techniques are presented that allow for the examination of morphological differences of abnormal clot structures due to diseased states such as diabetes and sickle cell anemia. Our study focuses on the preparation and evaluation of fibrin clots in order to assess morphological differences using various experimental assays and confocal microscopy. In addition, a method is also described that allows for continuous, real-time calculation of lysis rates in fibrin clots. The techniques described herein are important for researchers and clinicians seeking to elucidate comorbid thrombotic pathologies such as myocardial infarctions, ischemic heart disease, and strokes in patients with diabetes or sickle cell disease.

Över 2 miljoner människor är nyligen diagnostiserats med diabetes mellitus varje år i USA. Det finns två typer av diabetes: typ I, där denorgan underlåter att producera tillräckliga mängder insulin, och typ II, där kroppen blir resistent mot insulin. Bland patienter med diabetes, är hjärt-kärlsjukdom (CVD) orsaken till 80% av sjuklighet och dödlighet i samband med sjukdomen 20,21.
Sicklecellanemi (SCD) är en genetisk blodsjukdom som drabbar mer än 100.000 människor i USA 22. SCD är en punkt-mutation sjukdom som orsakar röda blodkroppar för att bli halvmånformad, vilket gör det svårt för cellerna att passera genom blodvaskulaturen 23. Båda dessa sjukdomstillstånd ökar chanserna att utveckla aterotrombotiska förhållanden i kroppen. En av anledningarna till detta är ett resultat av förändrad fibrin struktur och funktion i sjukdomstillstånd 14,24-26.
I både diabetes och sicklecellanemi, det finns hyperkoagulation och hypofibrinolysis aktivitet som inducerar aterotrombos och hjärt-kärlsjukdom (CVD) jämfört med friska patienter 17,27,28. Det är känt att hypofibrinolysis främjar ateroskleros progression och alstrar återkommande ischemiska händelser för patienter med prematur kranskärlssjukdom 29. I den aktuella manuskriptet, undersökte vi den roll som fibrin fysikaliska egenskaper i denna särskilda miljö. Fibrinkoagel strukturer i icke-sjuka patienter består av tunna fibrer, större porer och generellt mindre tät 14,24. Den ökade porositet och mindre täta fibrinkoagel hos friska patienter har visat sig underlätta fibrinolys 16. I hyperthrombotic förhållanden såsom diabetiker och sicklecellanemi, det finns en ökning av fibrinogen produktion, vilket gör att fibrinogenkoncentration öka från normala nivåer av 2,5 mg / ml hos friska patienter 30-33. Fibrinkoagel bildas i diabetiska patienter har befunnits vara mindre porös, styvare, har fler förgreningspunkter, och tätare i jämförelse med friska, icke-diaBetic patienter 14,24,33-35. Den förändrade fibrin struktur är ett resultat av glykation mekanismer som förekommer i de proteiner som är involverade i koagelbildning. Icke-enzymatisk (irreversibel) glykation uppstår när glukosmolekyler binder till lysinrester på fibrinogenmolekylen, vilket hämmar mänskliga faktorn Xllla (FXIIIa) från rätt tvärbindnings glutamin och lysinrester 33,36,37.
Den strukturella analysen av fibrin-nät, har studerats i stor omfattning på senare tid. I synnerhet har forskarna utnyttjat elektronmikroskopi och 3D-rekonstruktion av fibrin nätverk 38, undersöktes hur både intravaskulära (endotelceller) celler och extravaskulära (fibroblaster och glatta muskelceller) celler påverkar fibrin struktur 39, utnyttjad viskoelastiska och spektralanalys att analysera fibrin strukturer 40, och utvecklade korrelationer mellan fibrin struktur och mekaniska egenskaper med hjälp av experimentella och beräknings närmar 41 </sup>. Fokus för den aktuella studien var att formulera koagel strukturer under simulerade diabetiker och skäran cell trombos förhållanden och använda konfokalmikroskopi för undersökning av struktur och funktion av blodproppar i sjukdomstillstånd. Fibrinkoagel bildades från humant fibrinogen, humant trombin, och FXIIIa. De blodproppar lyserades med hjälp av plasmin. För att simulera diabetes villkor, ökades koncentrationen av fibrinogen inkuberas i glukoslösning för att inducera in vitro fibrinogen glykation. För att simulera sicklecellanemi koagulering förhållanden, höjdes fibrinogenkoncentrationer blandat med sickle-cell hematokrit samlats in från patienter som gjort tidigare av vår grupp 42. Dessa metoder användes för att undersöka struktur och funktioner som är involverade i fibrinkoagel bildning och fibrinolys i sjuka förhållanden, samt de mekanismer som inducerar CVD. Baserat på aktuell information om dessa sjukdomar, de glykerade fibrinkoagel strukturer var tätare med färre and mindre porer. De fibrinkoagel med röda blodkroppar från sicklecellpatienter (RBC) var också tätare och visas aggregering av RBC och agglomererade fibrin kluster. Detta är en väletablerad företeelse som har bestämts tidigare 43. Det var också en hypotes att fibrinolys takten skulle vara betydligt lägre i glykerade fibrinkoagel med och utan minskad plasmin jämfört med friska, normala fibrin. Resultaten visade att för glycerade fibrinkoagel, var signifikant olika lys ränta resultat observerades endast under förhållanden med reducerad plasmin koncentration. Denna experimentella tekniken att använda konfokalmikroskopi erbjuder betydande fördelar jämfört med andra avbildningsmetoder eftersom cellerna och proteiner förblir i sitt nativa tillstånd, vilket möjliggör upptagning av video i realtid av koagulationsaktivitet. Denna metod för syntetiskt inducerande koagulering är också billigare och mer tidseffektivt än att få patientprover och filtrera bort individuellaproteiner och enzymer. Vidare, genom att använda separerade proteiner och enzymer för att syntetisera proppar ades proppar standardiserade så att det inte fanns variabilitet mellan prover som ett resultat av andra proteiner i plasmat.
För att få meningsfulla uppgifter om strukturen på koagulering mekanismer i sjukdomstillstånd, är det viktigt att isolera de faktorer som koagulering att bestämma effekterna av proteiner och celler i dessa villkor.
