Intracoronary acetylcholine testing has been established for the assessment of epicardial coronary spasm more than 30 years ago. Recently, the focus has shifted towards the microcirculation and it has been shown that microvascular spasm can be detected using ACH-testing. This article describes the ACH-test and its implementation in daily routine.
Intracoronary acetylcholine provocation testing (ACH-test) is an established method for assessment of epicardial coronary artery spasm in the catheterization laboratory which was introduced more than 30 years ago. Due to the short half-life of acetylcholine it can only be applied directly into the coronary arteries. Several studies have demonstrated the safety and clinical usefulness of this test. However, acetylcholine testing is only rarely applied in the U.S. or Europe. Nevertheless, it has been shown that 62% of Caucasian patients with stable angina and unobstructed coronary arteries on coronary angiography suffer from coronary vasomotor disorders that can be diagnosed with acetylcholine testing. In recent years it has been appreciated that the ACH-test not only assesses the presence of epicardial spasm but that it can also be useful for the detection of coronary microvascular spam. In such cases no epicardial spasm is seen after injection of acetylcholine but ischemic ECG shifts are present together with a reproduction of the patient’s symptoms during the test. This article describes the experience with the ACH-test and its implementation in daily clinical routine.
Angina pectoris er kendetegnende for koronararteriesygdom og begrebet epikardiel stenose forårsager myokardieiskæmi og anstrengelsesudløst angina er blevet etableret i mange år. Men mange patienter med angina pectoris ikke den typiske triade af retrosternal smerte, debut under træning og lindring ved nitroglycerin eller hvile. Ofte patienter rapporterer angina pectoris i hvile eller en kombination af anstrengelses og hvile angina samt åndenød ved anstrengelse som en mulig angina tilsvarende. I 1959 Prinzmetal var den første til at introducere begrebet forbigående spasmer af kranspulsårerne forårsager angina i hvile forbundet med ST-segment elevation på elektrokardiogram (EKG), men med bevaret motion kapacitet 1. Denne hypotese blev bekræftet senere med koronar angiografi og det viste sig, at koronar spasme kan være til stede i patienter med epikardiale stenoser eller normale koronararterier 2. I 1980ies intrakoronar acetylcholin provokation test (ACH-test) til påvisning af koronar spasmer blev etableret i Japan og efterfølgende interesseret i klinisk forskning for koronar spasmer steg 3.
Men efter indførelsen af perkutan koronar intervention i 1977 og den første stent implantation i 1986 interessen for koronar vasomotoriske lidelser faldet betydeligt i hvert fald i Europa og USA. Dette kan også skyldes den invasive natur af ACH-test (på grund af den korte halveringstid af ACH det kan kun indgives i koronararterierne for vurdering af koronar spasme). Stadig, mange patienter med tegn og symptomer på myokardieiskæmi ikke have nogen relevant epikardial stenose på koronar angiografi 4,5,6. I sådanne patienter intrakoronar acetylcholin provokation test er nyttigt at opdage en klinisk relevant koronar vasomotorisk uorden og indføre passende medicinsk behandling <sup> 7.
Acetylcholin er en neurotransmitter i det parasympatiske nervesystem. Det virker via nikotinerge samt muscarinergic (mAChR) receptorer. Sidstnævnte er vigtige for vaskulær homeostase og acetylcholin binder på mAChR som en ikke-selektiv agonist. Aktivering af disse receptorer på endotel niveau fører til nitrogenoxid medieret vasodilatation henviser aktivering af mAChR på de vaskulære glatte muskelceller fører til vasokonstriktion 8. Afhængigt af integriteten af endotelet og reaktiviteten af de glatte muskelceller nettoeffekten af intrakoronar acetylcholin administration er vasodilatation eller vasokonstriktion. Den fysiologiske reaktion af koronararterier som reaktion på acetylcholin med mennesker er ikke fuldt forstået, men det er blevet rapporteret, at vasodilatation samt vasokonstriktion på op til 25% af diameteren fartøj kan være fysiologisk som vist i patienter med normale koronararterier og ingen angina pecToris 9.
Intrakoronar acetylcholin provokation test er blevet anbefalet af European Society of Cardiology retningslinjer 10 samt de japanske Circulation Society retningslinjer 11 for vurdering af epicardiale og / eller mikrovaskulære krampe hos patienter med angina pectoris og uhindret kranspulsårerne. Intrakoronar acetylcholin provokation test er blevet etableret i daglig klinisk rutine i kateterisation laboratorium vores institution i 2003. Siden 2006 en standardiseret protokol er blevet fulgt 12. ACH-test er generelt udført i alle patienter med tegn og symptomer på myokardieiskæmi endnu ingen relevante epikardial stenose (<50%) på koronar angiografi. ACH-undersøgelserne udføres umiddelbart efter diagnostisk koronar angiografi ifølge protokollen beskrevet nedenfor. Ergonovin er et andet middel, der anvendes til krampe provokation test med en anden virkningsmekanisme. detaljeretoplysninger om ergonovin test kan findes andre steder 12.
Det er muligt at gennemføre den acetylcholin test i daglig klinisk rutine i kateterisering laboratorium.
