Ce protocole présente une méthode normalisée pour cultiver des cellules VX2 dans la culture et pour créer un modèle orthotopic VX2 du cancer de l’endomètre avec des métastases rétropéritonéales de ganglion lymphatique chez les lapins. Les modèles orthotopiques de cancer de l’endomètre sont importants pour l’étude préclinique des modalités d’imagerie nouvelles pour le diagnostic des métastases de ganglion lymphatique.
Le cancer de l’endomètre est la malignité gynécologique la plus courante en Amérique du Nord et l’incidence augmente dans le monde entier. Le traitement consiste en une chirurgie avec ou sans traitement adjuvant en fonction de la participation des ganglions lymphatiques telle que déterminée par lymphadenectomy. Lymphadenectomy est une procédure morbide, qui n’a pas été montrée pour avoir un avantage thérapeutique dans beaucoup de patients, et ainsi une nouvelle méthode pour diagnostiquer des métastases de ganglion lymphatique est exigée. Pour tester de nouveaux agents d’imagerie, un modèle fiable de cancer de l’endomètre avec métastases rétropéritones des ganglions lymphatiques est nécessaire. Le modèle de cancer endomètre vX2 a été décrit fréquemment dans la littérature ; toutefois, il existe des variations importantes en ce qui concerne la méthode d’établissement du modèle. En outre, aucune étude n’a rapporté sur l’utilisation de cellules VX2 cultivées pour créer ce modèle que seules les cellules propagées in vivo ont été précédemment utilisés. Ici, nous présentons une méthode chirurgicale normalisée et une méthode de surveillance postopératoire pour l’établissement du modèle de cancer de l’endomètre VX2 et rapportons sur la première utilisation des cellules VX2 cultivées pour créer ce modèle.
Le cancer de l’endomètre, ou cancer de la muqueuse de l’utérus, est la deuxième malignité gynécologique la plus commune dans le monde et la malignité la plus commune dans les nations développées1. L’incidence du cancer de l’endomètre n’a cessé d’augmenter, augmentant de 2,3 % par an entre 2005 et 2013, avec une augmentation correspondante de 2,2 % de la mortalité1,2,3. Le diagnostic des métastases de ganglion lymphatique est primordial car la présence des ganglions lymphatiques positifs est un prédicteur négatif fort de la survie4,5,6,7et peut guider l’administration de thérapie adjuvante8,9,10,11,12,13. Les métastases des ganglions lymphatiques sont actuellement diagnostiquées en enlevant chirurgicalement le tissu lymphatique qui recense les principaux vaisseaux sanguins du bassin et de l’abdomen. Cette procédure, connue sous le nom de lymphadenectomy, est controversée en raison des données contradictoires de survie de deux grands essais14,15,16,17,18 et le connu risque de15,19,20 et morbidité postopératoire21,22,23. Comme les modalités actuelles d’imagerie non invasive n’ont pas la sensibilité et la spécificité requises pour remplacer la dissection24des ganglions lymphatiques, il y a eu une poussée pour développer de nouvelles techniques d’imagerie diagnostique. Pour tester ces nouvelles techniques dans un cadre préclinique, un modèle fiable de cancer de l’endomètre avec métastases rétropéritones des ganglions lymphatiques est nécessaire.
Le modèle de tumeur de lapin VX2 est un modèle bien établi qui a été employé intensivement pour étudier les tumeurs pleines humaines multiples d’organe25 comprenant le poumon26,la tête et le cou27,28, foie29,rein30, os31,32, cerveau33, pancréas34 et utérus35,36,37. Le modèle VX2 a été initialement développé en 1940 par Kidd et Rous38 en transplantant avec succès un virus du papillome de lapin à queue de coton découvert par Shope en 193339. Depuis lors, le modèle VX2 a été maintenu in vivo, nécessitant un passage en série dans le muscle quadriceps de lapins blancs de Nouvelle-Zélande40. Plus récemment cependant, plusieurs groupes ont réussi à cultiver des cellules VX2 in vitro40,41,42 et a démontré la tumorigénéité retenue de la lignée cellulaire cultivée31, 42,43. Les tumeurs vX2 sont histologiquement définies comme carcinomes squamous anapicieux de cellules44 et contiennent des dispositifs glandulaires qui ressemblent à l’adénocarcinome26. Les tumeurs sont caractérisées par la facilité d’implantation, la croissance rapide et l’hypervascularité44,45 et métastasé de façon fiable, le plus souvent aux ganglions lymphatiques régionaux et éloignés45. Les similitudes dans l’anatomie vasculaire et lymphatique utérine46 aussi bien que le site de croissance orthotopic assurent que le modèle métastatique du carcinome de Lapin VX2 imite celui du cancer humain d’endomètre, faisant le modèle de VX2 un modèle fiable pour étudier l’humain métastatique. En outre, les dispositifs histologiques tels que la prolifération microvasculaire anormale47, aussi bien que les similitudes immunologiques48 et génétiques49,50 entre les humains et les lapins suggèrent que la tumeur microenvironnement peut refléter celui du cancer de l’endomètre humain.
Plusieurs groupes ont fait rapport sur l’utilisation de VX2 pour créer un modèle de cancer de l’endomètre avec métastases rétropéritoéneavecles avec un taux élevé de réussite signalé36,51,52; cependant, il existe des variations significatives dans la littérature actuelle en ce qui concerne la méthode de création de modèles. Des doses de suspension cellulaire aussi bas que 4 x 105 cellules/corne utérine51 et aussi haut que 5 x 109 cellules/corne utérine37,53 ont été rapportées sans consensus standard sur la dose cellulaire VX2 requise. En outre, une série de méthodes d’inoculation ont été rapportées comprenant l’implantation micro-chirurgicale de la tumeur dans le myometrium utérin36,injection de suspension de cellules de VX237,44,52 , 53 et dans certains cas, l’ajout de la suture de corne utérine avant l’innoculation52. Enfin, aucun groupe n’a signalé l’utilisation de cellules VX2 cultivées pour créer ce modèle. Ainsi, le but de cette étude est de démontrer une méthode normalisée réussie de création de modèle de VX2 et de rapporter la première utilisation des cellules cultivées de VX2 pour créer un modèle du cancer d’endomètre avec des métastases rétropéritoéines dans un lapin.
Ici, nous avons rapporté une méthode chirurgicale normalisée pour l’établissement d’un modèle de cancer de l’endomètre VX2 et rapporté sur la première utilisation des cellules VX2 cultivées pour créer ce modèle.
