The lymphodepletive and immunomodulatory effects of chemotherapy and radiation standard of care can be leveraged to enhance the antitumor efficacy of T cell immunotherapy. We outline a method for generating EGFRvIII-specific chimeric antigen receptor (CAR) T cells and administering them in the context of glioblastoma standard of care.
Adoptive T cell immunotherapy offers a promising strategy for specifically targeting and eliminating malignant gliomas. T cells can be engineered ex vivo to express chimeric antigen receptors specific for glioma antigens (CAR T cells). The expansion and function of adoptively transferred CAR T cells can be potentiated by the lymphodepletive and tumoricidal effects of standard of care chemotherapy and radiotherapy. We describe a method for generating CAR T cells targeting EGFRvIII, a glioma-specific antigen, and evaluating their efficacy when combined with a murine model of glioblastoma standard of care. T cells are engineered by transduction with a retroviral vector containing the anti-EGFRvIII CAR gene. Tumor-bearing animals are subjected to host conditioning by a course of temozolomide and whole brain irradiation at dose regimens designed to model clinical standard of care. CAR T cells are then delivered intravenously to primed hosts. This method can be used to evaluate the antitumor efficacy of CAR T cells in the context of standard of care.
Das Glioblastom (GBM) ist die häufigste primären bösartigen Gehirntumor und ist immer tödlich. Die chirurgische Resektion in Verbindung mit nicht-spezifischen Standardtherapie Chemotherapie und Strahlentherapie nicht vollständig zu beseitigen Krebszellen, was zu einer düstere Prognose von weniger als 15 Monaten bei Patienten mit dieser Krankheit ein. Im Gegensatz dazu bietet die Immuntherapie eine genaue Vorgehensweise für die gezielt Tumorzellen angreift und somit das Potenzial hat, als eine sehr effektive Behandlungsplattform mit reduzierten Risiko von Sicherheiten Toxizität 2-4 dienen. T-Zellen entwickelt, ex vivo, um auszudrücken, chimären Antigenrezeptoren (CAR) bieten eine vielseitige Strategie zur Tumor-Immuntherapie. Die Autos werden durch Fusionieren der extrazellulären variable Region eines Antikörpers mit einem oder mehreren intrazellulären T-Zelle-Signalmolekül (en), die anstelle einer Volllängen-Haupthistokompatibilitätskomplex erzeugt (MHC) restringierten T-Zell-Rezeptor 5. Diese Art der antikörperähnlichen Antigens Sie AnAuf ermöglicht reaktiven Antigen-spezifische T-Zellen zu erkennen und zu reagieren, um Tumorantigene in Abwesenheit von MHC und kann für eine praktisch unendliche Antigenrepertoire angepasst werden.
CAR-T-Zellen gegenüber einer Vielzahl von Tumorantigenen konstruiert wurden vorklinische Wirksamkeit und hervorragende Versprechen in der Klinik 6-9 gezeigt. Insbesondere im Zusammenhang mit der GBM eine CAR-T-Zell-Targeting-Plattform den epidermalen Wachstumsfaktor-Rezeptorvariante III (EGFRvIII), ausgedrückt ein Tumor-spezifische Mutation an der Zelloberfläche 10, wurde gezeigt, dass das Überleben in Gliom-tragenden Mäusen 11 verlängern. Trotz ihrer Vielseitigkeit jedoch der klinische Nutzen von CAR adoptive Therapie nicht vollständig realisiert worden ist, was zum Teil an Tumor-assoziierte Immunsuppression und Immunumgehung 12-16 sowie Herausforderungen bei der Aufrechterhaltung Antigen-spezifischen T-Zellen in vivo. Aufbauend Standardtherapie (SOC) mit Immuntherapie kann potenziell zu überwinden mehrere dieser lImitationen, was zu einer erhöhten Wirksamkeit sowohl in der präklinischen und klinischen Einstellung.
SOC für post-Resektion GBM besteht aus hochdosiertem Temozolomid (TMZ), einem DNA-Alkylierungsmittel 17 und gesamte Gehirn Bestrahlung (WBI) 1. Diese Behandlungen gelten als mit Tumorvakzinen über Hochregulierung von Tumor-MHC-Expression von 18 bis 20 und das Vergießen von Antigenen durch toten Tumorzellen 17,19,21,22 Synergie. Tatsächlich ist die Zugabe von TMZ 20,23 oder 18,24 WBI führt zu verbesserten Antitumorwirksamkeit von immunbasierten Behandlungen in der präklinischen Einstellung. Ferner, wie viele nicht-spezifischen zytotoxischen Chemotherapeutika, TMZ ist bekannt, dass eine systemische Lymphopenie 25,26, die als Mittel zur Hostanlage für adoptive Therapie Plattformen 27-29 genutzt werden können verursachen. TMZ vermittelte lymphodepletion wurde gezeigt, dass die Häufigkeit und die Funktion von Antigen-spezifischen T-Zellen zu verstärken, was zu einer erhöhten Wirksamkeit eines ADOPtive Therapie Plattform gegen intrakranielle Tumore 30. Im Rahmen der CAR-Therapie dient lymphodepletion als Mittel zur Wirtsanlage sowohl durch die Reduzierung der Anzahl der endogenen Suppressor-T-Zellen 31 und 32 homöostatischen Proliferation induzierende über Einschränkungen des Wettbewerbs für Zytokine 33 und somit eine Verbesserung Antitumoraktivität 11,34. Angesichts der synergistische Beziehung zwischen GBM SOC und Immuntherapie-Plattformen, die Bewertung neuer Therapien und Adoptivimpfstoffplattformen im Rahmen der SOC ist von entscheidender Bedeutung für die Erstellung aussagekräftige Rückschlüsse auf die Wirksamkeit.
In diesem Protokoll beschreiben wir ein Verfahren für die Erzeugung und die intravenöse Verabreichung von murinen EGFRvIII spezifische CAR T-Zellen zusammen mit TMZ und WBI in Mäusen EGFRvIII-positive intrakraniellen Tumoren (siehe Abbildung 1 für die Behandlung Timeline). Kurz gesagt, werden CAR T-Zellen ex vivo durch retrovirale Transduktion gemacht. Menschliche embryonale Nierenzellen (HEK) 293T-Zellen werden mit einem DNA / Lipid-Komplex (enthaltend die CAR-Vektor und pCL-Eco-Plasmiden), um Viren, die dann verwendet wird, um aktivierten murinen Splenocyten, die geerntet werden und parallel kultivierten transduzieren erzeugen transfiziert. Im Verlauf der CAR Generierung werden murine Hosts Lager EGFRvIII-positiven intrakraniellen Tumoren in Dosierungen entsprechend der klinischen SOC verabreicht fraktionierte ganzen Gehirn Röntgenbestrahlung und systemischen TMZ Behandlung. CAR T-Zellen werden dann intravenös lymphodepleted Gastgeber geliefert.
Folgende Vorgehensweise wird in sieben getrennten Phasen beschrieben: (1) Die Verabreichung von Temozolomid an tumortragenden Mäusen, (2) Ganzhirnbestrahlung von tumortragenden Mäusen, (3) Die Transfektion, (4) Splenektomie und T-Zell-Vorbereitung, (5 ) Transduction, (6) CAR T-Zell-Kultur und Ernte, und (7) CAR T-Zell-Verwaltung auf tumortragenden Mäusen. Diese Phasen bestehen aus mehreren Schritten, die 6-7 Tage dauern und gleichzeitig durchgeführt werden.
Die hier beschriebene Behandlung Timeline wurde konzipiert, um die klinische Behandlungsstandard modellieren und zu nutzen, ihre Auswirkungen für CAR Adoptiv Therapie. CAR T Zelldosen, TMZ Schemata und Strahlentherapie Verabreichung kann verändert werden, um in vivo T-Zell-Aktivität, lymphodepletion und Tumortötung zu erhöhen.
